Add model card and metadata

This PR adds a comprehensive model card for EvolveR, linking it to the paper EvolveR: Self-Evolving LLM Agents through an Experience-Driven Lifecycle and its official GitHub repository. It also adds relevant metadata tags for library_name and pipeline_tag to ensure the model is correctly categorized and discoverable on the Hub.
