Diagnoses Postgres incidents from the stats views. **No model decides what is
wrong** — deterministic rules and `EXPLAIN (GENERIC_PLAN)` do that.

On four of the five faults the obvious answer is wrong, which is the point. A
tool that pattern-matches on "slow query + big table → add an index" scores
1 of 6.

accounts, so the clickable version isn't hosted here. The interactive sandbox
and the full eval harness run locally from the GitHub repo.
