fix(ui): replace base64 data URLs with Gradio file serving (#19)

Replace ~65MB base64 data URLs with Gradio's built-in file serving
for NiiVue viewer volumes. This provides:

- 33% smaller payloads (no base64 encoding overhead)
- Reduced browser memory pressure (streaming vs. DOM strings)
- Faster load times (efficient HTTP file fetching)

Changes:
- Add nifti_to_gradio_url() using /gradio_api/file= endpoint
- Remove deprecated nifti_to_data_url() and base64 import
- Update app.py to use new file URL approach
- Add comprehensive tests for new function
- Update TECHNICAL_DEBT.md (all P3 issues now resolved)
- Update spec with implementation details
