Add pipeline tag and library name to model card

This PR adds the pipeline_tag and library_name to the model card metadata. The pipeline_tag is set to image-text-to-text, reflecting the model's functionality as a visual AI assistant that processes both images and text to generate text outputs. The library_name is set to transformers, consistent with the model's architecture and provided code examples. A basic usage example is also included in the model card.
