Teaching LLMs Brazilian Healthcare: Injecting Knowledge from Official Clinical Guidelines

Brazil's Unified Health System (SUS) relies on official clinical guidelines that define diagnostic criteria, treatments, dosages, and monitoring procedures for over 200 million citizens. Yet current LLMs perform poorly on this guideline-specific knowledge, and no benchmark evaluates clinical recall grounded in Brazilian Portuguese protocols. We address this gap by adapting Qwen2.5-14B-Instruct to the Brazilian clinical domain. From 178 official guidelines (~5.4M tokens), we generate ~70M tokens of synthetic data in three formats -- rephrases, wiki-style articles, and question-answer pairs -- using four generator LLMs. We then apply continual pre-training followed by Group Relative Policy Optimization (GRPO). We introduce HealthBench-BR, with 1,780 balanced true/false clinical assertions, and PCDT-QA, with 890 open-ended clinical questions scored by an LLM judge. Our best model achieves 83.9% on HealthBench-BR and 85.4% on PCDT-QA, outperforming GPT-5.2, Claude Sonnet 4.6, Gemini 3.1 Pro, and Google AI Overview's web-grounded RAG despite having only 14B parameters. Ablations show that generator diversity and reinforcement learning are critical to these gains. We release all datasets, benchmarks, and model weights to support reproducible clinical NLP research for Brazilian Portuguese. SafeWatch: An Efficient Safety-Policy Following Video Guardrail Model with Transparent Explanations

With the rise of generative AI and rapid growth of high-quality video generation, video guardrails have become more crucial than ever to ensure safety and security across platforms. Current video guardrails, however, are either overly simplistic, relying on pure classification models trained on simple policies with limited unsafe categories, which lack detailed explanations, or prompting multimodal large language models (MLLMs) with long safety guidelines, which are inefficient and impractical for guardrailing real-world content. To bridge this gap, we propose SafeWatch, an efficient MLLM-based video guardrail model designed to follow customized safety policies and provide multi-label video guardrail outputs with content-specific explanations in a zero-shot manner. In particular, unlike traditional MLLM-based guardrails that encode all safety policies autoregressively, causing inefficiency and bias, SafeWatch uniquely encodes each policy chunk in parallel and eliminates their position bias such that all policies are attended simultaneously with equal importance. In addition, to improve efficiency and accuracy, SafeWatch incorporates a policy-aware visual token pruning algorithm that adaptively selects the most relevant video tokens for each policy, discarding noisy or irrelevant information. This allows for more focused, policy-compliant guardrail with significantly reduced computational overhead. Considering the limitations of existing video guardrail benchmarks, we propose SafeWatch-Bench, a large-scale video guardrail benchmark comprising over 2M videos spanning six safety categories which covers over 30 tasks to ensure a comprehensive coverage of all potential safety scenarios. SafeWatch outperforms SOTA by 28.2% on SafeWatch-Bench, 13.6% on benchmarks, cuts costs by 10%, and delivers top-tier explanations validated by LLM and human reviews.

Forging a Developed India: Growth Imperatives, Fiscal Sustainability, and Multilateral Partnerships for Viksit Bharat 2047

This paper examines the fiscal and macroeconomic strategies essential for transition of India to a high income economy by 2047, aligning with the vision of Viksit Bharat. A sustainable annual GDP growth rate of 7 to 8 percent is projected as necessary to achieve this milestone while maintaining fiscal prudence through a targeted deficit threshold below 3.5 percent of GDP. The study underscores the role of disciplined fiscal management in financing critical public investments in infrastructure, human capital development and technological innovation. Given constraints on domestic resource mobilization, the paper highlights the importance of multilateral financial institutions, including the World Bank, IMF and ADB, in expanding fiscal space in India through concessional financing, technical cooperation, and risk sharing mechanisms. Using econometric modeling and scenario analysis, the research identifies key policy interventions in infrastructure, healthcare, education and sustainable energy that can maximize growth while ensuring fiscal sustainability. Policy recommendations include enhancing tax buoyancy, rationalizing expenditure, optimizing public private partnerships and strengthening fiscal responsibility frameworks. The findings suggest that a calibrated approach to growth, prudent fiscal management and strategic international collaborations are critical to achieving long term economic aspirations of India.

Ensuring Safe and High-Quality Outputs: A Guideline Library Approach for Language Models

Large Language Models (LLMs) exhibit impressive capabilities but also present risks such as biased content generation and privacy issues. One of the current alignment techniques includes principle-driven integration, but it faces challenges arising from the imprecision of manually crafted rules and inadequate risk perception in models without safety training. To address these, we introduce Guide-Align, a two-stage approach. Initially, a safety-trained model identifies potential risks and formulates specific guidelines for various inputs, establishing a comprehensive library of guidelines and a model for input-guidelines retrieval. Subsequently, the retrieval model correlates new inputs with relevant guidelines, which guide LLMs in response generation to ensure safe and high-quality outputs, thereby aligning with human values. An additional optional stage involves fine-tuning a model with well-aligned datasets generated through the process implemented in the second stage. Our method customizes guidelines to accommodate diverse inputs, thereby enhancing the fine-grainedness and comprehensiveness of the guideline library. Furthermore, it incorporates safety expertise from a safety-trained LLM through a lightweight retrieval model. We evaluate our approach on three benchmarks, demonstrating significant improvements in LLM security and quality. Notably, our fine-tuned model, Labrador, even at 13 billion parameters, outperforms GPT-3.5-turbo and surpasses GPT-4 in alignment capabilities.

The Ethics of ChatGPT in Medicine and Healthcare: A Systematic Review on Large Language Models (LLMs)

With the introduction of ChatGPT, Large Language Models (LLMs) have received enormous attention in healthcare. Despite their potential benefits, researchers have underscored various ethical implications. While individual instances have drawn much attention, the debate lacks a systematic overview of practical applications currently researched and ethical issues connected to them. Against this background, this work aims to map the ethical landscape surrounding the current stage of deployment of LLMs in medicine and healthcare. Electronic databases and preprint servers were queried using a comprehensive search strategy. Studies were screened and extracted following a modified rapid review approach. Methodological quality was assessed using a hybrid approach. For 53 records, a meta-aggregative synthesis was performed. Four fields of applications emerged and testify to a vivid exploration phase. Advantages of using LLMs are attributed to their capacity in data analysis, personalized information provisioning, support in decision-making, mitigating information loss and enhancing information accessibility. However, we also identifies recurrent ethical concerns connected to fairness, bias, non-maleficence, transparency, and privacy. A distinctive concern is the tendency to produce harmful misinformation or convincingly but inaccurate content. A recurrent plea for ethical guidance and human oversight is evident. Given the variety of use cases, it is suggested that the ethical guidance debate be reframed to focus on defining what constitutes acceptable human oversight across the spectrum of applications. This involves considering diverse settings, varying potentials for harm, and different acceptable thresholds for performance and certainty in healthcare. In addition, a critical inquiry is necessary to determine the extent to which the current experimental use of LLMs is necessary and justified.

Jurisdiction as Structural Barrier: How Privacy Policy Organization May Reduce Visibility of Substantive Disclosures

Privacy policies are supposed to provide notice. But what if substantive information appears only where users skip it? We identify a structural pattern we call jurisdiction-siloed disclosure: information about data practices appearing in specific, actionable form only within regional compliance sections labeled "California Residents" or "EU/UK Users," while general sections use vague or qualified language for the same practices. Our audit of 123 major companies identifies 282 potential instances across 77 companies (62.6% of this purposive sample). A conservative estimate restricted to practice categories validated against OPP-115 human annotations finds 138 instances across 54 companies (44%); post-2018 categories central to our findings await independent validation. If users skip jurisdiction-labeled sections as information foraging theory predicts, users outside regulated jurisdictions would receive less specific information about practices affecting them--a transparency failure operating through document architecture rather than omission. We propose universal substantive disclosure: practices affecting all users should appear in the main policy body, with regional sections containing only procedural rights information. This standard finds support in analogous disclosure regimes (securities, truth-in-lending, nutritional labeling) where material information must reach all affected parties. Regulators could operationalize this through the FTC's "clear and conspicuous" standard and GDPR transparency principles. This work is hypothesis-generating: we establish that the structural pattern exists and ground the transparency concern in behavioral theory, but direct measurement of jurisdiction-specific section skipping remains the critical validation priority. Challenges and Considerations in Annotating Legal Data: A Comprehensive Overview

The process of annotating data within the legal sector is filled with distinct challenges that differ from other fields, primarily due to the inherent complexities of legal language and documentation. The initial task usually involves selecting an appropriate raw dataset that captures the intricate aspects of legal texts. Following this, extracting text becomes a complicated task, as legal documents often have complex structures, footnotes, references, and unique terminology. The importance of data cleaning is magnified in this context, ensuring that redundant information is eliminated while maintaining crucial legal details and context. Creating comprehensive yet straightforward annotation guidelines is imperative, as these guidelines serve as the road map for maintaining uniformity and addressing the subtle nuances of legal terminology. Another critical aspect is the involvement of legal professionals in the annotation process. Their expertise is valuable in ensuring that the data not only remains contextually accurate but also adheres to prevailing legal standards and interpretations. This paper provides an expanded view of these challenges and aims to offer a foundational understanding and guidance for researchers and professionals engaged in legal data annotation projects. In addition, we provide links to our created and fine-tuned datasets and language models. These resources are outcomes of our discussed projects and solutions to challenges faced while working on them.

Identifying Climate Targets in National Laws and Policies using Machine Learning

Quantified policy targets are a fundamental element of climate policy, typically characterised by domain-specific and technical language. Current methods for curating comprehensive views of global climate policy targets entail significant manual effort. At present there are few scalable methods for extracting climate targets from national laws or policies, which limits policymakers' and researchers' ability to (1) assess private and public sector alignment with global goals and (2) inform policy decisions. In this paper we present an approach for extracting mentions of climate targets from national laws and policies. We create an expert-annotated dataset identifying three categories of target ('Net Zero', 'Reduction' and 'Other' (e.g. renewable energy targets)) and train a classifier to reliably identify them in text. We investigate bias and equity impacts related to our model and identify specific years and country names as problematic features. Finally, we investigate the characteristics of the dataset produced by running this classifier on the Climate Policy Radar (CPR) dataset of global national climate laws and policies and UNFCCC submissions, highlighting the potential of automated and scalable data collection for existing climate policy databases and supporting further research. Our work represents a significant upgrade in the accessibility of these key climate policy elements for policymakers and researchers. Awareness in Practice: Tensions in Access to Sensitive Attribute Data for Antidiscrimination

Organizations cannot address demographic disparities that they cannot see. Recent research on machine learning and fairness has emphasized that awareness of sensitive attributes, such as race and sex, is critical to the development of interventions. However, on the ground, the existence of these data cannot be taken for granted. This paper uses the domains of employment, credit, and healthcare in the United States to surface conditions that have shaped the availability of sensitive attribute data. For each domain, we describe how and when private companies collect or infer sensitive attribute data for antidiscrimination purposes. An inconsistent story emerges: Some companies are required by law to collect sensitive attribute data, while others are prohibited from doing so. Still others, in the absence of legal mandates, have determined that collection and imputation of these data are appropriate to address disparities. This story has important implications for fairness research and its future applications. If companies that mediate access to life opportunities are unable or hesitant to collect or infer sensitive attribute data, then proposed techniques to detect and mitigate bias in machine learning models might never be implemented outside the lab. We conclude that today's legal requirements and corporate practices, while highly inconsistent across domains, offer lessons for how to approach the collection and inference of sensitive data in appropriate circumstances. We urge stakeholders, including machine learning practitioners, to actively help chart a path forward that takes both policy goals and technical needs into account.

Reflexive Behaviour: How publication pressure affects research quality in Astronomy

Reflexive metrics is a branch of science studies which explores how the demand for accountability and performance measurement in science has shaped the research culture in recent decades. Hypercompetition and publication pressure are part of this neoliberal culture. How do scientists respond to these pressures? Studies on research integrity and organizational culture suggest that people who feel treated unfairly by their institution are more likely to engage in deviant behaviour, such as scientific misconduct. By building up on reflexive metrics, combined with studies on the influence of organisational culture on research integrity, this study reflects on the research behaviour of astronomers: 1) To what extent is research (mis-)behaviour reflexive, i.e. dependent on perceptions of publication pressure and distributive & organisational justice? 2) What impact does scientific misconduct have on research quality? In order to perform this reflection, we conducted a comprehensive survey of academic and non-academic astronomers worldwide and received 3,509 responses. We found that publication pressure explains 19% of the variance in occurrence of misconduct and between 7 and 13% of the variance of the perception of distributive & organisational justice as well as overcommitment to work. Our results on the perceived impact of scientific misconduct on research quality show that the epistemic harm of questionable research practices should not be underestimated. This suggests there is a need for a policy change. In particular, lesser attention to metrics (such as publication rate) in the allocation of grants, telescope time and institutional rewards would foster better scientific conduct and hence research quality.

Testing Hateful Speeches against Policies

In the recent years, many software systems have adopted AI techniques, especially deep learning techniques. Due to their black-box nature, AI-based systems brought challenges to traceability, because AI system behaviors are based on models and data, whereas the requirements or policies are rules in the form of natural or programming language. To the best of our knowledge, there is a limited amount of studies on how AI and deep neural network-based systems behave against rule-based requirements/policies. This experience paper examines deep neural network behaviors against rule-based requirements described in natural language policies. In particular, we focus on a case study to check AI-based content moderation software against content moderation policies. First, using crowdsourcing, we collect natural language test cases which match each moderation policy, we name this dataset HateModerate; second, using the test cases in HateModerate, we test the failure rates of state-of-the-art hate speech detection software, and we find that these models have high failure rates for certain policies; finally, since manual labeling is costly, we further proposed an automated approach to augument HateModerate by finetuning OpenAI's large language models to automatically match new examples to policies. Foundation Models and Fair Use

Existing foundation models are trained on copyrighted material. Deploying these models can pose both legal and ethical risks when data creators fail to receive appropriate attribution or compensation. In the United States and several other countries, copyrighted content may be used to build foundation models without incurring liability due to the fair use doctrine. However, there is a caveat: If the model produces output that is similar to copyrighted data, particularly in scenarios that affect the market of that data, fair use may no longer apply to the output of the model. In this work, we emphasize that fair use is not guaranteed, and additional work may be necessary to keep model development and deployment squarely in the realm of fair use. First, we survey the potential risks of developing and deploying foundation models based on copyrighted content. We review relevant U.S. case law, drawing parallels to existing and potential applications for generating text, source code, and visual art. Experiments confirm that popular foundation models can generate content considerably similar to copyrighted material. Second, we discuss technical mitigations that can help foundation models stay in line with fair use. We argue that more research is needed to align mitigation strategies with the current state of the law. Lastly, we suggest that the law and technical mitigations should co-evolve. For example, coupled with other policy mechanisms, the law could more explicitly consider safe harbors when strong technical tools are used to mitigate infringement harms. This co-evolution may help strike a balance between intellectual property and innovation, which speaks to the original goal of fair use. But we emphasize that the strategies we describe here are not a panacea and more work is needed to develop policies that address the potential harms of foundation models.

GUIDE: A Guideline-Guided Dataset for Instructional Video Comprehension

There are substantial instructional videos on the Internet, which provide us tutorials for completing various tasks. Existing instructional video datasets only focus on specific steps at the video level, lacking experiential guidelines at the task level, which can lead to beginners struggling to learn new tasks due to the lack of relevant experience. Moreover, the specific steps without guidelines are trivial and unsystematic, making it difficult to provide a clear tutorial. To address these problems, we present the GUIDE (Guideline-Guided) dataset, which contains 3.5K videos of 560 instructional tasks in 8 domains related to our daily life. Specifically, we annotate each instructional task with a guideline, representing a common pattern shared by all task-related videos. On this basis, we annotate systematic specific steps, including their associated guideline steps, specific step descriptions and timestamps. Our proposed benchmark consists of three sub-tasks to evaluate comprehension ability of models: (1) Step Captioning: models have to generate captions for specific steps from videos. (2) Guideline Summarization: models have to mine the common pattern in task-related videos and summarize a guideline from them. (3) Guideline-Guided Captioning: models have to generate captions for specific steps under the guide of guideline. We evaluate plenty of foundation models with GUIDE and perform in-depth analysis. Given the diversity and practicality of GUIDE, we believe that it can be used as a better benchmark for instructional video comprehension.

Harness-1: Reinforcement Learning for Search Agents with State-Externalizing Harnesses

Search agents are often trained as policies over growing transcripts: the model must decide how to search while also remembering what it has seen, which evidence is useful, which constraints remain open, and which claims have actually been checked. We argue that this formulation puts too much routine state management inside the policy: reinforcement learning is forced to optimize both semantic search decisions and recoverable bookkeeping that the environment can maintain more reliably. We introduce Harness-1, a 20B search agent (retrieval subagent) trained with reinforcement learning inside a stateful search harness. The harness maintains environment-side working memory, including a candidate pool, an importance-tagged curated set, compact evidence links, verification records, compressed and deduplicated observations, and budget-aware context rendering. The policy retains the semantic decisions: what to search, which documents to keep or discard, what to verify, and when to stop. Across eight retrieval benchmarks spanning web, finance, patents, and multi-hop QA, Harness-1 achieves 0.730 average curated recall, outperforming the next strongest open search subagent by +11.4 points and remaining competitive with much larger frontier-model searchers. Its gains are especially strong on held-out transfer benchmarks, suggesting that reinforcement learning over explicit search state can produce retrieval behaviors that generalize beyond the training domains. The POLUSA Dataset: 0.9M Political News Articles Balanced by Time and Outlet Popularity

News articles covering policy issues are an essential source of information in the social sciences and are also frequently used for other use cases, e.g., to train NLP language models. To derive meaningful insights from the analysis of news, large datasets are required that represent real-world distributions, e.g., with respect to the contained outlets' popularity, topically, or across time. Information on the political leanings of media publishers is often needed, e.g., to study differences in news reporting across the political spectrum, which is one of the prime use cases in the social sciences when studying media bias and related societal issues. Concerning these requirements, existing datasets have major flaws, resulting in redundant and cumbersome effort in the research community for dataset creation. To fill this gap, we present POLUSA, a dataset that represents the online media landscape as perceived by an average US news consumer. The dataset contains 0.9M articles covering policy topics published between Jan. 2017 and Aug. 2019 by 18 news outlets representing the political spectrum. Each outlet is labeled by its political leaning, which we derive using a systematic aggregation of eight data sources. The news dataset is balanced with respect to publication date and outlet popularity. POLUSA enables studying a variety of subjects, e.g., media effects and political partisanship. Due to its size, the dataset allows to utilize data-intense deep learning methods.

Building Human Values into Recommender Systems: An Interdisciplinary Synthesis

Recommender systems are the algorithms which select, filter, and personalize content across many of the worlds largest platforms and apps. As such, their positive and negative effects on individuals and on societies have been extensively theorized and studied. Our overarching question is how to ensure that recommender systems enact the values of the individuals and societies that they serve. Addressing this question in a principled fashion requires technical knowledge of recommender design and operation, and also critically depends on insights from diverse fields including social science, ethics, economics, psychology, policy and law. This paper is a multidisciplinary effort to synthesize theory and practice from different perspectives, with the goal of providing a shared language, articulating current design approaches, and identifying open problems. It is not a comprehensive survey of this large space, but a set of highlights identified by our diverse author cohort. We collect a set of values that seem most relevant to recommender systems operating across different domains, then examine them from the perspectives of current industry practice, measurement, product design, and policy approaches. Important open problems include multi-stakeholder processes for defining values and resolving trade-offs, better values-driven measurements, recommender controls that people use, non-behavioral algorithmic feedback, optimization for long-term outcomes, causal inference of recommender effects, academic-industry research collaborations, and interdisciplinary policy-making.

Inteligencia Artificial jurídica y el desafío de la veracidad: análisis de alucinaciones, optimización de RAG y principios para una integración responsable

This technical report analyzes the challenge of "hallucinations" (false information) in LLMs applied to law. It examines their causes, manifestations, and the effectiveness of the RAG mitigation strategy, highlighting its limitations and proposing holistic optimizations. The paper explores the ethical and regulatory implications, emphasizing human oversight as an irreplaceable role. It concludes that the solution lies not in incrementally improving generative models, but in adopting a "consultative" AI paradigm that prioritizes veracity and traceability, acting as a tool to amplify, not replace, professional judgment. -- Este informe t\'ecnico analiza el desaf\'io de las "alucinaciones" (informaci\'on falsa) en los LLMs aplicados al derecho. Se examinan sus causas, manifestaciones y la efectividad de la estrategia de mitigaci\'on RAG, exponiendo sus limitaciones y proponiendo optimizaciones hol\'isticas. Se exploran las implicaciones \'eticas y regulatorias, enfatizando la supervisi\'on humana como un rol insustituible. El documento concluye que la soluci\'on no reside en mejorar incrementalmente los modelos generativos, sino en adoptar un paradigma de IA "consultiva" que priorice la veracidad y la trazabilidad, actuando como una herramienta para amplificar, y no sustituir, el juicio profesional.

Examining the Impact of Income Inequality and Gender on School Completion in Malaysia: A Machine Learning Approach Utilizing Malaysia's Public Sector Open Data

This study examines the relationship between income inequality, gender, and school completion rates in Malaysia using machine learning techniques. The dataset utilized is from the Malaysia's Public Sector Open Data Portal, covering the period 2016-2022. The analysis employs various machine learning techniques, including K-means clustering, ARIMA modeling, Random Forest regression, and Prophet for time series forecasting. These models are used to identify patterns, trends, and anomalies in the data, and to predict future school completion rates. Key findings reveal significant disparities in school completion rates across states, genders, and income levels. The analysis also identifies clusters of states with similar completion rates, suggesting potential regional factors influencing educational outcomes. Furthermore, time series forecasting models accurately predict future completion rates, highlighting the importance of ongoing monitoring and intervention strategies. The study concludes with recommendations for policymakers and educators to address the observed disparities and improve school completion rates in Malaysia. These recommendations include targeted interventions for specific states and demographic groups, investment in early childhood education, and addressing the impact of income inequality on educational opportunities. The findings of this study contribute to the understanding of the factors influencing school completion in Malaysia and provide valuable insights for policymakers and educators to develop effective strategies to improve educational outcomes.

How life-table right-censoring affected the Brazilian Social Security Factor: an application of the gamma-Gompertz-Makeham model

Automatic Adjustment Mechanisms (AAM) are legal instruments that help social security systems respond to demographic and economic changes. In Brazil, the Social Security Factor (SSF) was introduced in the late 1990s as an AAM to link retirement benefits to life expectancy at the retirement age, with the hope of promoting contributory justice and discouraging early retirement. Recent research has highlighted the limitations of right-censored life tables, such as those used in Brazil. It has recommended using the gamma-Gompertz-Makeham (GGM) model to estimate adult and old-age mortality. This study investigated the impact of right-censoring on the SSF by comparing the official SSF and other social security metrics with a counterfactual scenario computed based on fitted GGM models. The results indicate that from 2004 to 2012, official life tables may have negatively impacted retirees' income, particularly for those who delayed their retirement. Furthermore, the GGM-fitted models' life expectancies had more stable paths over time, which could have helped with long-term planning. This study's findings are significant for policymakers as they highlight the importance of using appropriate mortality metrics in AAMs to ensure accurate retirement benefit payments. They also underscore the need to consider the potential impacts of seemingly innocuous hypotheses on public action outcomes.
