README: the Phase 3 adapter table now lists seed{42,123} for the 20k, cond-3 and cond-5 folders, marks the adapters still training (cond-2 zh/ur 20k, cond-5 es), adds a short note on the seed-123 batch and the cond-3 folder name, and updates the provenance table's tier/seed summary. MANIFEST: adds rows for the five seed-123 adapters now on main (cond-3 under its native-code folder name, which PR #17 applies). No weights or configs change.
