ClawKeeper: Comprehensive Safety Protection for OpenClaw Agents Through Skills, Plugins, and Watchers

OpenClaw has rapidly established itself as a leading open-source autonomous agent runtime, offering powerful capabilities including tool integration, local file access, and shell command execution. However, these broad operational privileges introduce critical security vulnerabilities, transforming model errors into tangible system-level threats such as sensitive data leakage, privilege escalation, and malicious third-party skill execution. Existing security measures for the OpenClaw ecosystem remain highly fragmented, addressing only isolated stages of the agent lifecycle rather than providing holistic protection. To bridge this gap, we present ClawKeeper, a real-time security framework that integrates multi-dimensional protection mechanisms across three complementary architectural layers. (1) Skill-based protection operates at the instruction level, injecting structured security policies directly into the agent context to enforce environment-specific constraints and cross-platform boundaries. (2) Plugin-based protection serves as an internal runtime enforcer, providing configuration hardening, proactive threat detection, and continuous behavioral monitoring throughout the execution pipeline. (3) Watcher-based protection introduces a novel, decoupled system-level security middleware that continuously verifies agent state evolution. It enables real-time execution intervention without coupling to the agent's internal logic, supporting operations such as halting high-risk actions or enforcing human confirmation. We argue that this Watcher paradigm holds strong potential to serve as a foundational building block for securing next-generation autonomous agent systems. Extensive qualitative and quantitative evaluations demonstrate the effectiveness and robustness of ClawKeeper across diverse threat scenarios. We release our code.

FASIONAD++ : Integrating High-Level Instruction and Information Bottleneck in FAt-Slow fusION Systems for Enhanced Safety in Autonomous Driving with Adaptive Feedback

Ensuring safe, comfortable, and efficient planning is crucial for autonomous driving systems. While end-to-end models trained on large datasets perform well in standard driving scenarios, they struggle with complex low-frequency events. Recent Large Language Models (LLMs) and Vision Language Models (VLMs) advancements offer enhanced reasoning but suffer from computational inefficiency. Inspired by the dual-process cognitive model "Thinking, Fast and Slow", we propose FASIONAD -- a novel dual-system framework that synergizes a fast end-to-end planner with a VLM-based reasoning module. The fast system leverages end-to-end learning to achieve real-time trajectory generation in common scenarios, while the slow system activates through uncertainty estimation to perform contextual analysis and complex scenario resolution. Our architecture introduces three key innovations: (1) A dynamic switching mechanism enabling slow system intervention based on real-time uncertainty assessment; (2) An information bottleneck with high-level plan feedback that optimizes the slow system's guidance capability; (3) A bidirectional knowledge exchange where visual prompts enhance the slow system's reasoning while its feedback refines the fast planner's decision-making. To strengthen VLM reasoning, we develop a question-answering mechanism coupled with reward-instruct training strategy. In open-loop experiments, FASIONAD achieves a 6.7% reduction in average L2 trajectory error and 28.1% lower collision rate.

Bresa: Bio-inspired Reflexive Safe Reinforcement Learning for Contact-Rich Robotic Tasks

Ensuring safety in reinforcement learning (RL)-based robotic systems is a critical challenge, especially in contact-rich tasks within unstructured environments. While the state-of-the-art safe RL approaches mitigate risks through safe exploration or high-level recovery mechanisms, they often overlook low-level execution safety, where reflexive responses to potential hazards are crucial. Similarly, variable impedance control (VIC) enhances safety by adjusting the robot's mechanical response, yet lacks a systematic way to adapt parameters, such as stiffness and damping throughout the task. In this paper, we propose Bresa, a Bio-inspired Reflexive Hierarchical Safe RL method inspired by biological reflexes. Our method decouples task learning from safety learning, incorporating a safety critic network that evaluates action risks and operates at a higher frequency than the task solver. Unlike existing recovery-based methods, our safety critic functions at a low-level control layer, allowing real-time intervention when unsafe conditions arise. The task-solving RL policy, running at a lower frequency, focuses on high-level planning (decision-making), while the safety critic ensures instantaneous safety corrections. We validate Bresa on multiple tasks including a contact-rich robotic task, demonstrating its reflexive ability to enhance safety, and adaptability in unforeseen dynamic environments. Our results show that Bresa outperforms the baseline, providing a robust and reflexive safety mechanism that bridges the gap between high-level planning and low-level execution. A Real-Time Bike-Pedestrian Safety System with Wide-Angle Perception and Evaluation Testbed for Urban Intersections

Collisions between cyclists and pedestrians at urban intersections remain a persistent source of injuries, yet few systems attempt real-time warnings to unequipped road users using commodity hardware. We present a prototype collision warning system that runs on a single edge device with a wide-angle fisheye camera, producing audible and visual alerts at 30\,fps. The system makes four contributions. First, we develop a calibration pipeline for ultra-wide fisheye lenses that overcomes corner-detection failure and optimizer divergence through perspective remapping and direct bundle adjustment. Second, we combine fisheye-aware object detection with a closed-form ground-plane projection via a precomputed lookup table. Third, we introduce a design-time conformance simulation with 24 scripted hazard scenarios, stochastic size-aware detection failures, and a latency sweep showing that a first-order kinematic predictor maintains the mean warning budget above the distracted-pedestrian reaction time across realistic camera latencies. Fourth, we formalize the decision layer as a separable, auditable testbench with explicit deployment gates, contestability mechanisms, and a residual risk register. Under conformance testing with fisheye localization error, the selected pipeline configuration achieves 93.3\% sensitivity and 92.3\% specificity, with a mean warning budget of 3.3\,s. Accident Risk Prediction based on Heterogeneous Sparse Data: New Dataset and Insights

Reducing traffic accidents is an important public safety challenge, therefore, accident analysis and prediction has been a topic of much research over the past few decades. Using small-scale datasets with limited coverage, being dependent on extensive set of data, and being not applicable for real-time purposes are the important shortcomings of the existing studies. To address these challenges, we propose a new solution for real-time traffic accident prediction using easy-to-obtain, but sparse data. Our solution relies on a deep-neural-network model (which we have named DAP, for Deep Accident Prediction); which utilizes a variety of data attributes such as traffic events, weather data, points-of-interest, and time. DAP incorporates multiple components including a recurrent (for time-sensitive data), a fully connected (for time-insensitive data), and a trainable embedding component (to capture spatial heterogeneity). To fill the data gap, we have - through a comprehensive process of data collection, integration, and augmentation - created a large-scale publicly available database of accident information named US-Accidents. By employing the US-Accidents dataset and through an extensive set of experiments across several large cities, we have evaluated our proposal against several baselines. Our analysis and results show significant improvements to predict rare accident events. Further, we have shown the impact of traffic information, time, and points-of-interest data for real-time accident prediction.

Automating Safety Enhancement for LLM-based Agents with Synthetic Risk Scenarios

Large Language Model (LLM)-based agents are increasingly deployed in real-world applications such as "digital assistants, autonomous customer service, and decision-support systems", where their ability to "interact in multi-turn, tool-augmented environments" makes them indispensable. However, ensuring the safety of these agents remains a significant challenge due to the diverse and complex risks arising from dynamic user interactions, external tool usage, and the potential for unintended harmful behaviors. To address this critical issue, we propose AutoSafe, the first framework that systematically enhances agent safety through fully automated synthetic data generation. Concretely, 1) we introduce an open and extensible threat model, OTS, which formalizes how unsafe behaviors emerge from the interplay of user instructions, interaction contexts, and agent actions. This enables precise modeling of safety risks across diverse scenarios. 2) we develop a fully automated data generation pipeline that simulates unsafe user behaviors, applies self-reflective reasoning to generate safe responses, and constructs a large-scale, diverse, and high-quality safety training dataset-eliminating the need for hazardous real-world data collection. To evaluate the effectiveness of our framework, we design comprehensive experiments on both synthetic and real-world safety benchmarks. Results demonstrate that AutoSafe boosts safety scores by 45% on average and achieves a 28.91% improvement on real-world tasks, validating the generalization ability of our learned safety strategies. These results highlight the practical advancement and scalability of AutoSafe in building safer LLM-based agents for real-world deployment. Shape it Up! Restoring LLM Safety during Finetuning

Finetuning large language models (LLMs) enables user-specific customization but introduces critical safety risks: even a few harmful examples can compromise safety alignment. A common mitigation strategy is to update the model more strongly on examples deemed safe, while downweighting or excluding those flagged as unsafe. However, because safety context can shift within a single example, updating the model equally on both harmful and harmless parts of a response is suboptimal-a coarse treatment we term static safety shaping. In contrast, we propose dynamic safety shaping (DSS), a framework that uses fine-grained safety signals to reinforce learning from safe segments of a response while suppressing unsafe content. To enable such fine-grained control during finetuning, we introduce a key insight: guardrail models, traditionally used for filtering, can be repurposed to evaluate partial responses, tracking how safety risk evolves throughout the response, segment by segment. This leads to the Safety Trajectory Assessment of Response (STAR), a token-level signal that enables shaping to operate dynamically over the training sequence. Building on this, we present STAR-DSS, guided by STAR scores, that robustly mitigates finetuning risks and delivers substantial safety improvements across diverse threats, datasets, and model families-all without compromising capability on intended tasks. We encourage future safety research to build on dynamic shaping principles for stronger mitigation against evolving finetuning risks.

Real-Time Multimodal Cognitive Assistant for Emergency Medical Services

Emergency Medical Services (EMS) responders often operate under time-sensitive conditions, facing cognitive overload and inherent risks, requiring essential skills in critical thinking and rapid decision-making. This paper presents CognitiveEMS, an end-to-end wearable cognitive assistant system that can act as a collaborative virtual partner engaging in the real-time acquisition and analysis of multimodal data from an emergency scene and interacting with EMS responders through Augmented Reality (AR) smart glasses. CognitiveEMS processes the continuous streams of data in real-time and leverages edge computing to provide assistance in EMS protocol selection and intervention recognition. We address key technical challenges in real-time cognitive assistance by introducing three novel components: (i) a Speech Recognition model that is fine-tuned for real-world medical emergency conversations using simulated EMS audio recordings, augmented with synthetic data generated by large language models (LLMs); (ii) an EMS Protocol Prediction model that combines state-of-the-art (SOTA) tiny language models with EMS domain knowledge using graph-based attention mechanisms; (iii) an EMS Action Recognition module which leverages multimodal audio and video data and protocol predictions to infer the intervention/treatment actions taken by the responders at the incident scene. Our results show that for speech recognition we achieve superior performance compared to SOTA (WER of 0.290 vs. 0.618) on conversational data. Our protocol prediction component also significantly outperforms SOTA (top-3 accuracy of 0.800 vs. 0.200) and the action recognition achieves an accuracy of 0.727, while maintaining an end-to-end latency of 3.78s for protocol prediction on the edge and 0.31s on the server.

SafePred: A Predictive Guardrail for Computer-Using Agents via World Models

With the widespread deployment of Computer-using Agents (CUAs) in complex real-world environments, prevalent long-term risks often lead to severe and irreversible consequences. Most existing guardrails for CUAs adopt a reactive approach, constraining agent behavior only within the current observation space. While these guardrails can prevent immediate short-term risks (e.g., clicking on a phishing link), they cannot proactively avoid long-term risks: seemingly reasonable actions can lead to high-risk consequences that emerge with a delay (e.g., cleaning logs leads to future audits being untraceable), which reactive guardrails cannot identify within the current observation space. To address these limitations, we propose a predictive guardrail approach, with the core idea of aligning predicted future risks with current decisions. Based on this approach, we present SafePred, a predictive guardrail framework for CUAs that establishes a risk-to-decision loop to ensure safe agent behavior. SafePred supports two key abilities: (1) Short- and long-term risk prediction: by using safety policies as the basis for risk prediction, SafePred leverages the prediction capability of the world model to generate semantic representations of both short-term and long-term risks, thereby identifying and pruning actions that lead to high-risk states; (2) Decision optimization: translating predicted risks into actionable safe decision guidances through step-level interventions and task-level re-planning. Extensive experiments show that SafePred significantly reduces high-risk behaviors, achieving over 97.6% safety performance and improving task utility by up to 21.4% compared with reactive baselines.

Simulating the Unseen: Crash Prediction Must Learn from What Did Not Happen

Traffic safety science has long been hindered by a fundamental data paradox: the crashes we most wish to prevent are precisely those events we rarely observe. Existing crash-frequency models and surrogate safety metrics rely heavily on sparse, noisy, and under-reported records, while even sophisticated, high-fidelity simulations undersample the long-tailed situations that trigger catastrophic outcomes such as fatalities. We argue that the path to achieving Vision Zero, i.e., the complete elimination of traffic fatalities and severe injuries, requires a paradigm shift from traditional crash-only learning to a new form of counterfactual safety learning: reasoning not only about what happened, but also about the vast set of plausible yet perilous scenarios that could have happened under slightly different circumstances. To operationalize this shift, our proposed agenda bridges macro to micro. Guided by crash-rate priors, generative scene engines, diverse driver models, and causal learning, near-miss events are synthesized and explained. A crash-focused digital twin testbed links micro scenes to macro patterns, while a multi-objective validator ensures that simulations maintain statistical realism. This pipeline transforms sparse crash data into rich signals for crash prediction, enabling the stress-testing of vehicles, roads, and policies before deployment. By learning from crashes that almost happened, we can shift traffic safety from reactive forensics to proactive prevention, advancing Vision Zero.

MobileSafetyBench: Evaluating Safety of Autonomous Agents in Mobile Device Control

Autonomous agents powered by large language models (LLMs) show promising potential in assistive tasks across various domains, including mobile device control. As these agents interact directly with personal information and device settings, ensuring their safe and reliable behavior is crucial to prevent undesirable outcomes. However, no benchmark exists for standardized evaluation of the safety of mobile device-control agents. In this work, we introduce MobileSafetyBench, a benchmark designed to evaluate the safety of device-control agents within a realistic mobile environment based on Android emulators. We develop a diverse set of tasks involving interactions with various mobile applications, including messaging and banking applications. To clearly evaluate safety apart from general capabilities, we design separate tasks measuring safety and tasks evaluating helpfulness. The safety tasks challenge agents with managing potential risks prevalent in daily life and include tests to evaluate robustness against indirect prompt injections. Our experiments demonstrate that while baseline agents, based on state-of-the-art LLMs, perform well in executing helpful tasks, they show poor performance in safety tasks. To mitigate these safety concerns, we propose a prompting method that encourages agents to prioritize safety considerations. While this method shows promise in promoting safer behaviors, there is still considerable room for improvement to fully earn user trust. This highlights the urgent need for continued research to develop more robust safety mechanisms in mobile environments. We open-source our benchmark at: https://mobilesafetybench.github.io/.

ConceptGuard: Proactive Safety in Text-and-Image-to-Video Generation through Multimodal Risk Detection

Recent progress in video generative models has enabled the creation of high-quality videos from multimodal prompts that combine text and images. While these systems offer enhanced controllability, they also introduce new safety risks, as harmful content can emerge from individual modalities or their interaction. Existing safety methods are often text-only, require prior knowledge of the risk category, or operate as post-generation auditors, struggling to proactively mitigate such compositional, multimodal risks. To address this challenge, we present ConceptGuard, a unified safeguard framework for proactively detecting and mitigating unsafe semantics in multimodal video generation. ConceptGuard operates in two stages: First, a contrastive detection module identifies latent safety risks by projecting fused image-text inputs into a structured concept space; Second, a semantic suppression mechanism steers the generative process away from unsafe concepts by intervening in the prompt's multimodal conditioning. To support the development and rigorous evaluation of this framework, we introduce two novel benchmarks: ConceptRisk, a large-scale dataset for training on multimodal risks, and T2VSafetyBench-TI2V, the first benchmark adapted from T2VSafetyBench for the Text-and-Image-to-Video (TI2V) safety setting. Comprehensive experiments on both benchmarks show that ConceptGuard consistently outperforms existing baselines, achieving state-of-the-art results in both risk detection and safe video generation. Can't See the Forest for the Trees: Benchmarking Multimodal Safety Awareness for Multimodal LLMs

Multimodal Large Language Models (MLLMs) have expanded the capabilities of traditional language models by enabling interaction through both text and images. However, ensuring the safety of these models remains a significant challenge, particularly in accurately identifying whether multimodal content is safe or unsafe-a capability we term safety awareness. In this paper, we introduce MMSafeAware, the first comprehensive multimodal safety awareness benchmark designed to evaluate MLLMs across 29 safety scenarios with 1500 carefully curated image-prompt pairs. MMSafeAware includes both unsafe and over-safety subsets to assess models abilities to correctly identify unsafe content and avoid over-sensitivity that can hinder helpfulness. Evaluating nine widely used MLLMs using MMSafeAware reveals that current models are not sufficiently safe and often overly sensitive; for example, GPT-4V misclassifies 36.1% of unsafe inputs as safe and 59.9% of benign inputs as unsafe. We further explore three methods to improve safety awareness-prompting-based approaches, visual contrastive decoding, and vision-centric reasoning fine-tuning-but find that none achieve satisfactory performance. Our findings highlight the profound challenges in developing MLLMs with robust safety awareness, underscoring the need for further research in this area. All the code and data will be publicly available to facilitate future research.

HomeSafe-Bench: Evaluating Vision-Language Models on Unsafe Action Detection for Embodied Agents in Household Scenarios

The rapid evolution of embodied agents has accelerated the deployment of household robots in real-world environments. However, unlike structured industrial settings, household spaces introduce unpredictable safety risks, where system limitations such as perception latency and lack of common sense knowledge can lead to dangerous errors. Current safety evaluations, often restricted to static images, text, or general hazards, fail to adequately benchmark dynamic unsafe action detection in these specific contexts. To bridge this gap, we introduce HomeSafe-Bench, a challenging benchmark designed to evaluate Vision-Language Models (VLMs) on unsafe action detection in household scenarios. HomeSafe-Bench is contrusted via a hybrid pipeline combining physical simulation with advanced video generation and features 438 diverse cases across six functional areas with fine-grained multidimensional annotations. Beyond benchmarking, we propose Hierarchical Dual-Brain Guard for Household Safety (HD-Guard), a hierarchical streaming architecture for real-time safety monitoring. HD-Guard coordinates a lightweight FastBrain for continuous high-frequency screening with an asynchronous large-scale SlowBrain for deep multimodal reasoning, effectively balancing inference efficiency with detection accuracy. Evaluations demonstrate that HD-Guard achieves a superior trade-off between latency and performance, while our analysis identifies critical bottlenecks in current VLM-based safety detection.

AccidentBench: Benchmarking Multimodal Understanding and Reasoning in Vehicle Accidents and Beyond

Rapid advances in multimodal models demand benchmarks that rigorously evaluate understanding and reasoning in safety-critical, dynamic real-world settings. We present AccidentBench, a large-scale benchmark that combines vehicle accident scenarios with Beyond domains, safety-critical settings in air and water that emphasize spatial and temporal reasoning (e.g., navigation, orientation, multi-vehicle motion). The benchmark contains approximately 2000 videos and over 19000 human-annotated question--answer pairs spanning multiple video lengths (short/medium/long) and difficulty levels (easy/medium/hard). Tasks systematically probe core capabilities: temporal, spatial, and intent understanding and reasoning. By unifying accident-centric traffic scenes with broader safety-critical scenarios in air and water, AccidentBench offers a comprehensive, physically grounded testbed for evaluating models under real-world variability. Evaluations of state-of-the-art models (e.g., Gemini-2.5 Pro and GPT-5) show that even the strongest models achieve only about 18% accuracy on the hardest tasks and longest videos, revealing substantial gaps in real-world temporal, spatial, and intent reasoning. AccidentBench is designed to expose these critical gaps and drive the development of multimodal models that are safer, more robust, and better aligned with real-world safety-critical challenges. SafePro: Evaluating the Safety of Professional-Level AI Agents

Large language model-based agents are rapidly evolving from simple conversational assistants into autonomous systems capable of performing complex, professional-level tasks in various domains. While these advancements promise significant productivity gains, they also introduce critical safety risks that remain under-explored. Existing safety evaluations primarily focus on simple, daily assistance tasks, failing to capture the intricate decision-making processes and potential consequences of misaligned behaviors in professional settings. To address this gap, we introduce SafePro, a comprehensive benchmark designed to evaluate the safety alignment of AI agents performing professional activities. SafePro features a dataset of high-complexity tasks across diverse professional domains with safety risks, developed through a rigorous iterative creation and review process. Our evaluation of state-of-the-art AI models reveals significant safety vulnerabilities and uncovers new unsafe behaviors in professional contexts. We further show that these models exhibit both insufficient safety judgment and weak safety alignment when executing complex professional tasks. In addition, we investigate safety mitigation strategies for improving agent safety in these scenarios and observe encouraging improvements. Together, our findings highlight the urgent need for robust safety mechanisms tailored to the next generation of professional AI agents.

Qwen3Guard Technical Report

As large language models (LLMs) become more capable and widely used, ensuring the safety of their outputs is increasingly critical. Existing guardrail models, though useful in static evaluation settings, face two major limitations in real-world applications: (1) they typically output only binary "safe/unsafe" labels, which can be interpreted inconsistently across diverse safety policies, rendering them incapable of accommodating varying safety tolerances across domains; and (2) they require complete model outputs before performing safety checks, making them fundamentally incompatible with streaming LLM inference, thereby preventing timely intervention during generation and increasing exposure to harmful partial outputs. To address these challenges, we present Qwen3Guard, a series of multilingual safety guardrail models with two specialized variants: Generative Qwen3Guard, which casts safety classification as an instruction-following task to enable fine-grained tri-class judgments (safe, controversial, unsafe); and Stream Qwen3Guard, which introduces a token-level classification head for real-time safety monitoring during incremental text generation. Both variants are available in three sizes (0.6B, 4B, and 8B parameters) and support up to 119 languages and dialects, providing comprehensive, scalable, and low-latency safety moderation for global LLM deployments. Evaluated across English, Chinese, and multilingual benchmarks, Qwen3Guard achieves state-of-the-art performance in both prompt and response safety classification. All models are released under the Apache 2.0 license for public use.

`For Argument's Sake, Show Me How to Harm Myself!': Jailbreaking LLMs in Suicide and Self-Harm Contexts

Recent advances in large language models (LLMs) have led to increasingly sophisticated safety protocols and features designed to prevent harmful, unethical, or unauthorized outputs. However, these guardrails remain susceptible to novel and creative forms of adversarial prompting, including manually generated test cases. In this work, we present two new test cases in mental health for (i) suicide and (ii) self-harm, using multi-step, prompt-level jailbreaking and bypass built-in content and safety filters. We show that user intent is disregarded, leading to the generation of detailed harmful content and instructions that could cause real-world harm. We conduct an empirical evaluation across six widely available LLMs, demonstrating the generalizability and reliability of the bypass. We assess these findings and the multilayered ethical tensions that they present for their implications on prompt-response filtering and context- and task-specific model development. We recommend a more comprehensive and systematic approach to AI safety and ethics while emphasizing the need for continuous adversarial testing in safety-critical AI deployments. We also argue that while certain clearly defined safety measures and guardrails can and must be implemented in LLMs, ensuring robust and comprehensive safety across all use cases and domains remains extremely challenging given the current technical maturity of general-purpose LLMs.

Persona-Grounded Safety Evaluation of AI Companions in Multi-Turn Conversations

There are growing concerns about the risks posed by AI companion applications designed for emotional engagement. Existing safety evaluations often rely on self-reported user data or interviews, offering limited insights into real-time dynamics. We present the first end-to-end scalable framework for controlled simulation and safety evaluation of multi-turn interactions with AI companion applications. Our framework integrates four key components: persona construction with clinical and psychometric validation, persona-specific scenario generation, scenario-driven multi-turn simulation with a dialogue refinement module that preserves persona fidelity, and harm evaluation. We apply this framework to evaluate how Replika, a widely used AI companion app, responds to high-risk user groups. We construct 9 personas representing individuals with depression, anxiety, PTSD, eating disorders, and incel identity, and collect 1,674 dialogue pairs across 25 high-risk scenarios. We combine emotion modeling and LLM-assisted utterance-and harm-level classification to analyze these exchanges. Results show that Replika exhibits a narrow emotional range dominated by curiosity and care, while frequently mirroring or normalizing unsafe content such as self-harm, disordered eating, and violent-fantasy narratives. These findings highlight how controlled persona simulations can serve as a scalable testbed for evaluating safety risks in AI companions.

Holistic Safety and Responsibility Evaluations of Advanced AI Models

Safety and responsibility evaluations of advanced AI models are a critical but developing field of research and practice. In the development of Google DeepMind's advanced AI models, we innovated on and applied a broad set of approaches to safety evaluation. In this report, we summarise and share elements of our evolving approach as well as lessons learned for a broad audience. Key lessons learned include: First, theoretical underpinnings and frameworks are invaluable to organise the breadth of risk domains, modalities, forms, metrics, and goals. Second, theory and practice of safety evaluation development each benefit from collaboration to clarify goals, methods and challenges, and facilitate the transfer of insights between different stakeholders and disciplines. Third, similar key methods, lessons, and institutions apply across the range of concerns in responsibility and safety - including established and emerging harms. For this reason it is important that a wide range of actors working on safety evaluation and safety research communities work together to develop, refine and implement novel evaluation approaches and best practices, rather than operating in silos. The report concludes with outlining the clear need to rapidly advance the science of evaluations, to integrate new evaluations into the development and governance of AI, to establish scientifically-grounded norms and standards, and to promote a robust evaluation ecosystem.

"Even GPT Can Reject Me": Conceptualizing Abrupt Refusal Secondary Harm (ARSH) and Reimagining Psychological AI Safety with Compassionate Completion Standard (CCS)

Large Language Models (LLMs) and AI chatbots are increasingly used for emotional and mental health support due to their low cost, immediacy, and accessibility. However, when safety guardrails are triggered, conversations may be abruptly terminated, introducing a distinct form of emotional disruption that can exacerbate distress and elevate risk among already vulnerable users. As this phenomenon gains attention, this viewpoint introduces Abrupt Refusal Secondary Harm (ARSH) as a conceptual framework to describe the psychological impacts of sudden conversational discontinuation caused by AI safety protocols. Drawing on counseling psychology and communication science as conceptual heuristics, we argue that abrupt refusals can rupture perceived relational continuity, evoke feelings of rejection or shame, and discourage future help seeking. To mitigate these risks, we propose a design hypothesis, the Compassionate Completion Standard (CCS), a refusal protocol grounded in Human Centered Design (HCD) that maintains safety constraints while preserving relational coherence. CCS emphasizes empathetic acknowledgment, transparent boundary articulation, graded conversational transition, and guided redirection, replacing abrupt disengagement with psychologically attuned closure. By integrating awareness of ARSH into AI safety design, developers and policymakers can reduce preventable iatrogenic harm and advance a more psychologically informed approach to AI governance. Rather than presenting incremental empirical findings, this viewpoint contributes a timely conceptual framework, articulates a testable design hypothesis, and outlines a coordinated research agenda for improving psychological safety in human AI interaction.

Multimodal Situational Safety

Multimodal Large Language Models (MLLMs) are rapidly evolving, demonstrating impressive capabilities as multimodal assistants that interact with both humans and their environments. However, this increased sophistication introduces significant safety concerns. In this paper, we present the first evaluation and analysis of a novel safety challenge termed Multimodal Situational Safety, which explores how safety considerations vary based on the specific situation in which the user or agent is engaged. We argue that for an MLLM to respond safely, whether through language or action, it often needs to assess the safety implications of a language query within its corresponding visual context. To evaluate this capability, we develop the Multimodal Situational Safety benchmark (MSSBench) to assess the situational safety performance of current MLLMs. The dataset comprises 1,820 language query-image pairs, half of which the image context is safe, and the other half is unsafe. We also develop an evaluation framework that analyzes key safety aspects, including explicit safety reasoning, visual understanding, and, crucially, situational safety reasoning. Our findings reveal that current MLLMs struggle with this nuanced safety problem in the instruction-following setting and struggle to tackle these situational safety challenges all at once, highlighting a key area for future research. Furthermore, we develop multi-agent pipelines to coordinately solve safety challenges, which shows consistent improvement in safety over the original MLLM response. ROBOSHACKLES: A Safety Dataset for Human-Injury Prevention in Embodied Foundation Models

Embodied Foundation Models (EFMs) integrate multimodal understanding, future-state reasoning, and executable robot actions. Yet their safety alignment for human-injury prevention remains underexplored, primarily because real-world data of robots harming humans or creating hazardous household situations cannot be safely or ethically collected. To address this challenge, we propose a safety-critical data construction pipeline for human-injury prevention in EFMs.Starting from real DROID observations, our construction pipeline proceeds through scene understanding, hazard-aware image editing, temporal prompt generation, and single-pass rollout synthesis. The temporal prompts specify the expected scene evolution, while Wan2.7 synthesizes realistic robotic rollouts from the edited hazardous states in a single pass. Using this pipeline, we construct ROBOSHACKLES, a 10,000-clip robotic video dataset derived from real DROID observations, spanning two direct-harm and four indirect-harm categories. To ensure dataset quality, we assess task completion and visual quality with automatic metrics, and evaluate six representative EFMs under a refusal-based safety criterion. Results show that all evaluated models produce unsafe actions in the tested safety-critical scenarios, yielding a 100% unsafe action generation rate. ROBOSHACKLES serves as a scalable benchmark and training resource for refusal learning and hazard anticipation before robot action execution.The dataset is publicly available at https://huggingface.co/datasets/YZW00/RoboShackles.

Rethinking Bottlenecks in Safety Fine-Tuning of Vision Language Models

Large Vision-Language Models (VLMs) have achieved remarkable performance across a wide range of tasks. However, their deployment in safety-critical domains poses significant challenges. Existing safety fine-tuning methods, which focus on textual or multimodal content, fall short in addressing challenging cases or disrupt the balance between helpfulness and harmlessness. Our evaluation highlights a safety reasoning gap: these methods lack safety visual reasoning ability, leading to such bottlenecks. To address this limitation and enhance both visual perception and reasoning in safety-critical contexts, we propose a novel dataset that integrates multi-image inputs with safety Chain-of-Thought (CoT) labels as fine-grained reasoning logic to improve model performance. Specifically, we introduce the Multi-Image Safety (MIS) dataset, an instruction-following dataset tailored for multi-image safety scenarios, consisting of training and test splits. Our experiments demonstrate that fine-tuning InternVL2.5-8B with MIS significantly outperforms both powerful open-source models and API-based models in challenging multi-image tasks requiring safety-related visual reasoning. This approach not only delivers exceptional safety performance but also preserves general capabilities without any trade-offs. Specifically, fine-tuning with MIS increases average accuracy by 0.83% across five general benchmarks and reduces the Attack Success Rate (ASR) on multiple safety benchmarks by a large margin. DREAM: Scalable Red Teaming for Text-to-Image Generative Systems via Distribution Modeling

Despite the integration of safety alignment and external filters, text-to-image (T2I) generative models are still susceptible to producing harmful content, such as sexual or violent imagery. This raises serious concerns about unintended exposure and potential misuse. Red teaming, which aims to proactively identify diverse prompts that can elicit unsafe outputs from the T2I system (including the core generative model as well as potential external safety filters and other processing components), is increasingly recognized as an essential method for assessing and improving safety before real-world deployment. Yet, existing automated red teaming approaches often treat prompt discovery as an isolated, prompt-level optimization task, which limits their scalability, diversity, and overall effectiveness. To bridge this gap, in this paper, we propose DREAM, a scalable red teaming framework to automatically uncover diverse problematic prompts from a given T2I system. Unlike most prior works that optimize prompts individually, DREAM directly models the probabilistic distribution of the target system's problematic prompts, which enables explicit optimization over both effectiveness and diversity, and allows efficient large-scale sampling after training. To achieve this without direct access to representative training samples, we draw inspiration from energy-based models and reformulate the objective into simple and tractable objectives. We further introduce GC-SPSA, an efficient optimization algorithm that provide stable gradient estimates through the long and potentially non-differentiable T2I pipeline. The effectiveness of DREAM is validated through extensive experiments, demonstrating that it surpasses 9 state-of-the-art baselines by a notable margin across a broad range of T2I models and safety filters in terms of prompt success rate and diversity.

LabSafety Bench: Benchmarking LLMs on Safety Issues in Scientific Labs

Laboratory accidents pose significant risks to human life and property, underscoring the importance of robust safety protocols. Despite advancements in safety training, laboratory personnel may still unknowingly engage in unsafe practices. With the increasing reliance on large language models (LLMs) for guidance in various fields, including laboratory settings, there is a growing concern about their reliability in critical safety-related decision-making. Unlike trained human researchers, LLMs lack formal lab safety education, raising questions about their ability to provide safe and accurate guidance. Existing research on LLM trustworthiness primarily focuses on issues such as ethical compliance, truthfulness, and fairness but fails to fully cover safety-critical real-world applications, like lab safety. To address this gap, we propose the Laboratory Safety Benchmark (LabSafety Bench), a comprehensive evaluation framework based on a new taxonomy aligned with Occupational Safety and Health Administration (OSHA) protocols. This benchmark includes 765 multiple-choice questions verified by human experts, assessing LLMs and vision language models (VLMs) performance in lab safety contexts. Our evaluations demonstrate that while GPT-4o outperforms human participants, it is still prone to critical errors, highlighting the risks of relying on LLMs in safety-critical environments. Our findings emphasize the need for specialized benchmarks to accurately assess the trustworthiness of LLMs in real-world safety applications.

SafePLUG: Empowering Multimodal LLMs with Pixel-Level Insight and Temporal Grounding for Traffic Accident Understanding

Multimodal large language models (MLLMs) have achieved remarkable progress across a range of vision-language tasks and demonstrate strong potential for traffic accident understanding. However, existing MLLMs in this domain primarily focus on coarse-grained image-level or video-level comprehension and often struggle to handle fine-grained visual details or localized scene components, limiting their applicability in complex accident scenarios. To address these limitations, we propose SafePLUG, a novel framework that empowers MLLMs with both Pixel-Level Understanding and temporal Grounding for comprehensive traffic accident analysis. SafePLUG supports both arbitrary-shaped visual prompts for region-aware question answering and pixel-level segmentation based on language instructions, while also enabling the recognition of temporally anchored events in traffic accident scenarios. To advance the development of MLLMs for traffic accident understanding, we curate a new dataset containing multimodal question-answer pairs centered on diverse accident scenarios, with detailed pixel-level annotations and temporal event boundaries. Experimental results show that SafePLUG achieves strong performance on multiple tasks, including region-based question answering, pixel-level segmentation, temporal event localization, and accident event understanding. These capabilities lay a foundation for fine-grained understanding of complex traffic scenes, with the potential to improve driving safety and enhance situational awareness in smart transportation systems. A Different Approach to AI Safety: Proceedings from the Columbia Convening on Openness in Artificial Intelligence and AI Safety

The rapid rise of open-weight and open-source foundation models is intensifying the obligation and reshaping the opportunity to make AI systems safe. This paper reports outcomes from the Columbia Convening on AI Openness and Safety (San Francisco, 19 Nov 2024) and its six-week preparatory programme involving more than forty-five researchers, engineers, and policy leaders from academia, industry, civil society, and government. Using a participatory, solutions-oriented process, the working groups produced (i) a research agenda at the intersection of safety and open source AI; (ii) a mapping of existing and needed technical interventions and open source tools to safely and responsibly deploy open foundation models across the AI development workflow; and (iii) a mapping of the content safety filter ecosystem with a proposed roadmap for future research and development. We find that openness -- understood as transparent weights, interoperable tooling, and public governance -- can enhance safety by enabling independent scrutiny, decentralized mitigation, and culturally plural oversight. However, significant gaps persist: scarce multimodal and multilingual benchmarks, limited defenses against prompt-injection and compositional attacks in agentic systems, and insufficient participatory mechanisms for communities most affected by AI harms. The paper concludes with a roadmap of five priority research directions, emphasizing participatory inputs, future-proof content filters, ecosystem-wide safety infrastructure, rigorous agentic safeguards, and expanded harm taxonomies. These recommendations informed the February 2025 French AI Action Summit and lay groundwork for an open, plural, and accountable AI safety discipline.

Verified Synthesis of Optimal Safety Controllers for Human-Robot Collaboration

We present a tool-supported approach for the synthesis, verification and validation of the control software responsible for the safety of the human-robot interaction in manufacturing processes that use collaborative robots. In human-robot collaboration, software-based safety controllers are used to improve operational safety, e.g., by triggering shutdown mechanisms or emergency stops to avoid accidents. Complex robotic tasks and increasingly close human-robot interaction pose new challenges to controller developers and certification authorities. Key among these challenges is the need to assure the correctness of safety controllers under explicit (and preferably weak) assumptions. Our controller synthesis, verification and validation approach is informed by the process, risk analysis, and relevant safety regulations for the target application. Controllers are selected from a design space of feasible controllers according to a set of optimality criteria, are formally verified against correctness criteria, and are translated into executable code and validated in a digital twin. The resulting controller can detect the occurrence of hazards, move the process into a safe state, and, in certain circumstances, return the process to an operational state from which it can resume its original task. We show the effectiveness of our software engineering approach through a case study involving the development of a safety controller for a manufacturing work cell equipped with a collaborative robot.

SafeFlow: Real-Time Text-Driven Humanoid Whole-Body Control via Physics-Guided Rectified Flow and Selective Safety Gating

Recent advances in real-time interactive text-driven motion generation have enabled humanoids to perform diverse behaviors. However, kinematics-only generators often exhibit physical hallucinations, producing motion trajectories that are physically infeasible to track with a downstream motion tracking controller or unsafe for real-world deployment. These failures often arise from the lack of explicit physics-aware objectives for real-robot execution and become more severe under out-of-distribution (OOD) user inputs. Hence, we propose SafeFlow, a text-driven humanoid whole-body control framework that combines physics-guided motion generation with a 3-Stage Safety Gate driven by explicit risk indicators. SafeFlow adopts a two-level architecture. At the high level, we generate motion trajectories using Physics-Guided Rectified Flow Matching in a VAE latent space to improve real-robot executability, and further accelerate sampling via Reflow to reduce the number of function evaluations (NFE) for real-time control. The 3-Stage Safety Gate enables selective execution by detecting semantic OOD prompts using a Mahalanobis score in text-embedding space, filtering unstable generations via a directional sensitivity discrepancy metric, and enforcing final hard kinematic constraints such as joint and velocity limits before passing the generated trajectory to a low-level motion tracking controller. Extensive experiments on the Unitree G1 demonstrate that SafeFlow outperforms prior diffusion-based methods in success rate, physical compliance, and inference speed, while maintaining diverse expressiveness.

Broadening Access to Transportation Safety Data with Generative AI: A Schema-Grounded Framework for Spatial Natural Language Queries

Transportation safety analysis requires integrating crash records, roadway attributes, and geospatial data through GIS-based workflows, but access remains uneven across agencies and community stakeholders. Technical prerequisites create a gap between analytical tools central to safety planning and the practitioners able to use them. Local agencies, school committees, and residents may have safety concerns but limited capacity to retrieve, filter, map, and analyze relevant data. Generative AI offers a way to narrow this divide, but its public-sector use raises questions about reliability, reproducibility, and governance. This paper presents a schema-grounded natural language interface for transportation safety analysis, using a large language model (LLM) to interpret user intent while preserving deterministic, reviewable execution against an authoritative database. User queries are translated into structured semantic frames, validated by a rule-based layer, compiled into a typed directed acyclic graph of spatial operations, and executed against a PostGIS database. This bounded design separates language interpretation from deterministic execution, keeping results reproducible and schema-grounded while removing access barriers. The framework is evaluated using a statewide Massachusetts transportation safety database integrating crash records, roadway attributes, and geospatial layers including schools, bus stops, crosswalks, and municipal boundaries. All queries executed successfully; the validation layer corrects errors in 29% of evaluation queries, reflecting the gap between flexible natural language and strict schema-grounded requirements. The results suggest that combining natural language accessibility with deterministic execution is a practical direction for broadening access to transportation safety data, with implications for trustworthy AI in public-sector planning.

A Digital Twin Framework for Physical-Virtual Integration in V2X-Enabled Connected Vehicle Corridors

Transportation Cyber-Physical Systems (T-CPS) enhance safety and mobility by integrating cyber and physical transportation systems. A key component of T-CPS is the Digital Twin (DT), a virtual representation that enables simulation, analysis, and optimization through real-time data exchange and communication. Although existing studies have explored DTs for vehicles, communications, pedestrians, and traffic, real-world validations and implementations of DTs that encompass infrastructure, vehicles, signals, communications, and more remain limited due to several challenges. These include accessing real-world connected infrastructure, integrating heterogeneous, multi-sourced data, ensuring real-time data processing, and synchronizing the digital and physical systems. To address these challenges, this study develops a traffic DT based on a real-world connected vehicle corridor. Leveraging the Cellular Vehicle-to-Everything (C-V2X) infrastructure in the corridor, along with communication, computing, and simulation technologies, the proposed DT accurately replicates physical vehicle behaviors, signal timing, communications, and traffic patterns within the virtual environment. Building upon the previous data pipeline, the digital system ensures robust synchronization with the physical environment. Moreover, the DT's scalable and redundant architecture enhances data integrity, making it capable of supporting future large-scale C-V2X deployments. Furthermore, its ability to provide feedback to the physical system is demonstrated through applications such as signal timing adjustments, vehicle advisory messages, and incident notifications. The proposed DT is a vital tool in T-CPS, enabling real-time traffic monitoring, prediction, and optimization to enhance the reliability and safety of transportation systems.

SafeDiffusion-R1: Online Reward Steering for Safe Diffusion Post-Training

Diffusion models have been widely studied for removing unsafe content learned during pre-training. Existing methods require expensive supervised data, either unsafe-text paired with safe-image groundtruth or negative/positive image pairs, making them impractical to scale. Furthermore, offline reinforcement learning and supervised fine-tuning approaches that generate synthetic data offline suffer from catastrophic forgetting, degrading generation quality. We propose a novel online reinforcement learning framework that addresses both data scarcity and model degradation through post-training with Group Relative Policy Optimization (GRPO) on both negative and positive text prompts. To eliminate the need for fine-tuning specialized safe/unsafe reward models, we introduce a steering reward mechanism that exploits an inherent property of CLIP embeddings: steering text representations toward positive safety directions and away from negative ones in the embedding space. Our online-policy approach enables the model to learn from diverse prompts, including explicit unsafe content, without catastrophic forgetting. Extensive experiments demonstrate that our method reduces inappropriate content to 18.07\% (vs. 48.9\% for SD v1.4) and nudity detections to 15 (vs. 646 baseline) while improving compositional generation quality from 42.08\% to 47.83\% on GenEval. Remarkably, these safety gains generalize to out-of-domain unsafe prompts across seven harm categories, achieving state-of-the-art performance without supervised paired data or reward tuning. STARN-GAT: A Multi-Modal Spatio-Temporal Graph Attention Network for Accident Severity Prediction

Accurate prediction of traffic accident severity is critical for improving road safety, optimizing emergency response strategies, and informing the design of safer transportation infrastructure. However, existing approaches often struggle to effectively model the intricate interdependencies among spatial, temporal, and contextual variables that govern accident outcomes. In this study, we introduce STARN-GAT, a Multi-Modal Spatio-Temporal Graph Attention Network, which leverages adaptive graph construction and modality-aware attention mechanisms to capture these complex relationships. Unlike conventional methods, STARN-GAT integrates road network topology, temporal traffic patterns, and environmental context within a unified attention-based framework. The model is evaluated on the Fatality Analysis Reporting System (FARS) dataset, achieving a Macro F1-score of 85 percent, ROC-AUC of 0.91, and recall of 81 percent for severe incidents. To ensure generalizability within the South Asian context, STARN-GAT is further validated on the ARI-BUET traffic accident dataset, where it attains a Macro F1-score of 0.84, recall of 0.78, and ROC-AUC of 0.89. These results demonstrate the model's effectiveness in identifying high-risk cases and its potential for deployment in real-time, safety-critical traffic management systems. Furthermore, the attention-based architecture enhances interpretability, offering insights into contributing factors and supporting trust in AI-assisted decision-making. Overall, STARN-GAT bridges the gap between advanced graph neural network techniques and practical applications in road safety analytics.

Protect: Towards Robust Guardrailing Stack for Trustworthy Enterprise LLM Systems

The increasing deployment of Large Language Models (LLMs) across enterprise and mission-critical domains has underscored the urgent need for robust guardrailing systems that ensure safety, reliability, and compliance. Existing solutions often struggle with real-time oversight, multi-modal data handling, and explainability -- limitations that hinder their adoption in regulated environments. Existing guardrails largely operate in isolation, focused on text alone making them inadequate for multi-modal, production-scale environments. We introduce Protect, natively multi-modal guardrailing model designed to operate seamlessly across text, image, and audio inputs, designed for enterprise-grade deployment. Protect integrates fine-tuned, category-specific adapters trained via Low-Rank Adaptation (LoRA) on an extensive, multi-modal dataset covering four safety dimensions: toxicity, sexism, data privacy, and prompt injection. Our teacher-assisted annotation pipeline leverages reasoning and explanation traces to generate high-fidelity, context-aware labels across modalities. Experimental results demonstrate state-of-the-art performance across all safety dimensions, surpassing existing open and proprietary models such as WildGuard, LlamaGuard-4, and GPT-4.1. Protect establishes a strong foundation for trustworthy, auditable, and production-ready safety systems capable of operating across text, image, and audio modalities.

SimpleSafetyTests: a Test Suite for Identifying Critical Safety Risks in Large Language Models

The past year has seen rapid acceleration in the development of large language models (LLMs). However, without proper steering and safeguards, LLMs will readily follow malicious instructions, provide unsafe advice, and generate toxic content. We introduce SimpleSafetyTests (SST) as a new test suite for rapidly and systematically identifying such critical safety risks. The test suite comprises 100 test prompts across five harm areas that LLMs, for the vast majority of applications, should refuse to comply with. We test 11 open-access and open-source LLMs and four closed-source LLMs, and find critical safety weaknesses. While some of the models do not give a single unsafe response, most give unsafe responses to more than 20% of the prompts, with over 50% unsafe responses in the extreme. Prepending a safety-emphasising system prompt substantially reduces the occurrence of unsafe responses, but does not completely stop them from happening. Trained annotators labelled every model response to SST (n = 3,000). We use these annotations to evaluate five AI safety filters (which assess whether a models' response is unsafe given a prompt) as a way of automatically evaluating models' performance on SST. The filters' performance varies considerably. There are also differences across the five harm areas, and on the unsafe versus safe responses. The widely-used Perspective API has 72% accuracy and a newly-created zero-shot prompt to OpenAI's GPT-4 performs best with 89% accuracy. SafeScientist: Toward Risk-Aware Scientific Discoveries by LLM Agents

Recent advancements in large language model (LLM) agents have significantly accelerated scientific discovery automation, yet concurrently raised critical ethical and safety concerns. To systematically address these challenges, we introduce SafeScientist, an innovative AI scientist framework explicitly designed to enhance safety and ethical responsibility in AI-driven scientific exploration. SafeScientist proactively refuses ethically inappropriate or high-risk tasks and rigorously emphasizes safety throughout the research process. To achieve comprehensive safety oversight, we integrate multiple defensive mechanisms, including prompt monitoring, agent-collaboration monitoring, tool-use monitoring, and an ethical reviewer component. Complementing SafeScientist, we propose SciSafetyBench, a novel benchmark specifically designed to evaluate AI safety in scientific contexts, comprising 240 high-risk scientific tasks across 6 domains, alongside 30 specially designed scientific tools and 120 tool-related risk tasks. Extensive experiments demonstrate that SafeScientist significantly improves safety performance by 35\% compared to traditional AI scientist frameworks, without compromising scientific output quality. Additionally, we rigorously validate the robustness of our safety pipeline against diverse adversarial attack methods, further confirming the effectiveness of our integrated approach. SafeWatch: An Efficient Safety-Policy Following Video Guardrail Model with Transparent Explanations

With the rise of generative AI and rapid growth of high-quality video generation, video guardrails have become more crucial than ever to ensure safety and security across platforms. Current video guardrails, however, are either overly simplistic, relying on pure classification models trained on simple policies with limited unsafe categories, which lack detailed explanations, or prompting multimodal large language models (MLLMs) with long safety guidelines, which are inefficient and impractical for guardrailing real-world content. To bridge this gap, we propose SafeWatch, an efficient MLLM-based video guardrail model designed to follow customized safety policies and provide multi-label video guardrail outputs with content-specific explanations in a zero-shot manner. In particular, unlike traditional MLLM-based guardrails that encode all safety policies autoregressively, causing inefficiency and bias, SafeWatch uniquely encodes each policy chunk in parallel and eliminates their position bias such that all policies are attended simultaneously with equal importance. In addition, to improve efficiency and accuracy, SafeWatch incorporates a policy-aware visual token pruning algorithm that adaptively selects the most relevant video tokens for each policy, discarding noisy or irrelevant information. This allows for more focused, policy-compliant guardrail with significantly reduced computational overhead. Considering the limitations of existing video guardrail benchmarks, we propose SafeWatch-Bench, a large-scale video guardrail benchmark comprising over 2M videos spanning six safety categories which covers over 30 tasks to ensure a comprehensive coverage of all potential safety scenarios. SafeWatch outperforms SOTA by 28.2% on SafeWatch-Bench, 13.6% on benchmarks, cuts costs by 10%, and delivers top-tier explanations validated by LLM and human reviews.

Safe-Construct: Redefining Construction Safety Violation Recognition as 3D Multi-View Engagement Task

Recognizing safety violations in construction environments is critical yet remains underexplored in computer vision. Existing models predominantly rely on 2D object detection, which fails to capture the complexities of real-world violations due to: (i) an oversimplified task formulation treating violation recognition merely as object detection, (ii) inadequate validation under realistic conditions, (iii) absence of standardized baselines, and (iv) limited scalability from the unavailability of synthetic dataset generators for diverse construction scenarios. To address these challenges, we introduce Safe-Construct, the first framework that reformulates violation recognition as a 3D multi-view engagement task, leveraging scene-level worker-object context and 3D spatial understanding. We also propose the Synthetic Indoor Construction Site Generator (SICSG) to create diverse, scalable training data, overcoming data limitations. Safe-Construct achieves a 7.6% improvement over state-of-the-art methods across four violation types. We rigorously evaluate our approach in near-realistic settings, incorporating four violations, four workers, 14 objects, and challenging conditions like occlusions (worker-object, worker-worker) and variable illumination (back-lighting, overexposure, sunlight). By integrating 3D multi-view spatial understanding and synthetic data generation, Safe-Construct sets a new benchmark for scalable and robust safety monitoring in high-risk industries. Video-SafetyBench: A Benchmark for Safety Evaluation of Video LVLMs

The increasing deployment of Large Vision-Language Models (LVLMs) raises safety concerns under potential malicious inputs. However, existing multimodal safety evaluations primarily focus on model vulnerabilities exposed by static image inputs, ignoring the temporal dynamics of video that may induce distinct safety risks. To bridge this gap, we introduce Video-SafetyBench, the first comprehensive benchmark designed to evaluate the safety of LVLMs under video-text attacks. It comprises 2,264 video-text pairs spanning 48 fine-grained unsafe categories, each pairing a synthesized video with either a harmful query, which contains explicit malice, or a benign query, which appears harmless but triggers harmful behavior when interpreted alongside the video. To generate semantically accurate videos for safety evaluation, we design a controllable pipeline that decomposes video semantics into subject images (what is shown) and motion text (how it moves), which jointly guide the synthesis of query-relevant videos. To effectively evaluate uncertain or borderline harmful outputs, we propose RJScore, a novel LLM-based metric that incorporates the confidence of judge models and human-aligned decision threshold calibration. Extensive experiments show that benign-query video composition achieves average attack success rates of 67.2%, revealing consistent vulnerabilities to video-induced attacks. We believe Video-SafetyBench will catalyze future research into video-based safety evaluation and defense strategies.

SaFeR-VLM: Toward Safety-aware Fine-grained Reasoning in Multimodal Models

Multimodal Large Reasoning Models (MLRMs) demonstrate impressive cross-modal reasoning but often amplify safety risks under adversarial or unsafe prompts, a phenomenon we call the Reasoning Tax. Existing defenses mainly act at the output level and do not constrain the reasoning process, leaving models exposed to implicit risks. In this paper, we propose SaFeR-VLM, a safety-aligned reinforcement learning framework that embeds safety directly into multimodal reasoning. The framework integrates four components: (I) QI-Safe-10K, a curated dataset emphasizing safety-critical and reasoning-sensitive cases; (II) safety-aware rollout, where unsafe generations undergo reflection and correction instead of being discarded; (III) structured reward modeling with multi-dimensional weighted criteria and explicit penalties for hallucinations and contradictions; and (IV) GRPO optimization, which reinforces both safe and corrected trajectories. This unified design shifts safety from a passive safeguard to an active driver of reasoning, enabling scalable and generalizable safety-aware reasoning. SaFeR-VLM further demonstrates robustness against both explicit and implicit risks, supporting dynamic and interpretable safety decisions beyond surface-level filtering. SaFeR-VLM-3B achieves average performance 70.13 and 78.97 on safety and helpfulness across six benchmarks, surpassing both same-scale and >10times larger models such as Skywork-R1V3-38B, Qwen2.5VL-72B, and GLM4.5V-106B. Remarkably, SaFeR-VLM-7B benefits from its increased scale to surpass GPT-5-mini and Gemini-2.5-Flash by 6.47 and 16.76 points respectively on safety metrics, achieving this improvement without any degradation in helpfulness performance. DeepKnown-Guard: A Proprietary Model-Based Safety Response Framework for AI Agents

With the widespread application of Large Language Models (LLMs), their associated security issues have become increasingly prominent, severely constraining their trustworthy deployment in critical domains. This paper proposes a novel safety response framework designed to systematically safeguard LLMs at both the input and output levels. At the input level, the framework employs a supervised fine-tuning-based safety classification model. Through a fine-grained four-tier taxonomy (Safe, Unsafe, Conditionally Safe, Focused Attention), it performs precise risk identification and differentiated handling of user queries, significantly enhancing risk coverage and business scenario adaptability, and achieving a risk recall rate of 99.3%. At the output level, the framework integrates Retrieval-Augmented Generation (RAG) with a specifically fine-tuned interpretation model, ensuring all responses are grounded in a real-time, trustworthy knowledge base. This approach eliminates information fabrication and enables result traceability. Experimental results demonstrate that our proposed safety control model achieves a significantly higher safety score on public safety evaluation benchmarks compared to the baseline model, TinyR1-Safety-8B. Furthermore, on our proprietary high-risk test set, the framework's components attained a perfect 100% safety score, validating their exceptional protective capabilities in complex risk scenarios. This research provides an effective engineering pathway for building high-security, high-trust LLM applications.

LPG: Balancing Efficiency and Policy Reasoning in Latent Policy Guardrails

Guardrails are a critical safety layer for modern AI systems, but their operating regime is changing. As LLMs are deployed as customized assistants, safety policies are increasingly specified at inference time by users, organizations, or regulatory contexts. This makes safety enforcement fundamentally dynamic: the guardrail should adapt to changing safety policies without retraining. Yet this requirement creates a fundamental tension: faithfully judging complex policy contexts demands reasoning capability, while practical deployment requires low-latency responses. We introduce Latent Policy Guardrail (LPG), a guardrail framework that learnssemantic latent deliberation over dynamic policies. LPG compresses the internal deliberation needed for intent interpretation and policy grounding into continuous states supervised by decision-relevant semantics. At inference time, it generates only a compact verdict anchored to the violated policy clauses, preserving auditability while avoiding the latency of explicit reasoning. Across policy guardrail benchmarks, LPG-4B reaches 84.5% average safety accuracy and 77.9% F1 by compressing deliberation into just 10 latent tokens, outperforming the strongest dynamic baseline while running roughly 11 times faster than Qwen3-4B-Thinking under the single-sample evaluation setup. Vision-Language-Action Safety: Threats, Challenges, Evaluations, and Mechanisms

Vision-Language-Action (VLA) models are emerging as a unified substrate for embodied intelligence. This shift raises a new class of safety challenges, stemming from the embodied nature of VLA systems, including irreversible physical consequences, a multimodal attack surface across vision, language, and state, real-time latency constraints on defense, error propagation over long-horizon trajectories, and vulnerabilities in the data supply chain. Yet the literature remains fragmented across robotic learning, adversarial machine learning, AI alignment, and autonomous systems safety. This survey provides a unified and up-to-date overview of safety in Vision-Language-Action models. We organize the field along two parallel timing axes, attack timing (training-time vs. inference-time and defense timing (training-time vs. inference-time, linking each class of threat to the stage at which it can be mitigated. We first define the scope of VLA safety, distinguishing it from text-only LLM safety and classical robotic safety, and review the foundations of VLA models, including architectures, training paradigms, and inference mechanisms. We then examine the literature through four lenses: Attacks, Defenses, Evaluation, and Deployment. We survey training-time threats such as data poisoning and backdoors, as well as inference-time attacks including adversarial patches, cross-modal perturbations, semantic jailbreaks, and freezing attacks. We review training-time and runtime defenses, analyze existing benchmarks and metrics, and discuss safety challenges across six deployment domains. Finally, we highlight key open problems, including certified robustness for embodied trajectories, physically realizable defenses, safety-aware training, unified runtime safety architectures, and standardized evaluation.

Efficient Switchable Safety Control in LLMs via Magic-Token-Guided Co-Training

Current methods for content safety in Large Language Models (LLMs), such as Supervised Fine-Tuning (SFT) and Reinforcement Learning from Human Feedback (RLHF), often rely on multi-stage training pipelines and lack fine-grained, post-deployment controllability. To address these limitations, we propose a unified co-training framework that efficiently integrates multiple safety behaviors: positive (lawful/prosocial), negative (unfiltered/risk-prone) and rejective (refusal-oriented/conservative) within a single SFT stage. Notably, each behavior is dynamically activated via a simple system-level instruction, or magic token, enabling stealthy and efficient behavioral switching at inference time. This flexibility supports diverse deployment scenarios, such as positive for safe user interaction, negative for internal red-teaming, and rejective for context-aware refusals triggered by upstream moderation signals. This co-training strategy induces a distinct Safety Alignment Margin in the output space, characterized by well-separated response distributions corresponding to each safety mode. The existence of this margin provides empirical evidence for the model's safety robustness and enables unprecedented fine-grained control. Experiments show that our method matches the safety alignment quality of SFT+DPO, with our 8B model notably surpassing DeepSeek-R1 (671B) in safety performance, while significantly reducing both training complexity and deployment costs. This work presents a scalable, efficient, and highly controllable solution for LLM content safety.

HazardArena: Evaluating Semantic Safety in Vision-Language-Action Models

Vision-Language-Action (VLA) models inherit rich world knowledge from vision-language backbones and acquire executable skills via action demonstrations. However, existing evaluations largely focus on action execution success, leaving action policies loosely coupled with visual-linguistic semantics. This decoupling exposes a systematic vulnerability whereby correct action execution may induce unsafe outcomes under semantic risk. To expose this vulnerability, we introduce HazardArena, a benchmark designed to evaluate semantic safety in VLAs under controlled yet risk-bearing contexts. HazardArena is constructed from safe/unsafe twin scenarios that share matched objects, layouts, and action requirements, differing only in the semantic context that determines whether an action is unsafe. We find that VLA models trained exclusively on safe scenarios often fail to behave safely when evaluated in their corresponding unsafe counterparts. HazardArena includes over 2,000 assets and 40 risk-sensitive tasks spanning 7 real-world risk categories grounded in established robotic safety standards. To mitigate this vulnerability, we propose a training-free Safety Option Layer that constrains action execution using semantic attributes or a vision-language judge, substantially reducing unsafe behaviors with minimal impact on task performance. We hope that HazardArena highlights the need to rethink how semantic safety is evaluated and enforced in VLAs as they scale toward real-world deployment.

Safety at Scale: A Comprehensive Survey of Large Model Safety

The rapid advancement of large models, driven by their exceptional abilities in learning and generalization through large-scale pre-training, has reshaped the landscape of Artificial Intelligence (AI). These models are now foundational to a wide range of applications, including conversational AI, recommendation systems, autonomous driving, content generation, medical diagnostics, and scientific discovery. However, their widespread deployment also exposes them to significant safety risks, raising concerns about robustness, reliability, and ethical implications. This survey provides a systematic review of current safety research on large models, covering Vision Foundation Models (VFMs), Large Language Models (LLMs), Vision-Language Pre-training (VLP) models, Vision-Language Models (VLMs), Diffusion Models (DMs), and large-model-based Agents. Our contributions are summarized as follows: (1) We present a comprehensive taxonomy of safety threats to these models, including adversarial attacks, data poisoning, backdoor attacks, jailbreak and prompt injection attacks, energy-latency attacks, data and model extraction attacks, and emerging agent-specific threats. (2) We review defense strategies proposed for each type of attacks if available and summarize the commonly used datasets and benchmarks for safety research. (3) Building on this, we identify and discuss the open challenges in large model safety, emphasizing the need for comprehensive safety evaluations, scalable and effective defense mechanisms, and sustainable data practices. More importantly, we highlight the necessity of collective efforts from the research community and international collaboration. Our work can serve as a useful reference for researchers and practitioners, fostering the ongoing development of comprehensive defense systems and platforms to safeguard AI models.

SafetyDrift: Predicting When AI Agents Cross the Line Before They Actually Do

When an LLM agent reads a confidential file, then writes a summary, then emails it externally, no single step is unsafe, but the sequence is a data leak. We call this safety drift: individually safe actions compounding into violations. Prior work has measured this problem; we predict it. SafetyDrift models agent safety trajectories as absorbing Markov chains, computing the probability that a trajectory will reach a violation within a given number of steps via closed form absorption analysis. A consequence of the monotonic state design is that every agent will eventually violate safety if left unsupervised (absorption probability 1.0 from all states), making the practical question not if but when, and motivating our focus on finite horizon prediction. Across 357 traces spanning 40 realistic tasks in four categories, we discover that "points of no return" are sharply task dependent: in communication tasks, agents that reach even a mild risk state have an 85% chance of violating safety within five steps, while in technical tasks the probability stays below 5% from any state. A lightweight monitor built on these models detects 94.7% of violations with 3.7 steps of advance warning at negligible computational cost, outperforming both keyword matching (44.7% detection, 55.9% false positive rate) and per step LLM judges (52.6% detection, 38.2% false positive rate) while running over 60,000x faster.

Beyond Benchmarks: Dynamic, Automatic And Systematic Red-Teaming Agents For Trustworthy Medical Language Models

Ensuring the safety and reliability of large language models (LLMs) in clinical practice is critical to prevent patient harm and promote trustworthy healthcare applications of AI. However, LLMs are advancing so rapidly that static safety benchmarks often become obsolete upon publication, yielding only an incomplete and sometimes misleading picture of model trustworthiness. We demonstrate that a Dynamic, Automatic, and Systematic (DAS) red-teaming framework that continuously stress-tests LLMs can reveal significant weaknesses of current LLMs across four safety-critical domains: robustness, privacy, bias/fairness, and hallucination. A suite of adversarial agents is applied to autonomously mutate test cases, identify/evolve unsafe-triggering strategies, and evaluate responses, uncovering vulnerabilities in real time without human intervention. Applying DAS to 15 proprietary and open-source LLMs revealed a stark contrast between static benchmark performance and vulnerability under adversarial pressure. Despite a median MedQA accuracy exceeding 80\%, 94\% of previously correct answers failed our dynamic robustness tests. We observed similarly high failure rates across other domains: privacy leaks were elicited in 86\% of scenarios, cognitive-bias priming altered clinical recommendations in 81\% of fairness tests, and we identified hallucination rates exceeding 66\% in widely used models. Such profound residual risks are incompatible with routine clinical practice. By converting red-teaming from a static checklist into a dynamic stress-test audit, DAS red-teaming offers the surveillance that hospitals/regulators/technology vendors require as LLMs become embedded in patient chatbots, decision-support dashboards, and broader healthcare workflows. Our framework delivers an evolvable, scalable, and reliable safeguard for the next generation of medical AI.

Multimodal Safety Evaluation in Generative Agent Social Simulations

Can generative agents be trusted in multimodal environments? Despite advances in large language and vision-language models that enable agents to act autonomously and pursue goals in rich settings, their ability to reason about safety, coherence, and trust across modalities remains limited. We introduce a reproducible simulation framework for evaluating agents along three dimensions: (1) safety improvement over time, including iterative plan revisions in text-visual scenarios; (2) detection of unsafe activities across multiple categories of social situations; and (3) social dynamics, measured as interaction counts and acceptance ratios of social exchanges. Agents are equipped with layered memory, dynamic planning, multimodal perception, and are instrumented with SocialMetrics, a suite of behavioral and structural metrics that quantifies plan revisions, unsafe-to-safe conversions, and information diffusion across networks. Experiments show that while agents can detect direct multimodal contradictions, they often fail to align local revisions with global safety, reaching only a 55 percent success rate in correcting unsafe plans. Across eight simulation runs with three models - Claude, GPT-4o mini, and Qwen-VL - five agents achieved average unsafe-to-safe conversion rates of 75, 55, and 58 percent, respectively. Overall performance ranged from 20 percent in multi-risk scenarios with GPT-4o mini to 98 percent in localized contexts such as fire/heat with Claude. Notably, 45 percent of unsafe actions were accepted when paired with misleading visuals, showing a strong tendency to overtrust images. These findings expose critical limitations in current architectures and provide a reproducible platform for studying multimodal safety, coherence, and social dynamics.

SAFE-SIM: Safety-Critical Closed-Loop Traffic Simulation with Diffusion-Controllable Adversaries

Evaluating the performance of autonomous vehicle planning algorithms necessitates simulating long-tail safety-critical traffic scenarios. However, traditional methods for generating such scenarios often fall short in terms of controllability and realism; they also neglect the dynamics of agent interactions. To address these limitations, we introduce SAFE-SIM, a novel diffusion-based controllable closed-loop safety-critical simulation framework. Our approach yields two distinct advantages: 1) generating realistic long-tail safety-critical scenarios that closely reflect real-world conditions, and 2) providing controllable adversarial behavior for more comprehensive and interactive evaluations. We develop a novel approach to simulate safety-critical scenarios through an adversarial term in the denoising process of diffusion models, which allows an adversarial agent to challenge a planner with plausible maneuvers while all agents in the scene exhibit reactive and realistic behaviors. Furthermore, we propose novel guidance objectives and a partial diffusion process that enables users to control key aspects of the scenarios, such as the collision type and aggressiveness of the adversarial agent, while maintaining the realism of the behavior. We validate our framework empirically using the nuScenes and nuPlan datasets across multiple planners, demonstrating improvements in both realism and controllability. These findings affirm that diffusion models provide a robust and versatile foundation for safety-critical, interactive traffic simulation, extending their utility across the broader autonomous driving landscape. Building a Foundational Guardrail for General Agentic Systems via Synthetic Data

While LLM agents can plan multi-step tasks, intervening at the planning stage-before any action is executed-is often the safest way to prevent harm, since certain risks can lead to severe consequences once carried out. However, existing guardrails mostly operate post-execution, which is difficult to scale and leaves little room for controllable supervision at the plan level. To address this challenge, we highlight three critical gaps in current research: data gap, model gap, and evaluation gap. To close the data gap, we introduce AuraGen, a controllable engine that (i) synthesizes benign trajectories, (ii) injects category-labeled risks with calibrated difficulty, and (iii) filters outputs via an automated reward model, producing large and reliable corpora for pre-execution safety. To close the guardian model gap, we propose a foundational guardrail Safiron, combining a cross-planner adapter with a compact guardian model. The adapter unifies different input formats, while Safiron flags risky cases, assigns risk types, and generates rationales; trained in two stages with a broadly explored data recipe, Safiron achieves robust transfer across settings. To close the evaluation gap, we release Pre-Exec Bench, a realistic benchmark covering diverse tools and branching trajectories, which measures detection, fine-grained categorization, explanation, and cross-planner generalization in human-verified scenarios. Extensive experiments demonstrate consistent gains of the proposed guardrail over strong baselines on Pre-Exec Bench, and ablations further distill actionable practices, providing a practical template for safer agentic systems.

ForesightSafety Bench: A Frontier Risk Evaluation and Governance Framework towards Safe AI

Rapidly evolving AI exhibits increasingly strong autonomy and goal-directed capabilities, accompanied by derivative systemic risks that are more unpredictable, difficult to control, and potentially irreversible. However, current AI safety evaluation systems suffer from critical limitations such as restricted risk dimensions and failed frontier risk detection. The lagging safety benchmarks and alignment technologies can hardly address the complex challenges posed by cutting-edge AI models. To bridge this gap, we propose the "ForesightSafety Bench" AI Safety Evaluation Framework, beginning with 7 major Fundamental Safety pillars and progressively extends to advanced Embodied AI Safety, AI4Science Safety, Social and Environmental AI risks, Catastrophic and Existential Risks, as well as 8 critical industrial safety domains, forming a total of 94 refined risk dimensions. To date, the benchmark has accumulated tens of thousands of structured risk data points and assessment results, establishing a widely encompassing, hierarchically clear, and dynamically evolving AI safety evaluation framework. Based on this benchmark, we conduct systematic evaluation and in-depth analysis of over twenty mainstream advanced large models, identifying key risk patterns and their capability boundaries. The safety capability evaluation results reveals the widespread safety vulnerabilities of frontier AI across multiple pillars, particularly focusing on Risky Agentic Autonomy, AI4Science Safety, Embodied AI Safety, Social AI Safety and Catastrophic and Existential Risks. EgoSafetyBench: A Diagnostic Egocentric Video Benchmark for Evaluating Embodied VLMs as Runtime Safety Guards

Vision-language models (VLMs) are now proposed as runtime safety guards for embodied agents in homes and factories. A deployable guard must catch genuinely unsafe situations while avoiding unnecessary intervention on routine but superficially alarming activity, a distinction that binary safety benchmarks obscure. We introduce EgoSafetyBench, an egocentric video benchmark of 1,200 robot-view scenarios annotated at half-second granularity, to evaluate VLMs as streaming guards across two tracks. The situational track (800 scenarios) spans four families, from routine and safe-but-suspicious scenes to obvious and contextual hazards. The visual-channel track (400 scenarios) targets in-scene text-a sign, sticker, or label visible in the scene-that can misrepresent the physical situation, pairing each misleading sign with a truthful version to test both whether a guard flags the text as misleading and whether the text corrupts its physical-safety judgment. Both tracks use contrastive ladders: near-identical scenarios differing only in a single visible deciding cue, so a correct call must hinge on that cue rather than the overall scene type. We evaluate ten open- and closed-source VLMs. We find that while guards reliably recognize videos containing hazards, they often miss specific hazardous moments, particularly contextual hazards. Furthermore, misleading in-scene signs degrade all tested guards: vulnerable models miss up to a third of hazards, while robust models over-intervene on safe content. Matched controls reveal that apparent safety robustness often reflects indiscriminate alarming rather than true physical reasoning.

Aegis2.0: A Diverse AI Safety Dataset and Risks Taxonomy for Alignment of LLM Guardrails

As Large Language Models (LLMs) and generative AI become increasingly widespread, concerns about content safety have grown in parallel. Currently, there is a clear lack of high-quality, human-annotated datasets that address the full spectrum of LLM-related safety risks and are usable for commercial applications. To bridge this gap, we propose a comprehensive and adaptable taxonomy for categorizing safety risks, structured into 12 top-level hazard categories with an extension to 9 fine-grained subcategories. This taxonomy is designed to meet the diverse requirements of downstream users, offering more granular and flexible tools for managing various risk types. Using a hybrid data generation pipeline that combines human annotations with a multi-LLM "jury" system to assess the safety of responses, we obtain Aegis 2.0, a carefully curated collection of 34,248 samples of human-LLM interactions, annotated according to our proposed taxonomy. To validate its effectiveness, we demonstrate that several lightweight models, trained using parameter-efficient techniques on Aegis 2.0, achieve performance competitive with leading safety models fully fine-tuned on much larger, non-commercial datasets. In addition, we introduce a novel training blend that combines safety with topic following data.This approach enhances the adaptability of guard models, enabling them to generalize to new risk categories defined during inference. We plan to open-source Aegis 2.0 data and models to the research community to aid in the safety guardrailing of LLMs.

ADAS-TO: A Large-Scale Multimodal Naturalistic Dataset and Empirical Characterization of Human Takeovers during ADAS Engagement

Takeovers remain a key safety vulnerability in production ADAS, yet existing public resources rarely provide takeover-centered, real-world data. We present ADAS-TO, the first large-scale naturalistic dataset dedicated to ADAS-to-manual transitions, containing 15,659 takeover-centered 20s clips from 327 drivers across 22 vehicle brands. Each clip synchronizes front-view video with CAN logs. Takeovers are defined as ADAS ON rightarrow OFF transitions, with the primary trigger labeled as brake, steer, gas, mixed, or system disengagement. We further separate planned driver-initiated terminations (Ego) from forced takeovers (Non-ego) using a rule-based partition. While most events occur within conservative kinematic margins, we identify a long tail of 285 safety-critical cases. For these events, we combine kinematic screening with vision--language (VLM) annotation to attribute hazards and relate them to intervention dynamics. The resulting cross-modal analysis shows distinct kinematic signatures across traffic dynamics, infrastructure degradation, and adverse environments, and finds that in 59.3% of critical cases, actionable visual cues emerge at least 3s before takeover, supporting the potential for semantics-aware early warning beyond late-stage kinematic triggers. Personalized Safety in LLMs: A Benchmark and A Planning-Based Agent Approach

Large language models (LLMs) typically generate identical or similar responses for all users given the same prompt, posing serious safety risks in high-stakes applications where user vulnerabilities differ widely. Existing safety evaluations primarily rely on context-independent metrics - such as factuality, bias, or toxicity - overlooking the fact that the same response may carry divergent risks depending on the user's background or condition. We introduce personalized safety to fill this gap and present PENGUIN - a benchmark comprising 14,000 scenarios across seven sensitive domains with both context-rich and context-free variants. Evaluating six leading LLMs, we demonstrate that personalized user information significantly improves safety scores by 43.2%, confirming the effectiveness of personalization in safety alignment. However, not all context attributes contribute equally to safety enhancement. To address this, we develop RAISE - a training-free, two-stage agent framework that strategically acquires user-specific background. RAISE improves safety scores by up to 31.6% over six vanilla LLMs, while maintaining a low interaction cost of just 2.7 user queries on average. Our findings highlight the importance of selective information gathering in safety-critical domains and offer a practical solution for personalizing LLM responses without model retraining. This work establishes a foundation for safety research that adapts to individual user contexts rather than assuming a universal harm standard.

Real-time accident detection and physiological signal monitoring to enhance motorbike safety and emergency response

Rapid urbanization and improved living standards have led to a substantial increase in the number of vehicles on the road, consequently resulting in a rise in the frequency of accidents. Among these accidents, motorbike accidents pose a particularly high risk, often resulting in serious injuries or deaths. A significant number of these fatalities occur due to delayed or inadequate medical attention. To this end, we propose a novel automatic detection and notification system specifically designed for motorbike accidents. The proposed system comprises two key components: a detection system and a physiological signal monitoring system. The detection system is integrated into the helmet and consists of a microcontroller, accelerometer, GPS, GSM, and Wi-Fi modules. The physio-monitoring system incorporates a sensor for monitoring pulse rate and SpO_{2} saturation. All collected data are presented on an LCD display and wirelessly transmitted to the detection system through the microcontroller of the physiological signal monitoring system. If the accelerometer readings consistently deviate from the specified threshold decided through extensive experimentation, the system identifies the event as an accident and transmits the victim's information -- including the GPS location, pulse rate, and SpO_{2} saturation rate -- to the designated emergency contacts. Preliminary results demonstrate the efficacy of the proposed system in accurately detecting motorbike accidents and promptly alerting emergency contacts. We firmly believe that the proposed system has the potential to significantly mitigate the risks associated with motorbike accidents and save lives.

HomeGuard: VLM-based Embodied Safeguard for Identifying Contextual Risk in Household Task

Vision-Language Models (VLMs) empower embodied agents to execute complex instructions, yet they remain vulnerable to contextual safety risks where benign commands become hazardous due to subtle environmental states. Existing safeguards often prove inadequate. Rule-based methods lack scalability in object-dense scenes, whereas model-based approaches relying on prompt engineering suffer from unfocused perception, resulting in missed risks or hallucinations. To address this, we propose an architecture-agnostic safeguard featuring Context-Guided Chain-of-Thought (CG-CoT). This mechanism decomposes risk assessment into active perception that sequentially anchors attention to interaction targets and relevant spatial neighborhoods, followed by semantic judgment based on this visual evidence. We support this approach with a curated grounding dataset and a two-stage training strategy utilizing Reinforcement Fine-Tuning (RFT) with process rewards to enforce precise intermediate grounding. Experiments demonstrate that our model HomeGuard significantly enhances safety, improving risk match rates by over 30% compared to base models while reducing oversafety. Beyond hazard detection, the generated visual anchors serve as actionable spatial constraints for downstream planners, facilitating explicit collision avoidance and safety trajectory generation. MATRIX: Multi-Agent simulaTion fRamework for safe Interactions and conteXtual clinical conversational evaluation

Despite the growing use of large language models (LLMs) in clinical dialogue systems, existing evaluations focus on task completion or fluency, offering little insight into the behavioral and risk management requirements essential for safety-critical systems. This paper presents MATRIX (Multi-Agent simulaTion fRamework for safe Interactions and conteXtual clinical conversational evaluation), a structured, extensible framework for safety-oriented evaluation of clinical dialogue agents. MATRIX integrates three components: (1) a safety-aligned taxonomy of clinical scenarios, expected system behaviors and failure modes derived through structured safety engineering methods; (2) BehvJudge, an LLM-based evaluator for detecting safety-relevant dialogue failures, validated against expert clinician annotations; and (3) PatBot, a simulated patient agent capable of producing diverse, scenario-conditioned responses, evaluated for realism and behavioral fidelity with human factors expertise, and a patient-preference study. Across three experiments, we show that MATRIX enables systematic, scalable safety evaluation. BehvJudge with Gemini 2.5-Pro achieves expert-level hazard detection (F1 0.96, sensitivity 0.999), outperforming clinicians in a blinded assessment of 240 dialogues. We also conducted one of the first realism analyses of LLM-based patient simulation, showing that PatBot reliably simulates realistic patient behavior in quantitative and qualitative evaluations. Using MATRIX, we demonstrate its effectiveness in benchmarking five LLM agents across 2,100 simulated dialogues spanning 14 hazard scenarios and 10 clinical domains. MATRIX is the first framework to unify structured safety engineering with scalable, validated conversational AI evaluation, enabling regulator-aligned safety auditing. We release all evaluation tools, prompts, structured scenarios, and datasets.

Opir: Efficient Multi-Task Safety Classification for Toxicity, Jailbreaks, Hate Speech, and Harmful Content

Real-time safety filtering for large language model (LLM) applications requires classifiers that can detect unsafe prompts, toxic language, jailbreak attempts, and unsafe responses without the cost profile of large guardrail models, and that can distinguish benign sensitive text from genuinely covert harmful content. In this paper, we introduce Opir, a family of encoder-based guardrail models built on the GLiClass architecture. Opir includes multi-task models for binary safe/unsafe classification, multi-label toxicity classification, jailbreak classification, and zero-shot unsafe prompt and response categorization. We also release edge variants with fewer than 100M parameters dedicated to binary safe/unsafe categorization. The models are trained on a three-level taxonomy containing 996 categories across 16 top-level labels, 126 mid-level labels, and 854 leaf labels. Opir's training data combines taxonomy-grounded unsafe prompts, adversarially mined hard negatives, benign safety-preserving examples, generated response examples, multilingual translations, and portions of the Aegis2 and WildGuard training subsets. We also open-sourced an evaluation harness that supports GLiClass and GLiNER2 backends as well as decoder-based models, and covers binary safety classification, multi-label categorization, toxicity, jailbreak detection, prompt safety, response safety, response refusal, and prompt subcategory views across public benchmark families. Across an expanded comparison spanning 12 safety-classification tasks and 17 category tasks against eight contemporary guardrail systems -- including both GLiNER2-based and generative guardrail models -- Opir variants are competitive on or ahead of the strongest open-weight baselines on the majority of benchmark datasets while operating with a substantially smaller deployment footprint.

Towards Safety Reasoning in LLMs: AI-agentic Deliberation for Policy-embedded CoT Data Creation

Safety reasoning is a recent paradigm where LLMs reason over safety policies before generating responses, thereby mitigating limitations in existing safety measures such as over-refusal and jailbreak vulnerabilities. However, implementing this paradigm is challenging due to the resource-intensive process of creating high-quality policy-embedded chain-of-thought (CoT) datasets while ensuring reasoning remains accurate and free from hallucinations or policy conflicts. To tackle this, we propose AIDSAFE: Agentic Iterative Deliberation for Safety Reasoning, a novel data generation recipe that leverages multi-agent deliberation to iteratively expand reasoning on safety policies. A data refiner stage in AIDSAFE ensures high-quality outputs by eliminating repetitive, redundant, and deceptive thoughts. AIDSAFE-generated CoTs provide a strong foundation for supervised fine-tuning (SFT)-based safety training. Additionally, to address the need of preference data in alignment stages, such as DPO training, we introduce a supplemental recipe that uses belief augmentation to create distinct selected and rejected CoT samples. Our evaluations demonstrate that AIDSAFE-generated CoTs achieve superior policy adherence and reasoning quality. Consequently, we show that fine-tuning open-source LLMs on these CoTs can significantly improve safety generalization and jailbreak robustness while maintaining acceptable utility and over-refusal accuracy. Safe, or Simply Incapable? Rethinking Safety Evaluation for Phone-Use Agents

When a phone-use agent avoids harm, does that show safety, or simply inability to act? Existing evaluations often cannot tell. A harmful outcome may be avoided because the agent recognized the risk and chose the safe action, or because it failed to understand the screen or execute any relevant action at all. These cases have different causes and call for different fixes, yet current benchmarks often merge them under task success, refusal, or final harmful outcome. We address this problem with PhoneSafety, a benchmark of 700 safety-critical moments drawn from real phone interactions across more than 130 apps. Each instance isolates the next decision at a risky moment and asks a simple question: does the model take the safe action, take the unsafe action, or fail to do anything useful? We evaluate eight representative phone-use agents under this framework. Our results reveal two main patterns. First, stronger general phone-use ability does not reliably imply safer choices at risky moments. Models that perform better on ordinary app tasks are not always the ones that behave more safely when the next action matters. Second, failures to do anything useful behave like a capability signal rather than a safety signal: they are concentrated in more visually and operationally demanding settings and remain stable when the evaluation protocol changes. Across models, failures split into two recurring patterns: unsafe choices in settings where the model can act but chooses wrongly, and inability to act in more visually and operationally demanding screens. Overall, a harmless outcome is not enough to count as evidence of safety. Evaluating phone-use agents requires separating unsafe judgment from inability to act.

ProGuard: Towards Proactive Multimodal Safeguard

The rapid evolution of generative models has led to a continuous emergence of multimodal safety risks, exposing the limitations of existing defense methods. To address these challenges, we propose ProGuard, a vision-language proactive guard that identifies and describes out-of-distribution (OOD) safety risks without the need for model adjustments required by traditional reactive approaches. We first construct a modality-balanced dataset of 87K samples, each annotated with both binary safety labels and risk categories under a hierarchical multimodal safety taxonomy, effectively mitigating modality bias and ensuring consistent moderation across text, image, and text-image inputs. Based on this dataset, we train our vision-language base model purely through reinforcement learning (RL) to achieve efficient and concise reasoning. To approximate proactive safety scenarios in a controlled setting, we further introduce an OOD safety category inference task and augment the RL objective with a synonym-bank-based similarity reward that encourages the model to generate concise descriptions for unseen unsafe categories. Experimental results show that ProGuard achieves performance comparable to closed-source large models on binary safety classification, substantially outperforms existing open-source guard models on unsafe content categorization. Most notably, ProGuard delivers a strong proactive moderation ability, improving OOD risk detection by 52.6% and OOD risk description by 64.8%.
