Improve model card: correct GitHub link, add project page, and fix HTML

This PR addresses several improvements to the model card:

  1. Corrects the introductory GitHub repository link to https://github.com/OpenBMB/MiniCPM-V to accurately reflect the model's codebase, as the previous link pointed to MiniCPM-o.
  2. Adds an explicit "Project Page" link using the documentation site found in the GitHub README (https://minicpm-o.readthedocs.io/en/latest/index.html), enhancing discoverability.
  3. Removes a malformed </a> HTML tag at the end of the introductory links.

All other metadata and content aspects are already well-documented.

I can accept formatting changes, but this is a V4.5 repository, so O2.6 content shouldn't be included.
If you'd like to merge this change, you can modify only a limited portion.
