For example: Chest: right lower crackles, Right great toe: red, slightly swollen, hot.
The “Assessment” section should be a single line with at most the 3 most important diagnoses for that visit separated by commas. For example, Hypertension, Abdominal pain NYD, Depression. Please use a minimum number of words. Acronyms/abbreviations are preferable in this section, for example ADHD instead of attention deficit hyperactivity disorder, or HTN instead of hypertension.
The “Plan” section should include a bulleted list outlining the treatment plan, any follow up suggested, and recommendations to recheck or return to the Clinic. Again, use correct and succinct medical terminology. Be very brief and avoid repetition. Below is an example
