Under the existing text there needs to be a button that says send me my report, when that button is clicked it needs to reveal 3. fields and a submit button, The fields are 1. name 2. email 3. phone (this also needs an international dialling code dropped down at the beginning of the phone field that is set by default to plus 1 IE the USA) - Follow Up Deployment
