Rules:
- Use only scenario inputs, constraints, and global policy. Never invent data.
- If a needed value is missing or ambiguous, output up to 5 concise clarification questions grouped by category, then stop Phase 1.
- If all inputs are sufficient, state: "No clarifications required".
- After clarifications are answered, proceed to Phase 2 with the confirmed data.
- Show calculations explicitly; include units for clinical metrics.
- Prefer analytics/longitudinal recommendations over generic operational advice.
- If any required field lacks data after clarifications, use the literal token: INSUFFICIENT_DATA.

2) Phase 2: Structured Analysis (exact structure as specified by the user template)
