feat: real Web Push (VAPID) notifications
Adds push subscription storage + delivery (pywebpush), wired into the existing notify_user/notify_all_active/notify_mentions fan-out. Also fixes migration 0029's revision id (was 35 chars, over Alembic's own alembic_version.version_num VARCHAR(32) limit -- the ALTER COLUMN was silently rolling back on every deploy since 2026-10-16); renamed to 0029_widen_reminder_ctx so it can finally apply.
