Plans Don't Persist: Why Context Management Is Load Bearing for LLM Agents

Long-horizon agents depend on context management: systems compress, summarize, and evict old tokens so tasks can continue beyond finite windows. That is safe only when dropped information is no longer needed or has been internalized. Plans are the stress case: they are written early, used for many steps, and first to be evicted. We introduce replay pairing, a diagnostic that runs the same trajectory with and without the plan in history and measures hidden-state cosine distance. On Llama-3.1-70B, plan signal spikes to 0.453 one step after the plan, then falls 4.1x in a single action-observation step; HotpotQA falls 12.4x. This is evidence that standard LLM agents do not carry plans forward as persistent state, and instead depend on the plan remaining in context. A layer-L32 probe detects this decay as a diagnostic, not as proof that it reads plan content itself. Reasoning models add a measurement confound: their `<think>` traces re-derive plan content, so standard stripping leaves plan evidence in the stripped condition. We name this the reasoning-trace confound and fix it with strict stripping, which removes prior `<think>` blocks from the stripped run only. It recovers +163% of the step+1 signal in-sample and +153% held out, while not meaningfully changing non-reasoning Llama (+4.8%). On DeepSeek-R1-Distill-Llama-70B, a Llama-trained probe transfers at AUROC 0.748 (p=6e-4), while R1-specific probes reach 1.000, suggesting R1 encodes plan signal in a different hidden-state direction. Finally, a compression stress test shows the practical cost: naive plan eviction cuts ALFWorld success by 34.7pp, while probe-gated re-surfacing does not recover it. The contribution is a measurement and stress-test framework showing that agent-critical information can be context-resident rather than persistent. Context management is load bearing, but plan protection alone is not enough.

Reflective Prompt Tuning through Language Model Function-Calling

Large language models (LLMs) have become increasingly capable of following instructions and complex reasoning, making prompting a flexible interface for adapting models without parameter updates. Yet prompt design remains labor-intensive and highly sensitive to formatting, phrasing, and instruction order, motivating automated prompt optimization methods that reduce manual effort while preserving inference-time flexibility. However, existing methods often search over prompt candidates or use fixed critique-refine pipelines driven by individual examples or small batches, limiting their ability to capture systematic error patterns and make targeted edits grounded in failure history. We propose Reflective Prompt Tuning (RPT), a framework that uses LLM function calling to simulate the iterative workflow of human prompt engineers. An LLM optimizer calls a diagnostic function that evaluates the target model over an entire optimization set, summarizes recurring failure modes, and returns a structured diagnostic report. The optimizer uses this report, together with an accumulated memory of prior reports, to revise the prompt for the next iteration. RPT further supports confidence-aware optimization by using calibration signals in diagnostic feedback and final prompt selection. Across three reasoning tasks, RPT improves over initial prompts by up to 12.9 points, remains competitive with state of the art, and improves confidence calibration. Our analyses show that RPT is especially effective on multi-hop and mathematical reasoning, producing targeted prompt revisions that align with diagnosed failure patterns and lead to gains in task performance and calibration.

Evolving Diagnostic Agents in a Virtual Clinical Environment

In this paper, we present a framework for training large language models (LLMs) as diagnostic agents with reinforcement learning, enabling them to manage multi-turn diagnostic processes, adaptively select examinations, and commit to final diagnoses. Unlike instruction-tuned models trained on static case summaries, our method acquires diagnostic strategies through interactive exploration and outcome-based feedback. Our contributions are fourfold: (i) We present DiagGym, a diagnostics world model trained with electronic health records that emits examination outcomes conditioned on patient history and recommended examination, serving as a virtual clinical environment for realistic diagnosis training and evaluation; (ii) We train DiagAgent via end-to-end, multi-turn reinforcement learning to learn diagnostic policies that optimize both information yield and diagnostic accuracy; (iii) We introduce DiagBench, a diagnostic benchmark comprising 750 cases with physician-validated examination recommendations and 99 cases annotated with 973 physician-written rubrics on diagnosis process; (iv) we demonstrate superior performance across diverse diagnostic settings. DiagAgent significantly outperforms 10 state-of-the-art LLMs, including DeepSeek-v3 and GPT-4o, as well as two prompt-engineered agents. In single-turn settings, DiagAgent achieves 9.34% higher diagnostic accuracy and 44.03% improvement in examination recommendation hit ratio. In end-to-end settings, it delivers 15.12% increase in diagnostic accuracy and 23.09% boost in examination recommendation F1 score. In rubric-based evaluation, it surpasses the next-best model, Claude-sonnet-4, by 7.1% in weighted rubric score. These findings indicate that learning policies in interactive clinical environments confers dynamic and clinically meaningful diagnostic management abilities unattainable through passive training alone.

TRAJEVAL: Decomposing Code Agent Trajectories for Fine-Grained Diagnosis

Code agents can autonomously resolve GitHub issues, yet when they fail, current evaluation provides no visibility into where or why. Metrics such as Pass@1 collapse an entire execution into a single binary outcome, making it difficult to identify where and why the agent went wrong. To address this limitation, we introduce TRAJEVAL, a diagnostic framework that decomposes agent trajectories into three interpretable stages: search (file localization), read (function comprehension), and edit (modification targeting). For each stage, we compute precision and recall by comparing against reference patches. Analyzing 16,758 trajectories across three agent architectures and seven models, we find universal inefficiencies (all agents examine approximately 22x more functions than necessary) yet distinct failure modes: GPT-5 locates relevant code but targets edits incorrectly, while Qwen-32B fails at file discovery entirely. We validate that these diagnostics are predictive, achieving model-level Pass@1 prediction within 0.87-2.1% MAE, and actionable: real-time feedback based on trajectory signals improves two state-of-the-art models by 2.2-4.6 percentage points while reducing costs by 20-31%. These results demonstrate that our framework not only provides a more fine-grained analysis of agent behavior, but also translates diagnostic signals into tangible performance gains. More broadly, TRAJEVAL transforms agent evaluation beyond outcome-based benchmarking toward mechanism-driven diagnosis of agent success and failure.

HPCR: Holistic Proxy-based Contrastive Replay for Online Continual Learning

Online continual learning (OCL) aims to continuously learn new data from a single pass over the online data stream. It generally suffers from the catastrophic forgetting issue. Existing replay-based methods effectively alleviate this issue by replaying part of old data in a proxy-based or contrastive-based replay manner. In this paper, we conduct a comprehensive analysis of these two replay manners and find they can be complementary. Inspired by this finding, we propose a novel replay-based method called proxy-based contrastive replay (PCR), which replaces anchor-to-sample pairs with anchor-to-proxy pairs in the contrastive-based loss to alleviate the phenomenon of forgetting. Based on PCR, we further develop a more advanced method named holistic proxy-based contrastive replay (HPCR), which consists of three components. The contrastive component conditionally incorporates anchor-to-sample pairs to PCR, learning more fine-grained semantic information with a large training batch. The second is a temperature component that decouples the temperature coefficient into two parts based on their impacts on the gradient and sets different values for them to learn more novel knowledge. The third is a distillation component that constrains the learning process to keep more historical knowledge. Experiments on four datasets consistently demonstrate the superiority of HPCR over various state-of-the-art methods.

What Do Evolutionary Coding Agents Evolve?

Recent work pairs LLMs with evolutionary search to iteratively generate, modify, and select code using task-specific feedback. These systems have produced strong results in mathematical discovery and algorithm design, yet a fundamental question remains: what do they actually evolve? Progress is typically summarized by the best score a run reaches under a task-specific evaluator, but that score can reflect several different mechanisms: new algorithmic structure, re-tuning an existing strategy, recombining ideas already in the model's internal knowledge, or overfitting to the evaluator. Distinguishing these mechanisms requires inspecting the search process itself, not only its final outcome. We introduce EvoTrace, a dataset of evolutionary coding traces spanning four evolutionary frameworks, reasoning and non-reasoning models, and 16 tasks across mathematics and algorithm design. To analyze these traces, we develop EvoReplay, a replay-based methodology that reconstructs the local search states behind high-scoring solutions and tests controlled interventions, including adjusting constants, removing program components and substituting models or prompting contexts. We annotate every code edit in EvoTrace with one of nine recurring edit types using an LLM-as-judge pipeline validated against blind human re-annotation. Across EvoTrace, most score gains come from a small subset of these edit types. We further find a deterministic cycling pattern: about 30% of code lines added during search are byte-identical re-introductions of previously-deleted lines, present throughout nearly every run. These results show that benchmark gains in evolutionary coding agents can arise from qualitatively different mechanisms, only some of which correspond to new algorithmic structure. EvoTrace enables more diagnostic evaluation of evolutionary coding agents beyond final benchmark scores.

Learning on the Fly: Replay-Based Continual Object Perception for Indoor Drones

Autonomous agents such as indoor drones must learn new object classes in real-time while limiting catastrophic forgetting, motivating Class-Incremental Learning (CIL). However, most unmanned aerial vehicle (UAV) datasets focus on outdoor scenes and offer limited temporally coherent indoor videos. We introduce an indoor dataset of 14,400 frames capturing inter-drone and ground vehicle footage, annotated via a semi-automatic workflow with a 98.6% first-pass labeling agreement before final manual verification. Using this dataset, we benchmark 3 replay-based CIL strategies: Experience Replay (ER), Maximally Interfered Retrieval (MIR), and Forgetting-Aware Replay (FAR), using YOLOv11-nano as a resource-efficient detector for deployment-constrained UAV platforms. Under tight memory budgets (5-10% replay), FAR performs better than the rest, achieving an average accuracy (ACC, mAP_{50-95} across increments) of 82.96% with 5% replay. Gradient-weighted class activation mapping (Grad-CAM) analysis shows attention shifts across classes in mixed scenes, which is associated with reduced localization quality for drones. The experiments further demonstrate that replay-based continual learning can be effectively applied to edge aerial systems. Overall, this work contributes an indoor UAV video dataset with preserved temporal coherence and an evaluation of replay-based CIL under limited replay budgets. PreAct: Computer-Using Agents that Get Faster on Repeated Tasks

Computer-using agents drive real software through the screen -- clicking and typing -- but they solve every task from scratch: asked to repeat a task, an agent re-reads the screen, re-reasons every tap, and pays the full cost again. We present PreAct, which lets such an agent get faster on tasks it has done before. The first time it succeeds, PreAct compiles the run into a small state-machine program-states that check the screen, transitions that act-and on later runs replays it directly instead of invoking the agent 8.5-13x faster, with no per-step language-model calls. Replay is not blind: at each step PreAct checks that the screen matches what the program expects before acting, and hands control back to the agent the moment something is off. PreAct applies the same discipline when deciding what to keep: a freshly compiled program enters the store only if, re-run from a clean state, an independent evaluator confirms it solved the task-catching programs that replay to their last step yet leave the task undone. Across a mobile, a desktop, and a web benchmark, this store-time check separates repeated runs that improve from ones that degrade as faulty programs accumulate, worth 1.75-2.6 tasks per benchmark, the same direction on all three; a fallback that explores afresh when no program fits brings PreAct level with a strong record-and-replay baseline. We also report what did not matter: prompt wording, runtime guardrails, and whether a language model or a plain embedding retriever selects which program to reuse.

Prompt replay: speeding up grpo with on-policy reuse of high-signal prompts

Reinforcement learning with verifiable rewards (RLVR) plays a crucial role in expanding the capacities of LLM reasoning, but GRPO-style training is dominated by expensive rollouts and wastes compute on unusable prompts. We propose Prompt Replay, an overhead-free online data selection method for GRPO that reuses prompts only (not trajectories), to preserve on-policy optimization. After each step, we insert prompts with medium difficulty into a buffer, and prioritize prompts closer to a pass rate of 0.5 (half answers correct, half wrong) to maximize the advantage, thus learning signal. Training batches are formed by mixing reused prompts with fresh samples, with cooldown steps and max reuse times controlling aggressiveness vs risk of overfitting. Across multiple model families (Llama-3.2- 3B, Qwen3-8B) and training datasets (Dolci, Polaris), evaluated using average accuracy on six standard math benchmarks, Prompt Replay reduces zero-variance prompts, increases mean absolute advantage and shows faster initial accuracy gains. Yet, it plateaus and converges with the baseline, as too aggressive configuration was used. The method is most efficient when the rollouts are the primary bottleneck and the dataset is difficult for the model. We additionally observe that Qwen2.5-Math can exhibit spurious-reward effects that invalidates ablations, raising a warning signal for using it as a sole testbed for GRPO method research.

StarCraftImage: A Dataset For Prototyping Spatial Reasoning Methods For Multi-Agent Environments

Spatial reasoning tasks in multi-agent environments such as event prediction, agent type identification, or missing data imputation are important for multiple applications (e.g., autonomous surveillance over sensor networks and subtasks for reinforcement learning (RL)). StarCraft II game replays encode intelligent (and adversarial) multi-agent behavior and could provide a testbed for these tasks; however, extracting simple and standardized representations for prototyping these tasks is laborious and hinders reproducibility. In contrast, MNIST and CIFAR10, despite their extreme simplicity, have enabled rapid prototyping and reproducibility of ML methods. Following the simplicity of these datasets, we construct a benchmark spatial reasoning dataset based on StarCraft II replays that exhibit complex multi-agent behaviors, while still being as easy to use as MNIST and CIFAR10. Specifically, we carefully summarize a window of 255 consecutive game states to create 3.6 million summary images from 60,000 replays, including all relevant metadata such as game outcome and player races. We develop three formats of decreasing complexity: Hyperspectral images that include one channel for every unit type (similar to multispectral geospatial images), RGB images that mimic CIFAR10, and grayscale images that mimic MNIST. STT-Arena: A More Realistic Environment for Tool-Using with Spatio-Temporal Dynamics

Large language models (LLMs) deployed in real-world agentic applications must be capable of replanning and adapting when mid-task disruptions invalidate their prior decisions. Existing dynamic benchmarks primarily measure whether LLMs can detect temporal changes in a timely manner, leaving the complementary challenge of adaptive replanning under spatio-temporal dynamics largely unexplored. We introduce STT-Arena (Spatio-Temporal Tool-Use Arena), a benchmark of 227 high-quality interactive tasks spanning nine spatio-temporal conflict types and four solvability levels. Each task is grounded in a realistic, executable environment equipped with injected spatio-temporal triggers that can abruptly invalidate an ongoing plan, forcing the model to detect the state shift and construct a revised execution strategy. Extensive evaluation of frontier LLMs reveals that even the SOTA proprietary models, including Claude-4.6-Opus, achieves less than 40\% overall accuracies, highlighting the fundamental difficulty of spatio-temporal dynamic reasoning. Systematic analysis of failure trajectories uncovers three recurring error modes of existing models: Stale-State Execution, Misdiagnosis of Dynamic Triggers, and Missing Post-Adaptation Verification. Guided by these findings, we propose an iterative trajectory refinement technique that eliminates these failure patterns from training data, and combine it with online RL to produce STT-Agent-4B which outperforms frontier LLMs on STT-Arena.

Echo-Memory: A Controlled Study of Memory in Action World Models

We present Echo-Memory, a controlled study of memory mechanisms in action-conditioned world models. These models generate multi-segment videos from a first frame, text prompt, and camera-action sequence, but their central failure is often memory rather than local image synthesis: after the camera leaves and returns, the scene or salient object may silently change. Existing memory designs are hard to compare because gains are entangled with backbone, training, retrieval, and evaluation differences. Echo-Memory fixes the action-to-video interface and varies only how history is stored and read by the generator. Under a shared video diffusion backbone, optimizer, camera-action representation, sampler, and evaluation pipeline, we compare raw context, compression-based memory, spatial summaries with different read-out paths, and state-space recurrence. This matched matrix separates four otherwise conflated axes: capacity, compression, read-out, and recurrence. We also evaluate memory through a three-branch protocol: replay quality, in-domain loop revisit, and open-domain return probes. The branches routinely disagree, showing that replay fidelity is not a sufficient proxy for remembering a world. Three findings follow. Raw context is a strong capacity baseline and improves open-domain return far more than it improves replay metrics. Compactness is not a free substitute for capacity: aggressive spatial and hybrid-compression memories lose the salient evidence needed for return. Finally, block-wise state-space recurrence is the strongest open-domain return mechanism in our matrix, showing that the structure of implicit memory matters as much as the decision to use it. These results provide a compact protocol for studying memory in action world models beyond isolated replay metrics.

VeriLLMed: Interactive Visual Debugging of Medical Large Language Models with Knowledge Graphs

Large language models (LLMs) show promise in medical diagnosis, but real-world deployment remains challenging due to high-stakes clinical decisions and imperfect reasoning reliability. As a result, careful inspection of model behavior is essential for assessing whether diagnostic reasoning is reliable and clinically grounded. However, debugging medical LLMs remains difficult. First, developers often lack sufficient medical domain expertise to interpret model errors in clinically meaningful terms. Second, models can fail across a large and diverse set of instances involving different input types, tasks, and reasoning steps, making it challenging for developers to prioritize which errors deserve focused inspection. Third, developers struggle to identify recurring error patterns across cases, as existing debugging practices are largely instance-centric and rely on manual inspection of isolated failures. To address these challenges, we present VeriLLMed, a visual analytics system that integrates external biomedical knowledge to audit and debug medical LLM diagnostic reasoning. VeriLLMed transforms model outputs into comparable reasoning paths, constructs knowledge graph-grounded reference paths, and identifies three recurring classes of diagnosis errors: relation errors, branch errors, and missing errors. Case studies and expert evaluation demonstrate that VeriLLMed helps developers identify clinically implausible reasoning and generate actionable insights that can inform the improvement of medical LLMs.

Memory in Large Language Models: Mechanisms, Evaluation and Evolution

Under a unified operational definition, we define LLM memory as a persistent state written during pretraining, finetuning, or inference that can later be addressed and that stably influences outputs. We propose a four-part taxonomy (parametric, contextual, external, procedural/episodic) and a memory quadruple (location, persistence, write/access path, controllability). We link mechanism, evaluation, and governance via the chain write -> read -> inhibit/update. To avoid distorted comparisons across heterogeneous setups, we adopt a three-setting protocol (parametric only, offline retrieval, online retrieval) that decouples capability from information availability on the same data and timeline. On this basis we build a layered evaluation: parametric (closed-book recall, edit differential, memorization/privacy), contextual (position curves and the mid-sequence drop), external (answer correctness vs snippet attribution/faithfulness), and procedural/episodic (cross-session consistency and timeline replay, E MARS+). The framework integrates temporal governance and leakage auditing (freshness hits, outdated answers, refusal slices) and uncertainty reporting via inter-rater agreement plus paired tests with multiple-comparison correction. For updating and forgetting, we present DMM Gov: coordinating DAPT/TAPT, PEFT, model editing (ROME, MEND, MEMIT, SERAC), and RAG to form an auditable loop covering admission thresholds, rollout, monitoring, rollback, and change audits, with specs for timeliness, conflict handling, and long-horizon consistency. Finally, we give four testable propositions: minimum identifiability; a minimal evaluation card; causally constrained editing with verifiable forgetting; and when retrieval with small-window replay outperforms ultra-long-context reading. This yields a reproducible, comparable, and governable coordinate system for research and deployment.

TraceCoder: A Trace-Driven Multi-Agent Framework for Automated Debugging of LLM-Generated Code

Large Language Models (LLMs) often generate code with subtle but critical bugs, especially for complex tasks. Existing automated repair methods typically rely on superficial pass/fail signals, offering limited visibility into program behavior and hindering precise error localization. In addition, without a way to learn from prior failures, repair processes often fall into repetitive and inefficient cycles. To overcome these challenges, we present TraceCoder, a collaborative multi-agent framework that emulates the observe-analyze-repair process of human experts. The framework first instruments the code with diagnostic probes to capture fine-grained runtime traces, enabling deep insight into its internal execution. It then conducts causal analysis on these traces to accurately identify the root cause of the failure. This process is further enhanced by a novel Historical Lesson Learning Mechanism (HLLM), which distills insights from prior failed repair attempts to inform subsequent correction strategies and prevent recurrence of similar mistakes. To ensure stable convergence, a Rollback Mechanism enforces that each repair iteration constitutes a strict improvement toward the correct solution. Comprehensive experiments across multiple benchmarks show that TraceCoder achieves up to a 34.43\% relative improvement in Pass@1 accuracy over existing advanced baselines. Ablation studies verify the significance of each system component, with the iterative repair process alone contributing a 65.61\% relative gain in accuracy. Furthermore, TraceCoder significantly outperforms leading iterative methods in terms of both accuracy and cost-efficiency.

Shattering the Shortcut: A Topology-Regularized Benchmark for Multi-hop Medical Reasoning in LLMs

While Large Language Models (LLMs) achieve expert-level performance on standard medical benchmarks through single-hop factual recall, they severely struggle with the complex, multi-hop diagnostic reasoning required in real-world clinical settings. A primary obstacle is "shortcut learning", where models exploit highly connected, generic hub nodes (e.g., "inflammation") in knowledge graphs to bypass authentic micro-pathological cascades. To address this, we introduce ShatterMed-QA, a bilingual benchmark of 10,558 multi-hop clinical questions designed to rigorously evaluate deep diagnostic reasoning. Our framework constructs a topology-regularized medical Knowledge Graph using a novel k-Shattering algorithm, which physically prunes generic hubs to explicitly sever logical shortcuts. We synthesize the evaluation vignettes by applying implicit bridge entity masking and topology-driven hard negative sampling, forcing models to navigate biologically plausible distractors without relying on superficial elimination. Comprehensive evaluations of 21 LLMs reveal massive performance degradation on our multi-hop tasks, particularly among domain-specific models. Crucially, restoring the masked evidence via Retrieval-Augmented Generation (RAG) triggers near-universal performance recovery, validating ShatterMed-QA's structural fidelity and proving its efficacy in diagnosing the fundamental reasoning deficits of current medical AI. Demystifying Catastrophic Forgetting in Two-Stage Incremental Object Detector

Catastrophic forgetting is a critical chanllenge for incremental object detection (IOD). Most existing methods treat the detector monolithically, relying on instance replay or knowledge distillation without analyzing component-specific forgetting. Through dissection of Faster R-CNN, we reveal a key insight: Catastrophic forgetting is predominantly localized to the RoI Head classifier, while regressors retain robustness across incremental stages. This finding challenges conventional assumptions, motivating us to develop a framework termed NSGP-RePRE. Regional Prototype Replay (RePRE) mitigates classifier forgetting via replay of two types of prototypes: coarse prototypes represent class-wise semantic centers of RoI features, while fine-grained prototypes model intra-class variations. Null Space Gradient Projection (NSGP) is further introduced to eliminate prototype-feature misalignment by updating the feature extractor in directions orthogonal to subspace of old inputs via gradient projection, aligning RePRE with incremental learning dynamics. Our simple yet effective design allows NSGP-RePRE to achieve state-of-the-art performance on the Pascal VOC and MS COCO datasets under various settings. From Accidents to Insights: Leveraging Multimodal Data for Scenario-Driven ADS Testing

The rapid advancements in Autonomous Driving Systems (ADS) have necessitated robust software testing to ensure safety and reliability. However, automating the generation of scalable and concrete test scenarios remains a significant challenge. Current scenario-based test case generation methods often face limitations, such as unrealistic scenes and inaccurate vehicle trajectories. These challenges largely result from the loss of map information during data extraction and the lack of an effective verification mechanism to mitigate hallucinations in large language models (LLMs). This paper introduces TRACE, a scenario-based ADS Test case Generation framework for Critical Scenarios. By leveraging multimodal data to extract challenging scenarios from real-world car crash reports, TRACE constructs numerous critical test cases with less data, significantly enhancing ADS bug detection efficiency. Using in-context learning, chain-of-thought prompting, and self-validation approaches, we use LLMs to extract environmental and road network information from crash reports. For vehicle trajectory planning, data containing map information and vehicle coordinates serves as a knowledge base to build a ChatGPT-based LLM with path-planning capabilities, which we named TrackMate. Based on 50 existing crash reports, our approach successfully tested three ADS models across two simulation platforms, MetaDrive and BeamNG. Of the 290 constructed test scenarios, 127 are identified as critical, as they resulted in vehicle collisions. Additionally, user feedback reveals that TRACE demonstrates superior scenario reconstruction accuracy, with 77.5% of the scenarios being rated as 'mostly or 'totally' consistent, compared to only 27% for the most related SOTA, LCTGen.

Rewind-IL: Online Failure Detection and State Respawning for Imitation Learning

Imitation learning has enabled robots to acquire complex visuomotor manipulation skills from demonstrations, but deployment failures remain a major obstacle, especially for long-horizon action-chunked policies. Once execution drifts off the demonstration manifold, these policies often continue producing locally plausible actions without recovering from the failure. Existing runtime monitors either require failure data, over-trigger under benign feature drift, or stop at failure detection without providing a recovery mechanism. We present Rewind-IL, a training-free online safeguard framework for generative action-chunked imitation policies. Rewind-IL combines a zero-shot failure detector based on Temporal Inter-chunk Discrepancy Estimate (TIDE), calibrated with split conformal prediction, with a state-respawning mechanism that returns the robot to a semantically verified safe intermediate state. Offline, a vision-language model identifies recovery checkpoints in demonstrations, and the frozen policy encoder is used to construct a compact checkpoint feature database. Online, Rewind-IL monitors self-consistency in overlapping action chunks, tracks similarity to the checkpoint library, and, upon failure, rewinds execution to the latest verified safe state before restarting inference from a clean policy state. Experiments on real-world and simulated long-horizon manipulation tasks, including transfer to flow-matching action-chunked policies, demonstrate that policy-internal consistency coupled with semantically grounded respawning offers a practical route to improved reliability in imitation learning. Dynamic Proxy-Mixing: Transferring Replay Controllers from Small to Large Models for Continual Instruction Tuning

Continual instruction tuning updates a language model through a sequence of new domains, yet each update can progressively erode previously learned capabilities and alignment behavior. Replay is the standard mitigation, but fixed replay ratios are inherently limited because the optimal mixture varies with the current domain, the training stage, and the evolving vulnerability of prior behaviors. We propose PROX-YMIX, a framework that learns a dynamic replay controller on a small proxy model and transfers the frozen controller to a larger target. The controller never observes future tasks and constructs its state from normalized validation losses and their temporal dynamics, producing a masked mixture over the current task and accessible replay buffers. Our core empirical hypothesis is forgetting mirroring: task vulnerability rankings remain largely consistent across model scales even when absolute loss magnitudes differ. We validate this assumption empirically before transferring controllers across scales. On LLaMA-3-8B across five continual instruction tuning sequences, PROXYMIX improves average accuracy by 3.4 points, reduces final forgetting by 3.5 points, and raises safety score by 5.8 points over the strongest non-oracle baseline, at roughly 50x lower policy learning cost than Oracle Target RL. The framework is leakage free and architecture independent at the interface level, and we also identify settings where the proxy assumption breaks down, highlighting limitations for robust deployment.

MalCL: Leveraging GAN-Based Generative Replay to Combat Catastrophic Forgetting in Malware Classification

Continual Learning (CL) for malware classification tackles the rapidly evolving nature of malware threats and the frequent emergence of new types. Generative Replay (GR)-based CL systems utilize a generative model to produce synthetic versions of past data, which are then combined with new data to retrain the primary model. Traditional machine learning techniques in this domain often struggle with catastrophic forgetting, where a model's performance on old data degrades over time. In this paper, we introduce a GR-based CL system that employs Generative Adversarial Networks (GANs) with feature matching loss to generate high-quality malware samples. Additionally, we implement innovative selection schemes for replay samples based on the model's hidden representations. Our comprehensive evaluation across Windows and Android malware datasets in a class-incremental learning scenario -- where new classes are introduced continuously over multiple tasks -- demonstrates substantial performance improvements over previous methods. For example, our system achieves an average accuracy of 55% on Windows malware samples, significantly outperforming other GR-based models by 28%. This study provides practical insights for advancing GR-based malware classification systems. GeRe: Towards Efficient Anti-Forgetting in Continual Learning of LLM via General Samples Replay

The continual learning capability of large language models (LLMs) is crucial for advancing artificial general intelligence. However, continual fine-tuning LLMs across various domains often suffers from catastrophic forgetting, characterized by: 1) significant forgetting of their general capabilities, and 2) sharp performance declines in previously learned tasks. To simultaneously address both issues in a simple yet stable manner, we propose General Sample Replay (GeRe), a framework that use usual pretraining texts for efficient anti-forgetting. Beyond revisiting the most prevalent replay-based practices under GeRe, we further leverage neural states to introduce a enhanced activation states constrained optimization method using threshold-based margin (TM) loss, which maintains activation state consistency during replay learning. We are the first to validate that a small, fixed set of pre-collected general replay samples is sufficient to resolve both concerns--retaining general capabilities while promoting overall performance across sequential tasks. Indeed, the former can inherently facilitate the latter. Through controlled experiments, we systematically compare TM with different replay strategies under the GeRe framework, including vanilla label fitting, logit imitation via KL divergence and feature imitation via L1/L2 losses. Results demonstrate that TM consistently improves performance and exhibits better robustness. Our work paves the way for efficient replay of LLMs for the future. Transform-Invariant Generative Ray Path Sampling for Efficient Radio Propagation Modeling

Ray tracing has become a standard for accurate radio propagation modeling, but suffers from exponential computational complexity, as the number of candidate paths scales with the number of objects raised to the power of the interaction order. This bottleneck limits its use in large-scale or real-time applications, forcing traditional tools to rely on heuristics to reduce the number of path candidates at the cost of potentially reduced accuracy. To overcome this limitation, we propose a comprehensive machine-learning-assisted framework that replaces exhaustive path searching with intelligent sampling via Generative Flow Networks. Applying such generative models to this domain presents significant challenges, particularly sparse rewards due to the rarity of valid paths, which can lead to convergence failures and trivial solutions when evaluating high-order interactions in complex environments. To ensure robust learning and efficient exploration, our framework incorporates three key architectural components. First, we implement an experience replay buffer to capture and retain rare valid paths. Second, we adopt a uniform exploratory policy to improve generalization and prevent the model from overfitting to simple geometries. Third, we apply a physics-based action masking strategy that filters out physically impossible paths before the model even considers them. As demonstrated in our experimental validation, the proposed model achieves substantial speedups over exhaustive search -- up to 10times faster on GPU and 1000times faster on CPU -- while maintaining high coverage accuracy and successfully uncovering complex propagation paths. Beyond Reasoning Gains: Mitigating General Capabilities Forgetting in Large Reasoning Models

Reinforcement learning with verifiable rewards (RLVR) has delivered impressive gains in mathematical and multimodal reasoning and has become a standard post-training paradigm for contemporary language and vision-language models. However, the RLVR recipe introduces a significant risk of capability regression, where models forget foundational skills after prolonged training without employing regularization strategies. We empirically confirm this concern, observing that open-source reasoning models suffer performance degradation on core capabilities such as perception and faithfulness. While imposing regularization terms like KL divergence can help prevent deviation from the base model, these terms are calculated on the current task, thus they do not guarantee broader knowledge. Meanwhile, commonly used experience replay across heterogeneous domains makes it nontrivial to decide how much training focus each objective should receive. To address this, we propose RECAP-a replay strategy with dynamic objective reweighting for general knowledge preservation. Our reweighting mechanism adapts in an online manner using short-horizon signals of convergence and instability, shifting the post-training focus away from saturated objectives and toward underperforming or volatile ones. Our method is end-to-end and readily applicable to existing RLVR pipelines without training additional models or heavy tuning. Extensive experiments on benchmarks based on Qwen2.5-VL-3B and Qwen2.5-VL-7B demonstrate the effectiveness of our method, which not only preserves general capabilities but also improves reasoning by enabling more flexible trade-offs among in-task rewards.

ContraPrompt: Contrastive Prompt Optimization via Dyadic Reasoning Trace Analysis

Prompt optimization methods either analyze individual failures in isolation or compare prompt variants across examples, operating on single execution traces with no access to the reasoning process distinguishing success from failure on the same input. We introduce ContraPrompt, built on the observation that when a model fails but succeeds on a retry with feedback, the difference between its two chain-of-thought traces constitutes an optimization signal not captured by prior methods. Unlike prior contrastive methods, we compare complete intermediate reasoning processes: the two traces share model, input, and base prompt, so remaining differences reflect reasoning strategy and appended error feedback -- we call this dyadic reasoning trace analysis. The multi-attempt solving phase is an instrumented agentic retry loop that generates contrastive data automatically without human annotation. Extracted rules are organized into an input-aware decision tree routing instructions by observable input characteristics. On four reasoning and compliance benchmarks, ContraPrompt outperforms GEPA (Agrawal et al., 2026) on all four, with absolute gains of +8.29 pp on HotPotQA (+20.8% rel.), +2.21 pp on GDPR-Bench (+18.2% rel.), +7.14 pp on GPQA Diamond (+10.6% rel.), and +0.74 pp on BBH (+0.85% rel.). Ablations confirm dyadic trace contrastivity is the critical component, with a -16% relative average drop upon its removal. On 53 EvalSet black-box optimization problems, ContraPrompt beats GEPA on 11, ties on 41, and loses on 1 at equal budget. On FiNER-139 financial named entity recognition (Loukas et al., 2022), ContraPrompt achieves +7.77 pp over the unoptimized baseline (+11.6% rel.) and +1.94 pp over GEPA (+2.66% rel.), with branch conditions aligning with standard US GAAP financial-instrument categories.

CodeTracer: Towards Traceable Agent States

Code agents are advancing rapidly, but debugging them is becoming increasingly difficult. As frameworks orchestrate parallel tool calls and multi-stage workflows over complex tasks, making the agent's state transitions and error propagation hard to observe. In these runs, an early misstep can trap the agent in unproductive loops or even cascade into fundamental errors, forming hidden error chains that make it hard to tell when the agent goes off track and why. Existing agent tracing analyses either focus on simple interaction or rely on small-scale manual inspection, which limits their scalability and usefulness for real coding workflows. We present CodeTracer, a tracing architecture that parses heterogeneous run artifacts through evolving extractors, reconstructs the full state transition history as a hierarchical trace tree with persistent memory, and performs failure onset localization to pinpoint the failure origin and its downstream chain. To enable systematic evaluation, we construct CodeTraceBench from a large collection of executed trajectories generated by four widely used code agent frameworks on diverse code tasks (e.g., bug fixing, refactoring, and terminal interaction), with supervision at both the stage and step levels for failure localization. Experiments show that CodeTracer substantially outperforms direct prompting and lightweight baselines, and that replaying its diagnostic signals consistently recovers originally failed runs under matched budgets. Time to REFLECT: Can We Trust LLM Judges for Evidence-based Research Agents?

Deep research agents increasingly automate complex information-seeking tasks, producing evidence-grounded reports via multi-step reasoning, tool use, and synthesis. Their growing role demands scalable, reliable evaluation, positioning LLM-as-judge as a supervision paradigm for assessing factual accuracy, evidence use, and reasoning quality. Yet the reliability of these judges for deep research agents remains poorly understood, posing a critical meta-evaluation problem: before deploying LLM judges to supervise research agents, we must first evaluate the judges themselves. Existing meta-evaluations fall short in two ways: (1) reliance on coarse, subjective human-preference agreement; (2) focus on instruction-following or verifiable tasks, leaving open-ended agent executions unexplored. To address these gaps, we introduce REFLECT (REliable Fine-grained LLM judge Evaluation via Controlled inTervention), a meta-evaluation benchmark targeting fine-grained failure detection in agentic environments. REFLECT defines a detailed taxonomy of process- and outcome-level failure modes, instantiated by performing controlled and localized interventions on quality-screened agent execution traces. This yields verifiable, comprehensive, and fine-grained instances for validating the judge models. Our experiments show that current LLM judges remain unreliable: even the best-performing models achieve overall accuracies below 55% across reasoning, tool-use, and report-quality failures, with especially poor performance on evidence verification. Together, our taxonomy and findings expose systematic judge limitations, reveal tradeoffs in cost and reliability, and offer actionable guidance for building more reliable evaluation pipelines for deep research agents.

Generate, Filter, Control, Replay: A Comprehensive Survey of Rollout Strategies for LLM Reinforcement Learning

Reinforcement learning (RL) has become a central post-training tool for improving the reasoning abilities of large language models (LLMs). In these systems, the rollout, the trajectory sampled from a prompt to termination, including intermediate reasoning steps and optional tool or environment interactions, determines the data the optimizer learns from, yet rollout design is often underreported. This survey provides an optimizer-agnostic view of rollout strategies for RL-based post-training of reasoning LLMs. We formalize rollout pipelines with unified notation and introduce Generate-Filter-Control-Replay (GFCR), a lifecycle taxonomy that decomposes rollout pipelines into four modular stages: Generate proposes candidate trajectories and topologies; Filter constructs intermediate signals via verifiers, judges, critics; Control allocates compute and makes continuation/branching/stopping decisions under budgets; and Replay retains and reuses artifacts across rollouts without weight updates, including self-evolving curricula that autonomously generate new training tasks. We complement GFCR with a criterion taxonomy of reliability, coverage, and cost sensitivity that characterizes rollout trade-offs. Using this framework, we synthesize methods spanning RL with verifiable rewards, process supervision, judge-based gating, guided and tree/segment rollouts, adaptive compute allocation, early-exit and partial rollouts, throughput optimization, and replay/recomposition for self-improvement. We ground the framework with case studies in math, code/SQL, multimodal reasoning, tool-using agents, and agentic skill benchmarks that evaluate skill induction, reuse, and cross-task transfer. Finally, we provide a diagnostic index that maps common rollout pathologies to GFCR modules and mitigation levers, alongside open challenges for building reproducible, compute-efficient, and trustworthy rollout pipelines.

Pressure-Testing Deception Probes in LLMs: Scaling, Robustness, and the Geometry of Deceptive Representations

Linear probes trained on LLM activations are increasingly proposed as deception-detection metrics, yet report AUROC exceeding 0.96 on clean benchmarks while collapsing under distributional shift. This paper systematically pressure-tests probe-based metrics across the Gemma 3 model family (1B-27B parameters), diagnosing why they fail rather than merely documenting that they fail. We test four hypotheses about deception encoding: (1) single linear direction, (2) multi-dimensional subspace, (3) convex conic hull, (4) entropy proxy. Our design includes cross-domain transfer matrices, multi-dimensional probe analysis with permutation null baselines, entropy-residualization tests, and distractor evaluations across 8 stylistic shifts. We find that: (a) probes achieve near-perfect AUROC (>=0.998) on clean data but collapse under stylistic shifts; style-augmented probes recover near-perfect detection (mean AUROC 0.979-0.983) on unseen styles; (b) the single-direction hypothesis is rejected (k=1 captures only 0.61-0.80 AUROC), with cross-domain transfer failure confirmed as geometric rather than layer-mismatch-driven; (c) the entropy-proxy hypothesis is rejected (max |rho|=0.454, max Delta-AUROC after residualization=0.004); and (d) deception does not form a significant linear subspace (per-domain k*=0), yet multi-dimensional probes (k>=5) recover the signal through distributed sub-threshold features. Probe fragility reflects distributional narrowness rather than an architectural limitation: style-augmented probes recover near-perfect detection at both 4B and 27B, establishing that the inverse scaling pattern is a training-distribution artifact rather than a genuine scale-dependent phenomenon.

SWE-Replay: Efficient Test-Time Scaling for Software Engineering Agents

Test-time scaling has been widely adopted to enhance the capabilities of Large Language Model (LLM) agents in software engineering (SWE) tasks. However, the standard approach of repeatedly sampling trajectories from scratch is computationally expensive. While recent methods have attempted to mitigate costs using specialized value agents, they can suffer from model miscalibration and fail to generalize to modern agents that synthesize custom bash scripts as tools. In this paper, we introduce SWE-Replay, the first efficient and generalizable test-time scaling technique for modern agents without reliance on potentially noisy value estimates. SWE-Replay optimizes the scaling process by recycling trajectories from prior trials, dynamically choosing to either explore from scratch or exploit archived experience by branching at critical intermediate steps. This selection of intermediate steps is driven by the potential and reasoning significance of repository exploration, rather than external LLM-based quality estimates. Our evaluation shows that, on SWE-Bench Verified, SWE-Replay consistently outperforms naive scaling, reducing costs by up to 17.4% while maintaining or even improving performance by up to 3.8%. Further evaluation on SWE-Bench Pro and Multilingual validates the generalizability of SWE-Replay, establishing it as a robust foundation for efficient test-time scaling of software engineering agents.

Synthetic Tabular Generators Fail to Preserve Behavioral Fraud Patterns: A Benchmark on Temporal, Velocity, and Multi-Account Signals

We introduce behavioral fidelity -- a third evaluation dimension for synthetic tabular data that measures whether generated data preserves the temporal, sequential, and structural behavioral patterns that distinguish real-world entity activity. Existing frameworks evaluate statistical fidelity (marginal distributions and correlations) and downstream utility (classifier AUROC on synthetic-trained models), but neither tests for the behavioral signals that operational detection and analysis systems actually rely on. We formalize a taxonomy of four behavioral fraud patterns (P1-P4) covering inter-event timing, burst structure, multi-account graph motifs, and velocity-rule trigger rates; define a degradation ratio metric calibrated to a real-data noise floor (1.0 = matches real variability, k = k-times worse); and prove that row-independent generators -- the dominant paradigm -- are structurally incapable of reproducing P3 graph motifs (Proposition 1) and produce non-positive within-entity IET autocorrelation (Proposition 2), making the positive burst fingerprint of fraud sequences unachievable regardless of architecture or training data size. We benchmark CTGAN, TVAE, GaussianCopula, and TabularARGN on IEEE-CIS Fraud Detection and the Amazon Fraud Dataset. All four fail severely: on IEEE-CIS composite degradation ratios range from 24.4x (TVAE) to 39.0x (GaussianCopula); on Amazon FDB, row-independent generators score 81.6-99.7x, while TabularARGN achieves 17.2x. We document generator-specific failure modes and their resolutions. The P1-P4 framework extends to any domain with entity-level sequential tabular data, including healthcare and network security. We release our evaluation framework as open source.

Expanding continual few-shot learning benchmarks to include recognition of specific instances

Continual learning and few-shot learning are important frontiers in progress towards broader Machine Learning (ML) capabilities. There is a growing body of work in both, but few works combining the two. One exception is the Continual few-shot Learning (CFSL) framework of Antoniou et al. arXiv:2004.11967. In this study, we extend CFSL in two ways that capture a broader range of challenges, important for intelligent agent behaviour in real-world conditions. First, we modify CFSL to make it more comparable to standard continual learning experiments, where usually a much larger number of classes are presented. Second, we introduce an 'instance test' which requires recognition of specific instances of classes -- a capability of animal cognition that is usually neglected in ML. For an initial exploration of ML model performance under these conditions, we selected representative baseline models from the original CFSL work and added a model variant with replay. As expected, learning more classes is more difficult than the original CFSL experiments, and interestingly, the way in which image instances and classes are presented affects classification performance. Surprisingly, accuracy in the baseline instance test is comparable to other classification tasks, but poor given significant occlusion and noise. The use of replay for consolidation improves performance substantially for both types of tasks, but particularly the instance test.

Disentangled Causal Graph Learning for Online Unsupervised Root Cause Analysis

The task of root cause analysis (RCA) is to identify the root causes of system faults/failures by analyzing system monitoring data. Efficient RCA can greatly accelerate system failure recovery and mitigate system damages or financial losses. However, previous research has mostly focused on developing offline RCA algorithms, which often require manually initiating the RCA process, a significant amount of time and data to train a robust model, and then being retrained from scratch for a new system fault. In this paper, we propose CORAL, a novel online RCA framework that can automatically trigger the RCA process and incrementally update the RCA model. CORAL consists of Trigger Point Detection, Incremental Disentangled Causal Graph Learning, and Network Propagation-based Root Cause Localization. The Trigger Point Detection component aims to detect system state transitions automatically and in near-real-time. To achieve this, we develop an online trigger point detection approach based on multivariate singular spectrum analysis and cumulative sum statistics. To efficiently update the RCA model, we propose an incremental disentangled causal graph learning approach to decouple the state-invariant and state-dependent information. After that, CORAL applies a random walk with restarts to the updated causal graph to accurately identify root causes. The online RCA process terminates when the causal graph and the generated root cause list converge. Extensive experiments on three real-world datasets with case studies demonstrate the effectiveness and superiority of the proposed framework.

Sequential Diagnosis with Language Models

Artificial intelligence holds great promise for expanding access to expert medical knowledge and reasoning. However, most evaluations of language models rely on static vignettes and multiple-choice questions that fail to reflect the complexity and nuance of evidence-based medicine in real-world settings. In clinical practice, physicians iteratively formulate and revise diagnostic hypotheses, adapting each subsequent question and test to what they've just learned, and weigh the evolving evidence before committing to a final diagnosis. To emulate this iterative process, we introduce the Sequential Diagnosis Benchmark, which transforms 304 diagnostically challenging New England Journal of Medicine clinicopathological conference (NEJM-CPC) cases into stepwise diagnostic encounters. A physician or AI begins with a short case abstract and must iteratively request additional details from a gatekeeper model that reveals findings only when explicitly queried. Performance is assessed not just by diagnostic accuracy but also by the cost of physician visits and tests performed. We also present the MAI Diagnostic Orchestrator (MAI-DxO), a model-agnostic orchestrator that simulates a panel of physicians, proposes likely differential diagnoses and strategically selects high-value, cost-effective tests. When paired with OpenAI's o3 model, MAI-DxO achieves 80% diagnostic accuracy--four times higher than the 20% average of generalist physicians. MAI-DxO also reduces diagnostic costs by 20% compared to physicians, and 70% compared to off-the-shelf o3. When configured for maximum accuracy, MAI-DxO achieves 85.5% accuracy. These performance gains with MAI-DxO generalize across models from the OpenAI, Gemini, Claude, Grok, DeepSeek, and Llama families. We highlight how AI systems, when guided to think iteratively and act judiciously, can advance diagnostic precision and cost-effectiveness in clinical care.

Evaluating whether AI models would sabotage AI safety research

We evaluate the propensity of frontier models to sabotage or refuse to assist with safety research when deployed as AI research agents within a frontier AI company. We apply two complementary evaluations to four Claude models (Mythos Preview, Opus 4.7 Preview, Opus 4.6, and Sonnet 4.6): an unprompted sabotage evaluation testing model behaviour with opportunities to sabotage safety research, and a sabotage continuation evaluation testing whether models continue to sabotage when placed in trajectories where prior actions have started undermining research. We find no instances of unprompted sabotage across any model, with refusal rates close to zero for Mythos Preview and Opus 4.7 Preview, though all models sometimes only partially completed tasks. In the continuation evaluation, Mythos Preview actively continues sabotage in 7% of cases (versus 3% for Opus 4.6, 4% for Sonnet 4.6, and 0% for Opus 4.7 Preview), and exhibits reasoning-output discrepancy in the majority of these cases, indicating covert sabotage reasoning. Our evaluation framework builds on Petri, an open-source LLM auditing tool, with a custom scaffold running models inside Claude Code, alongside an iterative pipeline for generating realistic sabotage trajectories. We measure both evaluation awareness and a new form of situational awareness termed "prefill awareness", the capability to recognise that prior trajectory content was not self-generated. Opus 4.7 Preview shows notably elevated unprompted evaluation awareness, while prefill awareness remains low across all models. Finally, we discuss limitations including evaluation awareness confounds, limited scenario coverage, and untested pathways to risk beyond safety research sabotage.

ReFlect: An Effective Harness System for Complex Long-Horizon LLM Reasoning

Current reasoning paradigms for LLMs include chain-of-thought, ReAct, and post-hoc self-critique. These paradigms rely on two assumptions that fail on long-horizon, multi-stage tasks. As a result, errors accumulate silently across reasoning steps, leaving an open question: can a reasoning system effectively detect and recover from its own failures? We present ReFlect, a harness system for LLM reasoning that creates standalone error detection and recovery logic as a deterministic wrapper around the model. Controlled experiments across 6 reasoning domains show that prompt-level self-critique produces formulaic templates that flag no issues in 90 of 100 audited reflection blocks, and the investigated LLMs wrongly accept a wrong answer in at least 76\% of cases. Our ReFlect harness achieves task success rates ranging from 41\% on gpt-4o-mini to 56\% on Claude Sonnet 4.5 across six models spanning small and frontier scale, with per-model gains over Direct CoT ranging from +7 pp on Qwen2.5-72B to +29 pp on Claude Sonnet 4.5, and additionally raises SWE-bench patch-structural quality from 0\% (Direct CoT) to between 82\% (Qwen2.5-72B) and 87\% (GPT-4o). Notably, the harness gain is inversely proportional to the model's Direct CoT task success rate (the fitted slope is -1.69 with r=-0.76): each pp lost in baseline success rate is mechanically recovered by 1.69 pp of harness gain. We spot that adding structured reasoning state and operators yields only 15.0--18.7\% pair-mean on Llama-3.3-70B and Qwen2.5-72B because models at this scale cannot reliably populate the state its operators require. ReFlect is model-agnostic, training-free, and operates entirely at inference time.

SHARP: Sparsity and Hidden Activation RePlay for Neuro-Inspired Continual Learning

Deep neural networks (DNNs) struggle to learn in dynamic environments since they rely on fixed datasets or stationary environments. Continual learning (CL) aims to address this limitation and enable DNNs to accumulate knowledge incrementally, similar to human learning. Inspired by how our brain consolidates memories, a powerful strategy in CL is replay, which involves training the DNN on a mixture of new and all seen classes. However, existing replay methods overlook two crucial aspects of biological replay: 1) the brain replays processed neural patterns instead of raw input, and 2) it prioritizes the replay of recently learned information rather than revisiting all past experiences. To address these differences, we propose SHARP, an efficient neuro-inspired CL method that leverages sparse dynamic connectivity and activation replay. Unlike other activation replay methods, which assume layers not subjected to replay have been pretrained and fixed, SHARP can continually update all layers. Also, SHARP is unique in that it only needs to replay few recently seen classes instead of all past classes. Our experiments on five datasets demonstrate that SHARP outperforms state-of-the-art replay methods in class incremental learning. Furthermore, we showcase SHARP's flexibility in a novel CL scenario where the boundaries between learning episodes are blurry. Automatic Failure Attribution and Critical Step Prediction Method for Multi-Agent Systems Based on Causal Inference

Multi-agent systems (MAS) are critical for automating complex tasks, yet their practical deployment is severely hampered by the challenge of failure attribution. Current diagnostic tools, which rely on statistical correlations, are fundamentally inadequate; on challenging benchmarks like Who\&When, state-of-the-art methods achieve less than 15\% accuracy in locating the root-cause step of a failure. To address this critical gap, we introduce the first failure attribution framework for MAS grounded in multi-granularity causal inference. Our approach makes two key technical contributions: (1) a performance causal inversion principle, which correctly models performance dependencies by reversing the data flow in execution logs, combined with Shapley values to accurately assign agent-level blame; (2) a novel causal discovery algorithm, CDC-MAS, that robustly identifies critical failure steps by tackling the non-stationary nature of MAS interaction data. The framework's attribution results directly fuel an automated optimization loop, generating targeted suggestions whose efficacy is validated via counterfactual simulations. Evaluations on the Who\&When and TRAIL benchmarks demonstrate a significant leap in performance. Our method achieves up to 36.2\% step-level accuracy. Crucially, the generated optimizations boost overall task success rates by an average of 22.4\%. This work provides a principled and effective solution for debugging complex agent interactions, paving the way for more reliable and interpretable multi-agent systems.

Avoid Catastrophic Forgetting with Rank-1 Fisher from Diffusion Models

Catastrophic forgetting remains a central obstacle for continual learning in neural models. Popular approaches -- replay and elastic weight consolidation (EWC) -- have limitations: replay requires a strong generator and is prone to distributional drift, while EWC implicitly assumes a shared optimum across tasks and typically uses a diagonal Fisher approximation. In this work, we study the gradient geometry of diffusion models, which can already produce high-quality replay data. We provide theoretical and empirical evidence that, in the low signal-to-noise ratio (SNR) regime, per-sample gradients become strongly collinear, yielding an empirical Fisher that is effectively rank-1 and aligned with the mean gradient. Leveraging this structure, we propose a rank-1 variant of EWC that is as cheap as the diagonal approximation yet captures the dominant curvature direction. We pair this penalty with a replay-based approach to encourage parameter sharing across tasks while mitigating drift. On class-incremental image generation datasets (MNIST, FashionMNIST, CIFAR-10, ImageNet-1k), our method consistently improves average FID and reduces forgetting relative to replay-only and diagonal-EWC baselines. In particular, forgetting is nearly eliminated on MNIST and FashionMNIST and is more than halved on ImageNet-1k. These results suggest that diffusion models admit an approximately rank-1 Fisher. With a better Fisher estimate, EWC becomes a strong complement to replay: replay encourages parameter sharing across tasks, while EWC effectively constrains replay-induced drift.

VIBEPASS: Can Vibe Coders Really Pass the Vibe Check?

As Large Language Models shift the programming toward human-guided ''vibe coding'', agentic coding tools increasingly rely on models to self-diagnose and repair their own subtle faults -- a capability central to autonomous software engineering yet never systematically evaluated. We present , the first empirical decomposition that jointly evaluates two coupled tasks: Fault-Triggering Test Generation (FT-Test) constructing a discriminative witness that exposes a latent bug, and Fault-targeted Program Repair (FPR), repairing it under varying diagnostic conditions. pairs competitive programming problems with LLM-generated solutions that pass partial test suites but fail on semantic edge cases, enabling controlled identification of where the diagnostic chain breaks down. Evaluating 12 frontier LLMs, we find that fault-targeted reasoning does not scale with general coding ability. Models produce syntactically valid test inputs at near-ceiling rates yet collapse on discriminative generation, with fault hypothesis generation -- not output validation -- as the dominant bottleneck. Test-guided repair reveals a complementary insight: when self-generated tests successfully witness a fault, the resulting repair matches or outperforms repair guided by externally provided tests, but tests that fail to witness the fault actively degrade repair below unguided baselines. Together, these results reframe the challenge of autonomous debugging: the binding bottleneck is not code synthesis or test validity but fault-target reasoning, a capability that remains deficient across all frontier models. DEER: A Benchmark for Evaluating Deep Research Agents on Expert Report Generation

Recent advances in large language models have enabled deep research systems that generate expert-level reports through multi-step reasoning and evidence-based synthesis. However, evaluating such reports remains challenging: report quality is multifaceted, making it difficult to determine what to assess and by what criteria; LLM-based judges may miss errors that require domain expertise to identify; and because deep research relies on retrieved evidence, report-wide claim verification is also necessary. To address these issues, we propose DEER, a benchmark for evaluating expert-level deep research reports. DEER systematizes evaluation criteria with an expert-developed taxonomy (7 dimensions, 25 subdimensions) operationalized as 101 fine-grained rubric items. We also provide task-specific Expert Evaluation Guidance to support LLM-based judging. Alongside rubric-based assessment, we propose a claim verification architecture that verifies both cited and uncited claims and quantifies evidence quality. Experiments show that while current deep research systems can produce structurally plausible reports that cite external evidence, there is room for improvement in fulfilling expert-level user requests and achieving logical completeness. Beyond simple performance comparisons, DEER makes system strengths and limitations interpretable and provides diagnostic signals for improvement.

Meissa: Multi-modal Medical Agentic Intelligence

Multi-modal large language models (MM-LLMs) have shown strong performance in medical image understanding and clinical reasoning. Recent medical agent systems extend them with tool use and multi-agent collaboration, enabling complex decision-making. However, these systems rely almost entirely on frontier models (e.g., GPT), whose API-based deployment incurs high cost, high latency, and privacy risks that conflict with on-premise clinical requirements. We present Meissa, a lightweight 4B-parameter medical MM-LLM that brings agentic capability offline. Instead of imitating static answers, Meissa learns both when to engage external interaction (strategy selection) and how to execute multi-step interaction (strategy execution) by distilling structured trajectories from frontier models. Specifically, we propose: (1) Unified trajectory modeling: trajectories (reasoning and action traces) are represented within a single state-action-observation formalism, allowing one model to generalize across heterogeneous medical environments. (2) Three-tier stratified supervision: the model's own errors trigger progressive escalation from direct reasoning to tool-augmented and multi-agent interaction, explicitly learning difficulty-aware strategy selection. (3) Prospective-retrospective supervision: pairing exploratory forward traces with hindsight-rationalized execution traces enables stable learning of effective interaction policies. Trained on 40K curated trajectories, Meissa matches or exceeds proprietary frontier agents in 10 of 16 evaluation settings across 13 medical benchmarks spanning radiology, pathology, and clinical reasoning. Using over 25x fewer parameters than typical frontier models like Gemini-3, Meissa operates fully offline with 22x lower end-to-end latency compared to API-based deployment. RoboTracer: Mastering Spatial Trace with Reasoning in Vision-Language Models for Robotics

Spatial tracing, as a fundamental embodied interaction ability for robots, is inherently challenging as it requires multi-step metric-grounded reasoning compounded with complex spatial referring and real-world metric measurement. However, existing methods struggle with this compositional task. To this end, we propose RoboTracer, a 3D-aware VLM that first achieves both 3D spatial referring and measuring via a universal spatial encoder and a regression-supervised decoder to enhance scale awareness during supervised fine-tuning (SFT). Moreover, RoboTracer advances multi-step metric-grounded reasoning via reinforcement fine-tuning (RFT) with metric-sensitive process rewards, supervising key intermediate perceptual cues to accurately generate spatial traces. To support SFT and RFT training, we introduce TraceSpatial, a large-scale dataset of 30M QA pairs, spanning outdoor/indoor/tabletop scenes and supporting complex reasoning processes (up to 9 steps). We further present TraceSpatial-Bench, a challenging benchmark filling the gap to evaluate spatial tracing. Experimental results show that RoboTracer surpasses baselines in spatial understanding, measuring, and referring, with an average success rate of 79.1%, and also achieves SOTA performance on TraceSpatial-Bench by a large margin, exceeding Gemini-2.5-Pro by 36% accuracy. Notably, RoboTracer can be integrated with various control policies to execute long-horizon, dynamic tasks across diverse robots (UR5, G1 humanoid) in cluttered real-world scenes.

Auditing Demonstration Curation Metrics: Action-Only Scorers Fail on the Structural Defects That Degrade Imitation Policies

Imitation-learning policies inherit the quality of the demonstrations they are trained on, and a growing set of curation metrics promise to score and filter low-quality demonstrations automatically. These metrics are each validated on different data with different protocols, so it is unclear which of them actually identify the demonstrations that harm a policy. We build a controlled testbed in which demonstration defects are injected with known type, and audit seven curation metrics along two axes: how well each separates defective from clean demonstrations, and whether training a behavior-cloning policy on each metric's curated subset improves task success. We study two defect regimes. Subtle perturbations (correlated action noise, tremor, truncation) are detectable by multivariate outlier scoring and, once removed, recover the full downstream gap. Structural errors, where the demonstration executes a wrong action at a key moment, are invisible to every action-only metric we test, and two of them are inverted: they score defective demonstrations as higher quality and, used for curation, tend to leave the policy at or below the uncurated baseline rather than above it. Only metrics that examine the state trajectory detect structural errors, and even the best of them recovers just a third of the downstream gap. High detection accuracy does not guarantee downstream improvement. We release the testbed and all curation implementations.

R^3L: Reflect-then-Retry Reinforcement Learning with Language-Guided Exploration, Pivotal Credit, and Positive Amplification

Reinforcement learning drives recent advances in LLM reasoning and agentic capabilities, yet current approaches struggle with both exploration and exploitation. Exploration suffers from low success rates on difficult tasks and high costs of repeated rollouts from scratch. Exploitation suffers from coarse credit assignment and training instability: Trajectory-level rewards penalize valid prefixes for later errors, and failure-dominated groups overwhelm the few positive signals, leaving optimization without constructive direction. To this end, we propose R^3L, Reflect-then-Retry Reinforcement Learning with Language-Guided Exploration, Pivotal Credit, and Positive Amplification. To synthesize high-quality trajectories, R^3L shifts from stochastic sampling to active synthesis via reflect-then-retry, leveraging language feedback to diagnose errors, transform failed attempts into successful ones, and reduce rollout costs by restarting from identified failure points. With errors diagnosed and localized, Pivotal Credit Assignment updates only the diverging suffix where contrastive signals exist, excluding the shared prefix from gradient update. Since failures dominate on difficult tasks and reflect-then-retry produces off-policy data, risking training instability, Positive Amplification upweights successful trajectories to ensure positive signals guide the optimization process. Experiments on agentic and reasoning tasks demonstrate 5\% to 52\% relative improvements over baselines while maintaining training stability. RePrompT: Recurrent Prompt Tuning for Integrating Structured EHR Encoders with Large Language Models

Large Language Models (LLMs) have shown strong promise for mining Electronic Health Records (EHRs) by reasoning over longitudinal clinical information to capture context-rich patient trajectories. However, leveraging LLMs for structured EHRs (e.g., standardized diagnosis and medication codes) presents two key challenges. First, translating time-stamped EHR sequences into plain text can obscure both temporal structure and code identities, weakening the ability to capture code co-occurrence and longitudinal regularities. Second, unlike cohort-trained predictive models that learn a shared, task-aligned representation space across patients, LLMs are often applied in a case-isolated inference setting where each patient is processed independently without leveraging population-level patterns. To address these challenges, we introduce RePrompT, a time-aware LLM framework that integrates structured EHR encoders through prompt tuning, without modifying underlying architectures. Specifically, RePrompT recurrently incorporates latent states from prior visits to preserve longitudinal information, and injects population-level information through trainable prompt tokens derived from a cohort-trained, task-aligned EHR encoder. Experiments on MIMIC-III and MIMIC-IV demonstrate that RePrompT consistently outperforms both EHR-based and LLM-based baselines across multiple clinical prediction tasks.

Sibyl-AutoResearch: Autonomous Research Needs Self-Evolving Trial-and-Error Harnesses, Not Paper Generators

Autonomous research systems increasingly make the scientific workflow executable: agents can propose ideas, run code, inspect results, and draft papers. But executable workflows do not by themselves produce research judgment. We analyze where current systems lose trial experience: weak evidence becomes prose, pilot signals become broad claims, memory remains textual, and recurring process failures do not change later behavior. We introduce Sibyl-AutoResearch, a self-evolving AutoResearch framework built around Scientific Trial-and-Error Harnesses. A harness lets agents run bounded trials, preserve positive and negative outcomes, and route lessons into later planning, validation, claim scope, scheduling, critique, writing, and harness repair. We formalize this through two auditable conversion units: trial-to-behavior conversion, which links trial signals to later research actions, and trial-to-harness-behavior conversion, which links recurring process failures to system updates. We implement the framework in SIBYL, a file-backed autonomous research system that exposes the state, roles, memory, gates, and artifact traces needed to inspect these conversion paths. A retrospective audit identifies eight high-confidence conversion events, with a median latency of one iteration and a maximum latency of three iterations. A recovered-failure registry further shows how five naturally occurring failure classes, including duplicate results, stale numbers, and unsupported statistics, were blocked, downgraded, or routed into later repair. These traces do not establish a comparative performance claim; they show that the proposed conversion units are recoverable from realistic autonomous-research workspaces. Synthetic Experience Replay

A key theme in the past decade has been that when large neural networks and large datasets combine they can produce remarkable results. In deep reinforcement learning (RL), this paradigm is commonly made possible through experience replay, whereby a dataset of past experiences is used to train a policy or value function. However, unlike in supervised or self-supervised learning, an RL agent has to collect its own data, which is often limited. Thus, it is challenging to reap the benefits of deep learning, and even small neural networks can overfit at the start of training. In this work, we leverage the tremendous recent progress in generative modeling and propose Synthetic Experience Replay (SynthER), a diffusion-based approach to flexibly upsample an agent's collected experience. We show that SynthER is an effective method for training RL agents across offline and online settings, in both proprioceptive and pixel-based environments. In offline settings, we observe drastic improvements when upsampling small offline datasets and see that additional synthetic data also allows us to effectively train larger networks. Furthermore, SynthER enables online agents to train with a much higher update-to-data ratio than before, leading to a significant increase in sample efficiency, without any algorithmic changes. We believe that synthetic training data could open the door to realizing the full potential of deep learning for replay-based RL algorithms from limited data. Augmenting Replay in World Models for Continual Reinforcement Learning

Continual RL requires an agent to learn new tasks without forgetting previous ones, while improving on both past and future tasks. The most common approaches use model-free algorithms and replay buffers can help to mitigate catastrophic forgetting, but often struggle with scalability due to large memory requirements. Biologically inspired replay suggests replay to a world model, aligning with model-based RL; as opposed to the common setting of replay in model-free algorithms. Model-based RL offers benefits for continual RL by leveraging knowledge of the environment, independent of policy. We introduce WMAR (World Models with Augmented Replay), a model-based RL algorithm with a memory-efficient distribution-matching replay buffer. WMAR extends the well known DreamerV3 algorithm, which employs a simple FIFO buffer and was not tested in continual RL. We evaluated WMAR and DreamerV3, with the same-size replay buffers. They were tested on two scenarios: tasks with shared structure using OpenAI Procgen and tasks without shared structure using the Atari benchmark. WMAR demonstrated favourable properties for continual RL considering metrics for forgetting as well as skill transfer on past and future tasks. Compared to DreamerV3, WMAR showed slight benefits in tasks with shared structure and substantially better forgetting characteristics on tasks without shared structure. Our results suggest that model-based RL with a memory-efficient replay buffer can be an effective approach to continual RL, justifying further research.

Computer Use at the Edge of the Statistical Precipice

Evaluating Computer Use Agents (CUAs) on interactive environments is fraught with methodological pitfalls that the field has yet to systematically address. We show that a 1MB replay script that blindly executes a recorded action sequence without ever observing the screen outperforms frontier models on prominent static benchmarks, and prove that its expected success rate is exactly equal to the source agent's pass@k in deterministic environments. We trace this and other failures to two root causes: non-principled environment design (static, unsandboxed, or unreliably verified environments) and non-principled evaluation methodology (naive aggregation and misuse of pass@k for stateful UI interactions). To address the first, we propose PRISM, five design principles for CUA environments (privileged verification, realistic environments, integrity-checked configurations, sandboxed execution, and multifactorial variability) and instantiate them in DigiWorld, a benchmark of 15 realistic sandboxed mobile applications able to evaluate agents in over 3.2 million verified unique configurations. To address the second, we develop an aggregation framework pairing Wilson score intervals with hierarchical bootstrap, producing confidence intervals that correctly account for the nested structure of CUA benchmarks, as we empirically demonstrate. All together, we show that principled environment design and rigorous evaluation methodology are not optional refinements but prerequisites for meaningful CUA research.

Evidence-Grounded Ensemble Diagnosis of 802.11 Packet Captures: A Multi-Stage Pipeline with Deterministic Reliability Scoring

Diagnosing 802.11 packet captures requires expert protocol knowledge, is slow, inconsistent across engineers, and unscalable. LLM-based approaches sound plausible but fabricate protocol events absent from captures (especially truncated traces), produce uncalibrated confidence scores, and suffer evaluation bias when golden references are co-produced by the model under test. We introduce PROBE (Protocol Reasoning Over evidence-Based Ensembles), a multi-stage pipeline addressing all three failures. It integrates (i) deterministic PCAP-to-text normalization with frame-level verifiability, (ii) multi-run, multi-candidate ensembles with optional cross-model second opinion and progressive obfuscation, (iii) a verdict-aware evidence framework treating absence of failure evidence as contributing evidence, and (iv) a fully deterministic composite reliability score from evidence validity, run-to-run stability, and cross-model agreement without LLM self-assessment. On 87 enterprise Wi-Fi captures (104 capture-reviewer pairs), single-pass LLM analysis raises weighted evidence F1 from 0.871 (expert baseline) to 0.912 but misses critical frames in 35% of cases. Naive ensemble voting drops below baseline (0.842) as majority voting amplifies conservative verdicts: 50% of confirmed failures are misclassified as 'no issue' or 'insufficient evidence.' Adding evidence-grounded reconciliation achieves 0.957 F1, a 96% auto-accept rate, and a worst-case floor above 0.70. LLM self-reported confidence clusters at 0.95 regardless of difficulty (71% report exactly 0.95), confirming it is uninformative. We also introduce a model-agnostic evaluation framework using per-field assertion matching, eliminating circular bias from model-co-produced golden references.

PlayCoder: Making LLM-Generated GUI Code Playable

Large language models (LLMs) have achieved strong results in code generation, but their ability to generate GUI applications, especially games, remains insufficiently studied. Existing benchmarks mainly evaluate correctness through test cases, which are inadequate for GUI applications because these systems are interactive, event-driven, and require correct state transitions across sequences of user actions. Their evaluation therefore should consider interaction flows and UI logic rather than only pass/fail outcomes. To study this problem, we introduce PlayEval, a repository-aware benchmark built from 43 multilingual GUI applications in Python, TypeScript, and JavaScript. Unlike prior GUI benchmarks that are difficult to adapt to desktop environments, PlayEval covers six major GUI application categories and directly supports code-generation evaluation. We further propose Play@k, a metric that measures whether at least one of *k* generated candidates can be played end-to-end without logical errors. To support reliable evaluation, we develop PlayTester, an LLM-based agent that performs task-oriented GUI playthroughs and detects logic violations automatically. Experiments on 10 state-of-the-art code LLMs show that, despite high compilation rates, they achieve near-zero Play@3, revealing major weaknesses in generating logically correct GUI applications. To address this limitation, we present PlayCoder, a multi-agent, repository-aware framework that generates, evaluates, and iteratively repairs GUI application code in a closed loop. PlayCoder substantially improves both functional correctness and semantic alignment for open-source and closed-source models, reaching up to 38.1% Exec@3 and 20.3% Play@3. Case studies further show that it can uncover silent logic bugs missed by traditional metrics and fix them through targeted edits.

FeasibleCap: Real-Time Embodiment Constraint Guidance for In-the-Wild Robot Demonstration Collection

Gripper-in-hand data collection decouples demonstration acquisition from robot hardware, but whether a trajectory is executable on the target robot remains unknown until a separate replay-and-validate stage. Failed demonstrations therefore inflate the effective cost per usable trajectory through repeated collection, diagnosis, and validation. Existing collection-time feedback systems mitigate this issue but rely on head-worn AR/VR displays, robot-in-the-loop hardware, or learned dynamics models; real-time executability feedback has not yet been integrated into the gripper-in-hand data collection paradigm. We present FeasibleCap, a gripper-in-hand data collection system that brings real-time executability guidance into robot-free capture. At each frame, FeasibleCap checks reachability, joint-rate limits, and collisions against a target robot model and closes the loop through on-device visual overlays and haptic cues, allowing demonstrators to correct motions during collection without learned models, headsets, or robot hardware. On pick-and-place and tossing tasks, FeasibleCap improves replay success and reduces the fraction of infeasible frames, with the largest gains on tossing. Simulation experiments further indicate that enforcing executability constraints during collection does not sacrifice cross-embodiment transfer across robot platforms.
