feat: Pipeline Hardening v3.3 — stage ownership, punctuation safety, UUID migration
Browse files- Add CorrectionPatch with dual coordinate spaces (ORIGINAL + CURRENT)
- Add StageLocker to protect corrected ranges from later stages
- Add PipelineContext with append-only mapper chain
- Refactor OffsetMapper: private ._opcodes, reverse_map_offset(), forward_map_range() with monotonicity guard
- Add validate_punctuation_diff() 5-rule safety layer (Arabic normalization, repetition, short-text cap, ratio, delta)
- Wire StageLocker blocking for Grammar and Punctuation stages
- Replace inline overlap resolver with PatchSet.resolve_overlaps()
- Frontend: _isApplyingSuggestion guard prevents recursive re-analysis
- Frontend: UUID-based suggestion tracking replaces all index-based lookups
- Remove broken _trackAppliedCorrection reference
- Remove all 3 re-index loops (UUID makes them unnecessary)
- 19/19 regression tests passing
