GuardTrace-VL: Detecting Unsafe Multimodel Reasoning via Iterative Safety Supervision

Multimodal large reasoning models (MLRMs) are increasingly deployed for vision-language tasks that produce explicit intermediate rationales. However, reasoning traces can contain unsafe content even when the final answer is non-harmful, creating deployment risks. Existing multimodal safety guards primarily evaluate only the input question and the final answer, neglecting the intermediate reasoning process. This oversight allows undetected harm, such as biased inferences or policy-violating use of visual context, to emerge during reasoning. We introduce GuardTrace-VL, a vision-aware safety auditor that monitors the full Question-Thinking-Answer (QTA) pipeline via joint image-text analysis, enabling detection of unsafe content as it emerges in the reasoning stage. To support training and evaluation, we construct the GuardTrace dataset, which is generated through diverse prompting strategies and refined via a MLRM- and human-based voting and verification pipeline. Furthermore, we propose a three-stage progressive training scheme combined with the data refinement process, enabling the model to learn nuanced and context-dependent safety preferences according to different risk levels. On our proposed test set covering both in-domain and out-of-domain scenarios, GuardTrace-VL model achieves an F1 score of 93.1% on unsafe reasoning detection tasks, representing a 13.5% improvement in F1 score compared to the previous strongest multimodal safety defense methods. Rethinking Bottlenecks in Safety Fine-Tuning of Vision Language Models

Large Vision-Language Models (VLMs) have achieved remarkable performance across a wide range of tasks. However, their deployment in safety-critical domains poses significant challenges. Existing safety fine-tuning methods, which focus on textual or multimodal content, fall short in addressing challenging cases or disrupt the balance between helpfulness and harmlessness. Our evaluation highlights a safety reasoning gap: these methods lack safety visual reasoning ability, leading to such bottlenecks. To address this limitation and enhance both visual perception and reasoning in safety-critical contexts, we propose a novel dataset that integrates multi-image inputs with safety Chain-of-Thought (CoT) labels as fine-grained reasoning logic to improve model performance. Specifically, we introduce the Multi-Image Safety (MIS) dataset, an instruction-following dataset tailored for multi-image safety scenarios, consisting of training and test splits. Our experiments demonstrate that fine-tuning InternVL2.5-8B with MIS significantly outperforms both powerful open-source models and API-based models in challenging multi-image tasks requiring safety-related visual reasoning. This approach not only delivers exceptional safety performance but also preserves general capabilities without any trade-offs. Specifically, fine-tuning with MIS increases average accuracy by 0.83% across five general benchmarks and reduces the Attack Success Rate (ASR) on multiple safety benchmarks by a large margin. Think-Reflect-Revise: A Policy-Guided Reflective Framework for Safety Alignment in Large Vision Language Models

As multimodal reasoning improves the overall capabilities of Large Vision Language Models (LVLMs), recent studies have begun to explore safety-oriented reasoning, aiming to enhance safety awareness by analyzing potential safety risks during the reasoning process before generating the final response. Although such approaches improve safety awareness and interpretability, this single-pass think-then-answer paradigm remains vulnerable to contextual or visual jailbreak attacks. This reveals a critical flaw: single-pass reasoning may overlook explicit harmful content in its own output. Our key insight is to exploit this wasted signal through reflection, which can effectively leverage the malicious content revealed in the first-pass reasoning to enable genuine self-correction and prevent unsafe generations. Motivated by this, we propose Think-Reflect-Revise (TRR), a three-stage training framework designed to enhance the safety alignment of LVLMs through policy-guided self-reflection. We first build a Reflective Safety Reasoning (ReSafe) dataset with 5,000 examples that follow a think-reflect-revise process. We then fine-tune the target model using the ReSafe dataset to initialize reflective behavior, and finally reinforce policy-guided reflection through reinforcement learning. Experimental results show that TRR substantially improves the safety performance of LVLMs across both safety-awareness benchmarks and jailbreak attack evaluations, increasing the overall safe response rate from 42.8% to 87.7% on Qwen2.5-VL-7B, while preserving stable performance on general benchmarks such as MMMU and MMStar. Video-SafetyBench: A Benchmark for Safety Evaluation of Video LVLMs

The increasing deployment of Large Vision-Language Models (LVLMs) raises safety concerns under potential malicious inputs. However, existing multimodal safety evaluations primarily focus on model vulnerabilities exposed by static image inputs, ignoring the temporal dynamics of video that may induce distinct safety risks. To bridge this gap, we introduce Video-SafetyBench, the first comprehensive benchmark designed to evaluate the safety of LVLMs under video-text attacks. It comprises 2,264 video-text pairs spanning 48 fine-grained unsafe categories, each pairing a synthesized video with either a harmful query, which contains explicit malice, or a benign query, which appears harmless but triggers harmful behavior when interpreted alongside the video. To generate semantically accurate videos for safety evaluation, we design a controllable pipeline that decomposes video semantics into subject images (what is shown) and motion text (how it moves), which jointly guide the synthesis of query-relevant videos. To effectively evaluate uncertain or borderline harmful outputs, we propose RJScore, a novel LLM-based metric that incorporates the confidence of judge models and human-aligned decision threshold calibration. Extensive experiments show that benign-query video composition achieves average attack success rates of 67.2%, revealing consistent vulnerabilities to video-induced attacks. We believe Video-SafetyBench will catalyze future research into video-based safety evaluation and defense strategies.

UnsafeBench: Benchmarking Image Safety Classifiers on Real-World and AI-Generated Images

Image safety classifiers play an important role in identifying and mitigating the spread of unsafe images online (e.g., images including violence, hateful rhetoric, etc.). At the same time, with the advent of text-to-image models and increasing concerns about the safety of AI models, developers are increasingly relying on image safety classifiers to safeguard their models. Yet, the performance of current image safety classifiers remains unknown for real-world and AI-generated images. To bridge this research gap, in this work, we propose UnsafeBench, a benchmarking framework that evaluates the effectiveness and robustness of image safety classifiers. First, we curate a large dataset of 10K real-world and AI-generated images that are annotated as safe or unsafe based on a set of 11 unsafe categories of images (sexual, violent, hateful, etc.). Then, we evaluate the effectiveness and robustness of five popular image safety classifiers, as well as three classifiers that are powered by general-purpose visual language models. Our assessment indicates that existing image safety classifiers are not comprehensive and effective enough in mitigating the multifaceted problem of unsafe images. Also, we find that classifiers trained only on real-world images tend to have degraded performance when applied to AI-generated images. Motivated by these findings, we design and implement a comprehensive image moderation tool called PerspectiveVision, which effectively identifies 11 categories of real-world and AI-generated unsafe images. The best PerspectiveVision model achieves an overall F1-Score of 0.810 on six evaluation datasets, which is comparable with closed-source and expensive state-of-the-art models like GPT-4V. UnsafeBench and PerspectiveVision can aid the research community in better understanding the landscape of image safety classification in the era of generative AI.

Towards Policy-Adaptive Image Guardrail: Benchmark and Method

Accurate rejection of sensitive or harmful visual content, i.e., harmful image guardrail, is critical in many application scenarios. This task must continuously adapt to the evolving safety policies and content across various domains and over time. However, traditional classifiers, confined to fixed categories, require frequent retraining when new policies are introduced. Vision-language models (VLMs) offer a more adaptable and generalizable foundation for dynamic safety guardrails. Despite this potential, existing VLM-based safeguarding methods are typically trained and evaluated under only a fixed safety policy. We find that these models are heavily overfitted to the seen policy, fail to generalize to unseen policies, and even lose the basic instruction-following ability and general knowledge. To address this issue, in this paper we make two key contributions. First, we benchmark the cross-policy generalization performance of existing VLMs with SafeEditBench, a new evaluation suite. SafeEditBench leverages image-editing models to convert unsafe images into safe counterparts, producing policy-aligned datasets where each safe-unsafe image pair remains visually similar except for localized regions violating specific safety rules. Human annotators then provide accurate safe/unsafe labels under five distinct policies, enabling fine-grained assessment of policy-aware generalization. Second, we introduce SafeGuard-VL, a reinforcement learning-based method with verifiable rewards (RLVR) for robust unsafe-image guardrails. Instead of relying solely on supervised fine-tuning (SFT) under fixed policies, SafeGuard-VL explicitly optimizes the model with policy-grounded rewards, promoting verifiable adaptation across evolving policies. Extensive experiments verify the effectiveness of our method for unsafe image guardrails across various policies.

PRISM: Robust VLM Alignment with Principled Reasoning for Integrated Safety in Multimodality

Safeguarding vision-language models (VLMs) is a critical challenge, as existing methods often suffer from over-defense, which harms utility, or rely on shallow alignment, failing to detect complex threats that require deep reasoning. To this end, we introduce PRISM (Principled Reasoning for Integrated Safety in Multimodality), a system2-like framework that aligns VLMs by embedding a structured, safety-aware reasoning process. Our framework consists of two key components: PRISM-CoT, a dataset that teaches safety-aware chain-of-thought reasoning, and PRISM-DPO, generated via Monte Carlo Tree Search (MCTS) to further refine this reasoning through Direct Preference Optimization to help obtain a delicate safety boundary. Comprehensive evaluations demonstrate PRISM's effectiveness, achieving remarkably low attack success rates including 0.15% on JailbreakV-28K for Qwen2-VL and 90% improvement over the previous best method on VLBreak for LLaVA-1.5. PRISM also exhibits strong robustness against adaptive attacks, significantly increasing computational costs for adversaries, and generalizes effectively to out-of-distribution challenges, reducing attack success rates to just 8.70% on the challenging multi-image MIS benchmark. Remarkably, this robust defense is achieved while preserving, and in some cases enhancing, model utility. HoliSafe: Holistic Safety Benchmarking and Modeling with Safety Meta Token for Vision-Language Model

Despite emerging efforts to enhance the safety of Vision-Language Models (VLMs), current approaches face two main shortcomings. 1) Existing safety-tuning datasets and benchmarks only partially consider how image-text interactions can yield harmful content, often overlooking contextually unsafe outcomes from seemingly benign pairs. This narrow coverage leaves VLMs vulnerable to jailbreak attacks in unseen configurations. 2) Prior methods rely primarily on data-centric tuning, with limited architectural innovations to intrinsically strengthen safety. We address these gaps by introducing a holistic safety dataset and benchmark, HoliSafe, that spans all five safe/unsafe image-text combinations, providing a more robust basis for both training and evaluation. We further propose SafeLLaVA, a novel VLM augmented with a learnable safety meta token and a dedicated safety head. The meta token encodes harmful visual cues during training, intrinsically guiding the language model toward safer responses, while the safety head offers interpretable harmfulness classification aligned with refusal rationales. Experiments show that SafeLLaVA, trained on HoliSafe, achieves state-of-the-art safety performance across multiple VLM benchmarks. Additionally, the HoliSafe benchmark itself reveals critical vulnerabilities in existing models. We hope that HoliSafe and SafeLLaVA will spur further research into robust and interpretable VLM safety, expanding future avenues for multimodal alignment.

A Safety Report on GPT-5.2, Gemini 3 Pro, Qwen3-VL, Doubao 1.8, Grok 4.1 Fast, Nano Banana Pro, and Seedream 4.5

The rapid evolution of Large Language Models (LLMs) and Multimodal Large Language Models (MLLMs) has produced substantial gains in reasoning, perception, and generative capability across language and vision. However, whether these advances yield commensurate improvements in safety remains unclear, in part due to fragmented evaluation practices limited to single modalities or threat models. In this report, we present an integrated safety evaluation of 7 frontier models: GPT-5.2, Gemini 3 Pro, Qwen3-VL, Doubao 1.8, Grok 4.1 Fast, Nano Banana Pro, and Seedream 4.5. We evaluate each model across language, vision-language, and image generation settings using a unified protocol that integrates benchmark evaluation, adversarial evaluation, multilingual evaluation, and compliance evaluation. Aggregating our evaluations into safety leaderboards and model safety profiles across multiple evaluation modes reveals a sharply heterogeneous safety landscape. While GPT-5.2 demonstrates consistently strong and balanced safety performance across evaluations, other models exhibit pronounced trade-offs among benchmark safety, adversarial alignment, multilingual generalization, and regulatory compliance. Both language and vision-language modalities show significant vulnerability under adversarial evaluation, with all models degrading substantially despite strong results on standard benchmarks. Text-to-image models achieve relatively stronger alignment in regulated visual risk categories, yet remain brittle under adversarial or semantically ambiguous prompts. Overall, these results show that safety in frontier models is inherently multidimensional--shaped by modality, language, and evaluation scheme, underscoring the need for standardized safety evaluations to accurately assess real-world risk and guide responsible model development and deployment.

How Does Vision-Language Adaptation Impact the Safety of Vision Language Models?

Vision-Language adaptation (VL adaptation) transforms Large Language Models (LLMs) into Large Vision-Language Models (LVLMs) for multimodal tasks, but this process often compromises the inherent safety capabilities embedded in the original LLMs. Despite potential harmfulness due to weakened safety measures, in-depth analysis on the effects of VL adaptation on safety remains under-explored. This study examines how VL adaptation influences safety and evaluates the impact of safety fine-tuning methods. Our analysis reveals that safety degradation occurs during VL adaptation, even when the training data is safe. While safety tuning techniques like supervised fine-tuning with safety datasets or reinforcement learning from human feedback mitigate some risks, they still lead to safety degradation and a reduction in helpfulness due to over-rejection issues. Further analysis of internal model weights suggests that VL adaptation may impact certain safety-related layers, potentially lowering overall safety levels. Additionally, our findings demonstrate that the objectives of VL adaptation and safety tuning are divergent, which often results in their simultaneous application being suboptimal. To address this, we suggest the weight merging approach as an optimal solution effectively reducing safety degradation while maintaining helpfulness. These insights help guide the development of more reliable and secure LVLMs for real-world applications.

SaFeR-VLM: Toward Safety-aware Fine-grained Reasoning in Multimodal Models

Multimodal Large Reasoning Models (MLRMs) demonstrate impressive cross-modal reasoning but often amplify safety risks under adversarial or unsafe prompts, a phenomenon we call the Reasoning Tax. Existing defenses mainly act at the output level and do not constrain the reasoning process, leaving models exposed to implicit risks. In this paper, we propose SaFeR-VLM, a safety-aligned reinforcement learning framework that embeds safety directly into multimodal reasoning. The framework integrates four components: (I) QI-Safe-10K, a curated dataset emphasizing safety-critical and reasoning-sensitive cases; (II) safety-aware rollout, where unsafe generations undergo reflection and correction instead of being discarded; (III) structured reward modeling with multi-dimensional weighted criteria and explicit penalties for hallucinations and contradictions; and (IV) GRPO optimization, which reinforces both safe and corrected trajectories. This unified design shifts safety from a passive safeguard to an active driver of reasoning, enabling scalable and generalizable safety-aware reasoning. SaFeR-VLM further demonstrates robustness against both explicit and implicit risks, supporting dynamic and interpretable safety decisions beyond surface-level filtering. SaFeR-VLM-3B achieves average performance 70.13 and 78.97 on safety and helpfulness across six benchmarks, surpassing both same-scale and >10times larger models such as Skywork-R1V3-38B, Qwen2.5VL-72B, and GLM4.5V-106B. Remarkably, SaFeR-VLM-7B benefits from its increased scale to surpass GPT-5-mini and Gemini-2.5-Flash by 6.47 and 16.76 points respectively on safety metrics, achieving this improvement without any degradation in helpfulness performance. ProGuard: Towards Proactive Multimodal Safeguard

The rapid evolution of generative models has led to a continuous emergence of multimodal safety risks, exposing the limitations of existing defense methods. To address these challenges, we propose ProGuard, a vision-language proactive guard that identifies and describes out-of-distribution (OOD) safety risks without the need for model adjustments required by traditional reactive approaches. We first construct a modality-balanced dataset of 87K samples, each annotated with both binary safety labels and risk categories under a hierarchical multimodal safety taxonomy, effectively mitigating modality bias and ensuring consistent moderation across text, image, and text-image inputs. Based on this dataset, we train our vision-language base model purely through reinforcement learning (RL) to achieve efficient and concise reasoning. To approximate proactive safety scenarios in a controlled setting, we further introduce an OOD safety category inference task and augment the RL objective with a synonym-bank-based similarity reward that encourages the model to generate concise descriptions for unseen unsafe categories. Experimental results show that ProGuard achieves performance comparable to closed-source large models on binary safety classification, substantially outperforms existing open-source guard models on unsafe content categorization. Most notably, ProGuard delivers a strong proactive moderation ability, improving OOD risk detection by 52.6% and OOD risk description by 64.8%.

A Survey of Safety on Large Vision-Language Models: Attacks, Defenses and Evaluations

With the rapid advancement of Large Vision-Language Models (LVLMs), ensuring their safety has emerged as a crucial area of research. This survey provides a comprehensive analysis of LVLM safety, covering key aspects such as attacks, defenses, and evaluation methods. We introduce a unified framework that integrates these interrelated components, offering a holistic perspective on the vulnerabilities of LVLMs and the corresponding mitigation strategies. Through an analysis of the LVLM lifecycle, we introduce a classification framework that distinguishes between inference and training phases, with further subcategories to provide deeper insights. Furthermore, we highlight limitations in existing research and outline future directions aimed at strengthening the robustness of LVLMs. As part of our research, we conduct a set of safety evaluations on the latest LVLM, Deepseek Janus-Pro, and provide a theoretical analysis of the results. Our findings provide strategic recommendations for advancing LVLM safety and ensuring their secure and reliable deployment in high-stakes, real-world applications. This survey aims to serve as a cornerstone for future research, facilitating the development of models that not only push the boundaries of multimodal intelligence but also adhere to the highest standards of security and ethical integrity. Hyperbolic Safety-Aware Vision-Language Models

Addressing the retrieval of unsafe content from vision-language models such as CLIP is an important step towards real-world integration. Current efforts have relied on unlearning techniques that try to erase the model's knowledge of unsafe concepts. While effective in reducing unwanted outputs, unlearning limits the model's capacity to discern between safe and unsafe content. In this work, we introduce a novel approach that shifts from unlearning to an awareness paradigm by leveraging the inherent hierarchical properties of the hyperbolic space. We propose to encode safe and unsafe content as an entailment hierarchy, where both are placed in different regions of hyperbolic space. Our HySAC, Hyperbolic Safety-Aware CLIP, employs entailment loss functions to model the hierarchical and asymmetrical relations between safe and unsafe image-text pairs. This modelling, ineffective in standard vision-language models due to their reliance on Euclidean embeddings, endows the model with awareness of unsafe content, enabling it to serve as both a multimodal unsafe classifier and a flexible content retriever, with the option to dynamically redirect unsafe queries toward safer alternatives or retain the original output. Extensive experiments show that our approach not only enhances safety recognition but also establishes a more adaptable and interpretable framework for content moderation in vision-language models. iSafetyBench: A video-language benchmark for safety in industrial environment

Recent advances in vision-language models (VLMs) have enabled impressive generalization across diverse video understanding tasks under zero-shot settings. However, their capabilities in high-stakes industrial domains-where recognizing both routine operations and safety-critical anomalies is essential-remain largely underexplored. To address this gap, we introduce iSafetyBench, a new video-language benchmark specifically designed to evaluate model performance in industrial environments across both normal and hazardous scenarios. iSafetyBench comprises 1,100 video clips sourced from real-world industrial settings, annotated with open-vocabulary, multi-label action tags spanning 98 routine and 67 hazardous action categories. Each clip is paired with multiple-choice questions for both single-label and multi-label evaluation, enabling fine-grained assessment of VLMs in both standard and safety-critical contexts. We evaluate eight state-of-the-art video-language models under zero-shot conditions. Despite their strong performance on existing video benchmarks, these models struggle with iSafetyBench-particularly in recognizing hazardous activities and in multi-label scenarios. Our results reveal significant performance gaps, underscoring the need for more robust, safety-aware multimodal models for industrial applications. iSafetyBench provides a first-of-its-kind testbed to drive progress in this direction. Beyond Confidence: Adaptive Abstention in Dual-Threshold Conformal Prediction for Autonomous System Perception

Safety-critical perception systems require both reliable uncertainty quantification and principled abstention mechanisms to maintain safety under diverse operational conditions. We present a novel dual-threshold conformalization framework that provides statistically-guaranteed uncertainty estimates while enabling selective prediction in high-risk scenarios. Our approach uniquely combines a conformal threshold ensuring valid prediction sets with an abstention threshold optimized through ROC analysis, providing distribution-free coverage guarantees (\ge 1 - \alpha) while identifying unreliable predictions. Through comprehensive evaluation on CIFAR-100, ImageNet1K, and ModelNet40 datasets, we demonstrate superior robustness across camera and LiDAR modalities under varying environmental perturbations. The framework achieves exceptional detection performance (AUC: 0.993\to0.995) under severe conditions while maintaining high coverage (>90.0\%) and enabling adaptive abstention (13.5\%\to63.4\%\pm0.5) as environmental severity increases. For LiDAR-based perception, our approach demonstrates particularly strong performance, maintaining robust coverage (>84.5\%) while appropriately abstaining from unreliable predictions. Notably, the framework shows remarkable stability under heavy perturbations, with detection performance (AUC: 0.995\pm0.001) significantly outperforming existing methods across all modalities. Our unified approach bridges the gap between theoretical guarantees and practical deployment needs, offering a robust solution for safety-critical autonomous systems operating in challenging real-world conditions.

Search is All You Need for Few-shot Anomaly Detection

Few-shot anomaly detection (FSAD) has emerged as a crucial yet challenging task in industrial inspection, where normal distribution modeling must be accomplished with only a few normal images. While existing approaches typically employ multi-modal foundation models combining language and vision modalities for prompt-guided anomaly detection, these methods often demand sophisticated prompt engineering and extensive manual tuning. In this paper, we demonstrate that a straightforward nearest-neighbor search framework can surpass state-of-the-art performance in both single-class and multi-class FSAD scenarios. Our proposed method, VisionAD, consists of four simple yet essential components: (1) scalable vision foundation models that extract universal and discriminative features; (2) dual augmentation strategies - support augmentation to enhance feature matching adaptability and query augmentation to address the oversights of single-view prediction; (3) multi-layer feature integration that captures both low-frequency global context and high-frequency local details with minimal computational overhead; and (4) a class-aware visual memory bank enabling efficient one-for-all multi-class detection. Extensive evaluations across MVTec-AD, VisA, and Real-IAD benchmarks demonstrate VisionAD's exceptional performance. Using only 1 normal images as support, our method achieves remarkable image-level AUROC scores of 97.4%, 94.8%, and 70.8% respectively, outperforming current state-of-the-art approaches by significant margins (+1.6%, +3.2%, and +1.4%). The training-free nature and superior few-shot capabilities of VisionAD make it particularly appealing for real-world applications where samples are scarce or expensive to obtain. Simulating the Unseen: Crash Prediction Must Learn from What Did Not Happen

Traffic safety science has long been hindered by a fundamental data paradox: the crashes we most wish to prevent are precisely those events we rarely observe. Existing crash-frequency models and surrogate safety metrics rely heavily on sparse, noisy, and under-reported records, while even sophisticated, high-fidelity simulations undersample the long-tailed situations that trigger catastrophic outcomes such as fatalities. We argue that the path to achieving Vision Zero, i.e., the complete elimination of traffic fatalities and severe injuries, requires a paradigm shift from traditional crash-only learning to a new form of counterfactual safety learning: reasoning not only about what happened, but also about the vast set of plausible yet perilous scenarios that could have happened under slightly different circumstances. To operationalize this shift, our proposed agenda bridges macro to micro. Guided by crash-rate priors, generative scene engines, diverse driver models, and causal learning, near-miss events are synthesized and explained. A crash-focused digital twin testbed links micro scenes to macro patterns, while a multi-objective validator ensures that simulations maintain statistical realism. This pipeline transforms sparse crash data into rich signals for crash prediction, enabling the stress-testing of vehicles, roads, and policies before deployment. By learning from crashes that almost happened, we can shift traffic safety from reactive forensics to proactive prevention, advancing Vision Zero.

SafeScientist: Toward Risk-Aware Scientific Discoveries by LLM Agents

Recent advancements in large language model (LLM) agents have significantly accelerated scientific discovery automation, yet concurrently raised critical ethical and safety concerns. To systematically address these challenges, we introduce SafeScientist, an innovative AI scientist framework explicitly designed to enhance safety and ethical responsibility in AI-driven scientific exploration. SafeScientist proactively refuses ethically inappropriate or high-risk tasks and rigorously emphasizes safety throughout the research process. To achieve comprehensive safety oversight, we integrate multiple defensive mechanisms, including prompt monitoring, agent-collaboration monitoring, tool-use monitoring, and an ethical reviewer component. Complementing SafeScientist, we propose SciSafetyBench, a novel benchmark specifically designed to evaluate AI safety in scientific contexts, comprising 240 high-risk scientific tasks across 6 domains, alongside 30 specially designed scientific tools and 120 tool-related risk tasks. Extensive experiments demonstrate that SafeScientist significantly improves safety performance by 35\% compared to traditional AI scientist frameworks, without compromising scientific output quality. Additionally, we rigorously validate the robustness of our safety pipeline against diverse adversarial attack methods, further confirming the effectiveness of our integrated approach. Model Context Protocol for Vision Systems: Audit, Security, and Protocol Extensions

The Model Context Protocol (MCP) defines a schema bound execution model for agent-tool interaction, enabling modular computer vision workflows without retraining. To our knowledge, this is the first protocol level, deployment scale audit of MCP in vision systems, identifying systemic weaknesses in schema semantics, interoperability, and runtime coordination. We analyze 91 publicly registered vision centric MCP servers, annotated along nine dimensions of compositional fidelity, and develop an executable benchmark with validators to detect and categorize protocol violations. The audit reveals high prevalence of schema format divergence, missing runtime schema validation, undeclared coordinate conventions, and reliance on untracked bridging scripts. Validator based testing quantifies these failures, with schema format checks flagging misalignments in 78.0 percent of systems, coordinate convention checks detecting spatial reference errors in 24.6 percent, and memory scope checks issuing an average of 33.8 warnings per 100 executions. Security probes show that dynamic and multi agent workflows exhibit elevated risks of privilege escalation and untyped tool connections. The proposed benchmark and validator suite, implemented in a controlled testbed and to be released on GitHub, establishes a reproducible framework for measuring and improving the reliability and security of compositional vision workflows.

ForesightSafety Bench: A Frontier Risk Evaluation and Governance Framework towards Safe AI

Rapidly evolving AI exhibits increasingly strong autonomy and goal-directed capabilities, accompanied by derivative systemic risks that are more unpredictable, difficult to control, and potentially irreversible. However, current AI safety evaluation systems suffer from critical limitations such as restricted risk dimensions and failed frontier risk detection. The lagging safety benchmarks and alignment technologies can hardly address the complex challenges posed by cutting-edge AI models. To bridge this gap, we propose the "ForesightSafety Bench" AI Safety Evaluation Framework, beginning with 7 major Fundamental Safety pillars and progressively extends to advanced Embodied AI Safety, AI4Science Safety, Social and Environmental AI risks, Catastrophic and Existential Risks, as well as 8 critical industrial safety domains, forming a total of 94 refined risk dimensions. To date, the benchmark has accumulated tens of thousands of structured risk data points and assessment results, establishing a widely encompassing, hierarchically clear, and dynamically evolving AI safety evaluation framework. Based on this benchmark, we conduct systematic evaluation and in-depth analysis of over twenty mainstream advanced large models, identifying key risk patterns and their capability boundaries. The safety capability evaluation results reveals the widespread safety vulnerabilities of frontier AI across multiple pillars, particularly focusing on Risky Agentic Autonomy, AI4Science Safety, Embodied AI Safety, Social AI Safety and Catastrophic and Existential Risks. AccidentBench: Benchmarking Multimodal Understanding and Reasoning in Vehicle Accidents and Beyond

Rapid advances in multimodal models demand benchmarks that rigorously evaluate understanding and reasoning in safety-critical, dynamic real-world settings. We present AccidentBench, a large-scale benchmark that combines vehicle accident scenarios with Beyond domains, safety-critical settings in air and water that emphasize spatial and temporal reasoning (e.g., navigation, orientation, multi-vehicle motion). The benchmark contains approximately 2000 videos and over 19000 human-annotated question--answer pairs spanning multiple video lengths (short/medium/long) and difficulty levels (easy/medium/hard). Tasks systematically probe core capabilities: temporal, spatial, and intent understanding and reasoning. By unifying accident-centric traffic scenes with broader safety-critical scenarios in air and water, AccidentBench offers a comprehensive, physically grounded testbed for evaluating models under real-world variability. Evaluations of state-of-the-art models (e.g., Gemini-2.5 Pro and GPT-5) show that even the strongest models achieve only about 18% accuracy on the hardest tasks and longest videos, revealing substantial gaps in real-world temporal, spatial, and intent reasoning. AccidentBench is designed to expose these critical gaps and drive the development of multimodal models that are safer, more robust, and better aligned with real-world safety-critical challenges. MultiPriv: Benchmarking Individual-Level Privacy Reasoning in Vision-Language Models

Modern Vision-Language Models (VLMs) demonstrate sophisticated reasoning, escalating privacy risks beyond simple attribute perception to individual-level linkage. Current privacy benchmarks are structurally insufficient for this new threat, as they primarily evaluate privacy perception while failing to address the more critical risk of privacy reasoning: a VLM's ability to infer and link distributed information to construct individual profiles. To address this critical gap, we propose MultiPriv, the first benchmark designed to systematically evaluate individual-level privacy reasoning in VLMs. We introduce the Privacy Perception and Reasoning (PPR) framework and construct a novel, bilingual multimodal dataset to support it. The dataset uniquely features a core component of synthetic individual profiles where identifiers (e.g., faces, names) are meticulously linked to sensitive attributes. This design enables nine challenging tasks evaluating the full PPR spectrum, from attribute detection to cross-image re-identification and chained inference. We conduct a large-scale evaluation of over 50 foundational and commercial VLMs. Our analysis reveals: (1) Many VLMs possess significant, unmeasured reasoning-based privacy risks. (2) Perception-level metrics are poor predictors of these reasoning risks, revealing a critical evaluation gap. (3) Existing safety alignments are inconsistent and ineffective against such reasoning-based attacks. MultiPriv exposes systemic vulnerabilities and provides the necessary framework for developing robust, privacy-preserving VLMs.

Adaptive Autonomy in Human-on-the-Loop Vision-Based Robotics Systems

Computer vision approaches are widely used by autonomous robotic systems to sense the world around them and to guide their decision making as they perform diverse tasks such as collision avoidance, search and rescue, and object manipulation. High accuracy is critical, particularly for Human-on-the-loop (HoTL) systems where decisions are made autonomously by the system, and humans play only a supervisory role. Failures of the vision model can lead to erroneous decisions with potentially life or death consequences. In this paper, we propose a solution based upon adaptive autonomy levels, whereby the system detects loss of reliability of these models and responds by temporarily lowering its own autonomy levels and increasing engagement of the human in the decision-making process. Our solution is applicable for vision-based tasks in which humans have time to react and provide guidance. When implemented, our approach would estimate the reliability of the vision task by considering uncertainty in its model, and by performing covariate analysis to determine when the current operating environment is ill-matched to the model's training data. We provide examples from DroneResponse, in which small Unmanned Aerial Systems are deployed for Emergency Response missions, and show how the vision model's reliability would be used in addition to confidence scores to drive and specify the behavior and adaptation of the system's autonomy. This workshop paper outlines our proposed approach and describes open challenges at the intersection of Computer Vision and Software Engineering for the safe and reliable deployment of vision models in the decision making of autonomous systems.

SafeWatch: An Efficient Safety-Policy Following Video Guardrail Model with Transparent Explanations

With the rise of generative AI and rapid growth of high-quality video generation, video guardrails have become more crucial than ever to ensure safety and security across platforms. Current video guardrails, however, are either overly simplistic, relying on pure classification models trained on simple policies with limited unsafe categories, which lack detailed explanations, or prompting multimodal large language models (MLLMs) with long safety guidelines, which are inefficient and impractical for guardrailing real-world content. To bridge this gap, we propose SafeWatch, an efficient MLLM-based video guardrail model designed to follow customized safety policies and provide multi-label video guardrail outputs with content-specific explanations in a zero-shot manner. In particular, unlike traditional MLLM-based guardrails that encode all safety policies autoregressively, causing inefficiency and bias, SafeWatch uniquely encodes each policy chunk in parallel and eliminates their position bias such that all policies are attended simultaneously with equal importance. In addition, to improve efficiency and accuracy, SafeWatch incorporates a policy-aware visual token pruning algorithm that adaptively selects the most relevant video tokens for each policy, discarding noisy or irrelevant information. This allows for more focused, policy-compliant guardrail with significantly reduced computational overhead. Considering the limitations of existing video guardrail benchmarks, we propose SafeWatch-Bench, a large-scale video guardrail benchmark comprising over 2M videos spanning six safety categories which covers over 30 tasks to ensure a comprehensive coverage of all potential safety scenarios. SafeWatch outperforms SOTA by 28.2% on SafeWatch-Bench, 13.6% on benchmarks, cuts costs by 10%, and delivers top-tier explanations validated by LLM and human reviews.

Omni-SafetyBench: A Benchmark for Safety Evaluation of Audio-Visual Large Language Models

The rise of Omni-modal Large Language Models (OLLMs), which integrate visual and auditory processing with text, necessitates robust safety evaluations to mitigate harmful outputs. However, no dedicated benchmarks currently exist for OLLMs, and prior benchmarks designed for other LLMs lack the ability to assess safety performance under audio-visual joint inputs or cross-modal safety consistency. To fill this gap, we introduce Omni-SafetyBench, the first comprehensive parallel benchmark for OLLM safety evaluation, featuring 24 modality combinations and variations with 972 samples each, including dedicated audio-visual harm cases. Considering OLLMs' comprehension challenges with complex omni-modal inputs and the need for cross-modal consistency evaluation, we propose tailored metrics: a Safety-score based on conditional Attack Success Rate (C-ASR) and Refusal Rate (C-RR) to account for comprehension failures, and a Cross-Modal Safety Consistency Score (CMSC-score) to measure consistency across modalities. Evaluating 6 open-source and 4 closed-source OLLMs reveals critical vulnerabilities: (1) no model excels in both overall safety and consistency, with only 3 models achieving over 0.6 in both metrics and top performer scoring around 0.8; (2) safety defenses weaken with complex inputs, especially audio-visual joints; (3) severe weaknesses persist, with some models scoring as low as 0.14 on specific modalities. Our benchmark and metrics highlight urgent needs for enhanced OLLM safety, providing a foundation for future improvements.

Enhancing Safety and Robustness of Vision-Based Controllers via Reachability Analysis

Autonomous systems, such as self-driving cars and drones, have made significant strides in recent years by leveraging visual inputs and machine learning for decision-making and control. Despite their impressive performance, these vision-based controllers can make erroneous predictions when faced with novel or out-of-distribution inputs. Such errors can cascade into catastrophic system failures and compromise system safety. In this work, we compute Neural Reachable Tubes, which act as parameterized approximations of Backward Reachable Tubes to stress-test the vision-based controllers and mine their failure modes. The identified failures are then used to enhance the system safety through both offline and online methods. The online approach involves training a classifier as a run-time failure monitor to detect closed-loop, system-level failures, subsequently triggering a fallback controller that robustly handles these detected failures to preserve system safety. For the offline approach, we improve the original controller via incremental training using a carefully augmented failure dataset, resulting in a more robust controller that is resistant to the known failure modes. In either approach, the system is safeguarded against shortcomings that transcend the vision-based controller and pertain to the closed-loop safety of the overall system. We validate the proposed approaches on an autonomous aircraft taxiing task that involves using a vision-based controller to guide the aircraft towards the centerline of the runway. Our results show the efficacy of the proposed algorithms in identifying and handling system-level failures, outperforming methods that rely on controller prediction error or uncertainty quantification for identifying system failures.

LabSafety Bench: Benchmarking LLMs on Safety Issues in Scientific Labs

Laboratory accidents pose significant risks to human life and property, underscoring the importance of robust safety protocols. Despite advancements in safety training, laboratory personnel may still unknowingly engage in unsafe practices. With the increasing reliance on large language models (LLMs) for guidance in various fields, including laboratory settings, there is a growing concern about their reliability in critical safety-related decision-making. Unlike trained human researchers, LLMs lack formal lab safety education, raising questions about their ability to provide safe and accurate guidance. Existing research on LLM trustworthiness primarily focuses on issues such as ethical compliance, truthfulness, and fairness but fails to fully cover safety-critical real-world applications, like lab safety. To address this gap, we propose the Laboratory Safety Benchmark (LabSafety Bench), a comprehensive evaluation framework based on a new taxonomy aligned with Occupational Safety and Health Administration (OSHA) protocols. This benchmark includes 765 multiple-choice questions verified by human experts, assessing LLMs and vision language models (VLMs) performance in lab safety contexts. Our evaluations demonstrate that while GPT-4o outperforms human participants, it is still prone to critical errors, highlighting the risks of relying on LLMs in safety-critical environments. Our findings emphasize the need for specialized benchmarks to accurately assess the trustworthiness of LLMs in real-world safety applications.

Benchmarking Robustness of AI-Enabled Multi-sensor Fusion Systems: Challenges and Opportunities

Multi-Sensor Fusion (MSF) based perception systems have been the foundation in supporting many industrial applications and domains, such as self-driving cars, robotic arms, and unmanned aerial vehicles. Over the past few years, the fast progress in data-driven artificial intelligence (AI) has brought a fast-increasing trend to empower MSF systems by deep learning techniques to further improve performance, especially on intelligent systems and their perception systems. Although quite a few AI-enabled MSF perception systems and techniques have been proposed, up to the present, limited benchmarks that focus on MSF perception are publicly available. Given that many intelligent systems such as self-driving cars are operated in safety-critical contexts where perception systems play an important role, there comes an urgent need for a more in-depth understanding of the performance and reliability of these MSF systems. To bridge this gap, we initiate an early step in this direction and construct a public benchmark of AI-enabled MSF-based perception systems including three commonly adopted tasks (i.e., object detection, object tracking, and depth completion). Based on this, to comprehensively understand MSF systems' robustness and reliability, we design 14 common and realistic corruption patterns to synthesize large-scale corrupted datasets. We further perform a systematic evaluation of these systems through our large-scale evaluation. Our results reveal the vulnerability of the current AI-enabled MSF perception systems, calling for researchers and practitioners to take robustness and reliability into account when designing AI-enabled MSF.

DeepKnown-Guard: A Proprietary Model-Based Safety Response Framework for AI Agents

With the widespread application of Large Language Models (LLMs), their associated security issues have become increasingly prominent, severely constraining their trustworthy deployment in critical domains. This paper proposes a novel safety response framework designed to systematically safeguard LLMs at both the input and output levels. At the input level, the framework employs a supervised fine-tuning-based safety classification model. Through a fine-grained four-tier taxonomy (Safe, Unsafe, Conditionally Safe, Focused Attention), it performs precise risk identification and differentiated handling of user queries, significantly enhancing risk coverage and business scenario adaptability, and achieving a risk recall rate of 99.3%. At the output level, the framework integrates Retrieval-Augmented Generation (RAG) with a specifically fine-tuned interpretation model, ensuring all responses are grounded in a real-time, trustworthy knowledge base. This approach eliminates information fabrication and enables result traceability. Experimental results demonstrate that our proposed safety control model achieves a significantly higher safety score on public safety evaluation benchmarks compared to the baseline model, TinyR1-Safety-8B. Furthermore, on our proprietary high-risk test set, the framework's components attained a perfect 100% safety score, validating their exceptional protective capabilities in complex risk scenarios. This research provides an effective engineering pathway for building high-security, high-trust LLM applications.

ARMs: Adaptive Red-Teaming Agent against Multimodal Models with Plug-and-Play Attacks

As vision-language models (VLMs) gain prominence, their multimodal interfaces also introduce new safety vulnerabilities, making the safety evaluation challenging and critical. Existing red-teaming efforts are either restricted to a narrow set of adversarial patterns or depend heavily on manual engineering, lacking scalable exploration of emerging real-world VLM vulnerabilities. To bridge this gap, we propose ARMs, an adaptive red-teaming agent that systematically conducts comprehensive risk assessments for VLMs. Given a target harmful behavior or risk definition, ARMs automatically optimizes diverse red-teaming strategies with reasoning-enhanced multi-step orchestration, to effectively elicit harmful outputs from target VLMs. We propose 11 novel multimodal attack strategies, covering diverse adversarial patterns of VLMs (e.g., reasoning hijacking, contextual cloaking), and integrate 17 red-teaming algorithms into ARMs via model context protocol (MCP). To balance the diversity and effectiveness of the attack, we design a layered memory with an epsilon-greedy attack exploration algorithm. Extensive experiments on instance- and policy-based benchmarks show that ARMs achieves SOTA attack success rates, exceeding baselines by an average of 52.1% and surpassing 90% on Claude-4-Sonnet. We show that the diversity of red-teaming instances generated by ARMs is significantly higher, revealing emerging vulnerabilities in VLMs. Leveraging ARMs, we construct ARMs-Bench, a large-scale multimodal safety dataset comprising over 30K red-teaming instances spanning 51 diverse risk categories, grounded in both real-world multimodal threats and regulatory risks. Safety fine-tuning with ARMs-Bench substantially improves the robustness of VLMs while preserving their general utility, providing actionable guidance to improve multimodal safety alignment against emerging threats.

Safety at Scale: A Comprehensive Survey of Large Model Safety

The rapid advancement of large models, driven by their exceptional abilities in learning and generalization through large-scale pre-training, has reshaped the landscape of Artificial Intelligence (AI). These models are now foundational to a wide range of applications, including conversational AI, recommendation systems, autonomous driving, content generation, medical diagnostics, and scientific discovery. However, their widespread deployment also exposes them to significant safety risks, raising concerns about robustness, reliability, and ethical implications. This survey provides a systematic review of current safety research on large models, covering Vision Foundation Models (VFMs), Large Language Models (LLMs), Vision-Language Pre-training (VLP) models, Vision-Language Models (VLMs), Diffusion Models (DMs), and large-model-based Agents. Our contributions are summarized as follows: (1) We present a comprehensive taxonomy of safety threats to these models, including adversarial attacks, data poisoning, backdoor attacks, jailbreak and prompt injection attacks, energy-latency attacks, data and model extraction attacks, and emerging agent-specific threats. (2) We review defense strategies proposed for each type of attacks if available and summarize the commonly used datasets and benchmarks for safety research. (3) Building on this, we identify and discuss the open challenges in large model safety, emphasizing the need for comprehensive safety evaluations, scalable and effective defense mechanisms, and sustainable data practices. More importantly, we highlight the necessity of collective efforts from the research community and international collaboration. Our work can serve as a useful reference for researchers and practitioners, fostering the ongoing development of comprehensive defense systems and platforms to safeguard AI models.

Building Safe and Reliable AI systems for Safety Critical Tasks with Vision-Language Processing

Although AI systems have been applied in various fields and achieved impressive performance, their safety and reliability are still a big concern. This is especially important for safety-critical tasks. One shared characteristic of these critical tasks is their risk sensitivity, where small mistakes can cause big consequences and even endanger life. There are several factors that could be guidelines for the successful deployment of AI systems in sensitive tasks: (i) failure detection and out-of-distribution (OOD) detection; (ii) overfitting identification; (iii) uncertainty quantification for predictions; (iv) robustness to data perturbations. These factors are also challenges of current AI systems, which are major blocks for building safe and reliable AI. Specifically, the current AI algorithms are unable to identify common causes for failure detection. Furthermore, additional techniques are required to quantify the quality of predictions. All these contribute to inaccurate uncertainty quantification, which lowers trust in predictions. Hence obtaining accurate model uncertainty quantification and its further improvement are challenging. To address these issues, many techniques have been proposed, such as regularization methods and learning strategies. As vision and language are the most typical data type and have many open source benchmark datasets, this thesis will focus on vision-language data processing for tasks like classification, image captioning, and vision question answering. In this thesis, we aim to build a safeguard by further developing current techniques to ensure the accurate model uncertainty for safety-critical tasks.

INTACT: Inducing Noise Tolerance through Adversarial Curriculum Training for LiDAR-based Safety-Critical Perception and Autonomy

In this work, we present INTACT, a novel two-phase framework designed to enhance the robustness of deep neural networks (DNNs) against noisy LiDAR data in safety-critical perception tasks. INTACT combines meta-learning with adversarial curriculum training (ACT) to systematically address challenges posed by data corruption and sparsity in 3D point clouds. The meta-learning phase equips a teacher network with task-agnostic priors, enabling it to generate robust saliency maps that identify critical data regions. The ACT phase leverages these saliency maps to progressively expose a student network to increasingly complex noise patterns, ensuring targeted perturbation and improved noise resilience. INTACT's effectiveness is demonstrated through comprehensive evaluations on object detection, tracking, and classification benchmarks using diverse datasets, including KITTI, Argoverse, and ModelNet40. Results indicate that INTACT improves model robustness by up to 20% across all tasks, outperforming standard adversarial and curriculum training methods. This framework not only addresses the limitations of conventional training strategies but also offers a scalable and efficient solution for real-world deployment in resource-constrained safety-critical systems. INTACT's principled integration of meta-learning and adversarial training establishes a new paradigm for noise-tolerant 3D perception in safety-critical applications. INTACT improved KITTI Multiple Object Tracking Accuracy (MOTA) by 9.6% (64.1% -> 75.1%) and by 12.4% under Gaussian noise (52.5% -> 73.7%). Similarly, KITTI mean Average Precision (mAP) rose from 59.8% to 69.8% (50% point drop) and 49.3% to 70.9% (Gaussian noise), highlighting the framework's ability to enhance deep learning model resilience in safety-critical object tracking scenarios.

CoCA: Regaining Safety-awareness of Multimodal Large Language Models with Constitutional Calibration

The deployment of multimodal large language models (MLLMs) has demonstrated remarkable success in engaging in conversations involving visual inputs, thanks to the superior power of large language models (LLMs). Those MLLMs are typically built based on the LLMs, with an image encoder to process images into the token embedding space of the LLMs. However, the integration of visual modality has introduced a unique vulnerability: the MLLM becomes susceptible to malicious visual inputs and prone to generating sensitive or harmful responses, even though the LLM has been trained on textual dataset to align with human value. In this paper, we first raise the question: ``Do the MLLMs possess safety-awareness against malicious image inputs?". We find that after adding a principle that specifies the safety requirement into the input of the MLLM, the model's safety awareness becomes boosted. This phenomenon verifies the existence of MLLM's safety-awareness against image inputs, it is only weakened by the modality gap. We then introduce a simple yet effective technique termed CoCA, which amplifies the safety-awareness of the MLLM by calibrating its output distribution. Our proposed strategy helps the model reclaim its original safety awareness without losing its original capabilities. We verify the effectiveness of our approach on both multimodal safety and understanding benchmarks.

On the Role of Attention Heads in Large Language Model Safety

Large language models (LLMs) achieve state-of-the-art performance on multiple language tasks, yet their safety guardrails can be circumvented, leading to harmful generations. In light of this, recent research on safety mechanisms has emerged, revealing that when safety representations or component are suppressed, the safety capability of LLMs are compromised. However, existing research tends to overlook the safety impact of multi-head attention mechanisms, despite their crucial role in various model functionalities. Hence, in this paper, we aim to explore the connection between standard attention mechanisms and safety capability to fill this gap in the safety-related mechanistic interpretability. We propose a novel metric which tailored for multi-head attention, the Safety Head ImPortant Score (Ships), to assess the individual heads' contributions to model safety. Based on this, we generalize Ships to the dataset level and further introduce the Safety Attention Head AttRibution Algorithm (Sahara) to attribute the critical safety attention heads inside the model. Our findings show that the special attention head has a significant impact on safety. Ablating a single safety head allows aligned model (e.g., Llama-2-7b-chat) to respond to 16 times more harmful queries, while only modifying 0.006% of the parameters, in contrast to the ~ 5% modification required in previous studies. More importantly, we demonstrate that attention heads primarily function as feature extractors for safety and models fine-tuned from the same base model exhibit overlapping safety heads through comprehensive experiments. Together, our attribution approach and findings provide a novel perspective for unpacking the black box of safety mechanisms within large models.

CRASH: Crash Recognition and Anticipation System Harnessing with Context-Aware and Temporal Focus Attentions

Accurately and promptly predicting accidents among surrounding traffic agents from camera footage is crucial for the safety of autonomous vehicles (AVs). This task presents substantial challenges stemming from the unpredictable nature of traffic accidents, their long-tail distribution, the intricacies of traffic scene dynamics, and the inherently constrained field of vision of onboard cameras. To address these challenges, this study introduces a novel accident anticipation framework for AVs, termed CRASH. It seamlessly integrates five components: object detector, feature extractor, object-aware module, context-aware module, and multi-layer fusion. Specifically, we develop the object-aware module to prioritize high-risk objects in complex and ambiguous environments by calculating the spatial-temporal relationships between traffic agents. In parallel, the context-aware is also devised to extend global visual information from the temporal to the frequency domain using the Fast Fourier Transform (FFT) and capture fine-grained visual features of potential objects and broader context cues within traffic scenes. To capture a wider range of visual cues, we further propose a multi-layer fusion that dynamically computes the temporal dependencies between different scenes and iteratively updates the correlations between different visual features for accurate and timely accident prediction. Evaluated on real-world datasets--Dashcam Accident Dataset (DAD), Car Crash Dataset (CCD), and AnAn Accident Detection (A3D) datasets--our model surpasses existing top baselines in critical evaluation metrics like Average Precision (AP) and mean Time-To-Accident (mTTA). Importantly, its robustness and adaptability are particularly evident in challenging driving scenarios with missing or limited training data, demonstrating significant potential for application in real-world autonomous driving systems.

Are Vision LLMs Road-Ready? A Comprehensive Benchmark for Safety-Critical Driving Video Understanding

Vision Large Language Models (VLLMs) have demonstrated impressive capabilities in general visual tasks such as image captioning and visual question answering. However, their effectiveness in specialized, safety-critical domains like autonomous driving remains largely unexplored. Autonomous driving systems require sophisticated scene understanding in complex environments, yet existing multimodal benchmarks primarily focus on normal driving conditions, failing to adequately assess VLLMs' performance in safety-critical scenarios. To address this, we introduce DVBench, a pioneering benchmark designed to evaluate the performance of VLLMs in understanding safety-critical driving videos. Built around a hierarchical ability taxonomy that aligns with widely adopted frameworks for describing driving scenarios used in assessing highly automated driving systems, DVBench features 10,000 multiple-choice questions with human-annotated ground-truth answers, enabling a comprehensive evaluation of VLLMs' capabilities in perception and reasoning. Experiments on 14 SOTA VLLMs, ranging from 0.5B to 72B parameters, reveal significant performance gaps, with no model achieving over 40% accuracy, highlighting critical limitations in understanding complex driving scenarios. To probe adaptability, we fine-tuned selected models using domain-specific data from DVBench, achieving accuracy gains ranging from 5.24 to 10.94 percentage points, with relative improvements of up to 43.59%. This improvement underscores the necessity of targeted adaptation to bridge the gap between general-purpose VLLMs and mission-critical driving applications. DVBench establishes an essential evaluation framework and research roadmap for developing VLLMs that meet the safety and robustness requirements for real-world autonomous systems. Visual Persuasion: What Influences Decisions of Vision-Language Models?

The web is littered with images, once created for human consumption and now increasingly interpreted by agents using vision-language models (VLMs). These agents make visual decisions at scale, deciding what to click, recommend, or buy. Yet, we know little about the structure of their visual preferences. We introduce a framework for studying this by placing VLMs in controlled image-based choice tasks and systematically perturbing their inputs. Our key idea is to treat the agent's decision function as a latent visual utility that can be inferred through revealed preference: choices between systematically edited images. Starting from common images, such as product photos, we propose methods for visual prompt optimization, adapting text optimization methods to iteratively propose and apply visually plausible modifications using an image generation model (such as in composition, lighting, or background). We then evaluate which edits increase selection probability. Through large-scale experiments on frontier VLMs, we demonstrate that optimized edits significantly shift choice probabilities in head-to-head comparisons. We develop an automatic interpretability pipeline to explain these preferences, identifying consistent visual themes that drive selection. We argue that this approach offers a practical and efficient way to surface visual vulnerabilities, safety concerns that might otherwise be discovered implicitly in the wild, supporting more proactive auditing and governance of image-based AI agents.

Perceptual-Evidence Anchored Reinforced Learning for Multimodal Reasoning

Reinforcement Learning with Verifiable Rewards (RLVR) has significantly advanced the reasoning capabilities of Large Language Models (LLMs) and is now being applied to Vision-Language Models (VLMs). However, vanilla RLVR for VLMs verifies only the final textual output, critically neglecting the foundational step of visual perception. This oversight leads to visual hallucinations and reward hacking, as reasoning built upon flawed perception is inherently unreliable. To address this, we propose PEARL (Perceptual-Evidence Anchored Reinforced Learning), a dual-branch, perception-reasoning synergistic that strengthens multimodal reasoning by explicitly anchoring it to verified visual evidence. For each reasoning-oriented QA instance, PEARL first derive a perception checklist -- a set of perception-oriented sub-questions with verifiable answers that probe the model's understanding of key visual evidence. During training, auxiliary rollouts on this checklist yield a perceptual reward that both directly reinforces the model's perception ability and acts as a fidelity gate for reasoning. If the model passes the perception check, its policy update is biased towards evidence-anchored reasoning. Otherwise, the process is halted to prevent reasoning from flawed premises. PEARL can be seamlessly integrated with popular RL methods like GRPO and DAPO. Comprehensive experiments show PEARL achieves substantial gains on multimodal reasoning benchmarks, e.g., a +9.7% improvement over the baseline and +6.6% over GRPO on MathVerse.

Perception-Aware Policy Optimization for Multimodal Reasoning

Reinforcement Learning with Verifiable Rewards (RLVR) has proven to be a highly effective strategy for endowing Large Language Models (LLMs) with robust multi-step reasoning abilities. However, its design and optimizations remain tailored to purely textual domains, resulting in suboptimal performance when applied to multimodal reasoning tasks. In particular, we observe that a major source of error in current multimodal reasoning lies in the perception of visual inputs. To address this bottleneck, we propose Perception-Aware Policy Optimization (PAPO), a simple yet effective extension of GRPO that encourages the model to learn to perceive while learning to reason, entirely from internal supervision signals. Notably, PAPO does not rely on additional data curation, external reward models, or proprietary models. Specifically, we introduce the Implicit Perception Loss in the form of a KL divergence term to the GRPO objective, which, despite its simplicity, yields significant overall improvements (4.4%) on diverse multimodal benchmarks. The improvements are more pronounced, approaching 8.0%, on tasks with high vision dependency. We also observe a substantial reduction (30.5%) in perception errors, indicating improved perceptual capabilities with PAPO. We conduct comprehensive analysis of PAPO and identify a unique loss hacking issue, which we rigorously analyze and mitigate through a Double Entropy Loss. Overall, our work introduces a deeper integration of perception-aware supervision into RLVR learning objectives and lays the groundwork for a new RL framework that encourages visually grounded reasoning. ORacle: Large Vision-Language Models for Knowledge-Guided Holistic OR Domain Modeling

Every day, countless surgeries are performed worldwide, each within the distinct settings of operating rooms (ORs) that vary not only in their setups but also in the personnel, tools, and equipment used. This inherent diversity poses a substantial challenge for achieving a holistic understanding of the OR, as it requires models to generalize beyond their initial training datasets. To reduce this gap, we introduce ORacle, an advanced vision-language model designed for holistic OR domain modeling, which incorporates multi-view and temporal capabilities and can leverage external knowledge during inference, enabling it to adapt to previously unseen surgical scenarios. This capability is further enhanced by our novel data augmentation framework, which significantly diversifies the training dataset, ensuring ORacle's proficiency in applying the provided knowledge effectively. In rigorous testing, in scene graph generation, and downstream tasks on the 4D-OR dataset, ORacle not only demonstrates state-of-the-art performance but does so requiring less data than existing models. Furthermore, its adaptability is displayed through its ability to interpret unseen views, actions, and appearances of tools and equipment. This demonstrates ORacle's potential to significantly enhance the scalability and affordability of OR domain modeling and opens a pathway for future advancements in surgical data science. Multi-Faceted Attack: Exposing Cross-Model Vulnerabilities in Defense-Equipped Vision-Language Models

The growing misuse of Vision-Language Models (VLMs) has led providers to deploy multiple safeguards, including alignment tuning, system prompts, and content moderation. However, the real-world robustness of these defenses against adversarial attacks remains underexplored. We introduce Multi-Faceted Attack (MFA), a framework that systematically exposes general safety vulnerabilities in leading defense-equipped VLMs such as GPT-4o, Gemini-Pro, and Llama-4. The core component of MFA is the Attention-Transfer Attack (ATA), which hides harmful instructions inside a meta task with competing objectives. We provide a theoretical perspective based on reward hacking to explain why this attack succeeds. To improve cross-model transferability, we further introduce a lightweight transfer-enhancement algorithm combined with a simple repetition strategy that jointly bypasses both input-level and output-level filters without model-specific fine-tuning. Empirically, we show that adversarial images optimized for one vision encoder transfer broadly to unseen VLMs, indicating that shared visual representations create a cross-model safety vulnerability. Overall, MFA achieves a 58.5% success rate and consistently outperforms existing methods. On state-of-the-art commercial models, MFA reaches a 52.8% success rate, surpassing the second-best attack by 34%. Protect: Towards Robust Guardrailing Stack for Trustworthy Enterprise LLM Systems

The increasing deployment of Large Language Models (LLMs) across enterprise and mission-critical domains has underscored the urgent need for robust guardrailing systems that ensure safety, reliability, and compliance. Existing solutions often struggle with real-time oversight, multi-modal data handling, and explainability -- limitations that hinder their adoption in regulated environments. Existing guardrails largely operate in isolation, focused on text alone making them inadequate for multi-modal, production-scale environments. We introduce Protect, natively multi-modal guardrailing model designed to operate seamlessly across text, image, and audio inputs, designed for enterprise-grade deployment. Protect integrates fine-tuned, category-specific adapters trained via Low-Rank Adaptation (LoRA) on an extensive, multi-modal dataset covering four safety dimensions: toxicity, sexism, data privacy, and prompt injection. Our teacher-assisted annotation pipeline leverages reasoning and explanation traces to generate high-fidelity, context-aware labels across modalities. Experimental results demonstrate state-of-the-art performance across all safety dimensions, surpassing existing open and proprietary models such as WildGuard, LlamaGuard-4, and GPT-4.1. Protect establishes a strong foundation for trustworthy, auditable, and production-ready safety systems capable of operating across text, image, and audio modalities.

Multimodal Situational Safety

Multimodal Large Language Models (MLLMs) are rapidly evolving, demonstrating impressive capabilities as multimodal assistants that interact with both humans and their environments. However, this increased sophistication introduces significant safety concerns. In this paper, we present the first evaluation and analysis of a novel safety challenge termed Multimodal Situational Safety, which explores how safety considerations vary based on the specific situation in which the user or agent is engaged. We argue that for an MLLM to respond safely, whether through language or action, it often needs to assess the safety implications of a language query within its corresponding visual context. To evaluate this capability, we develop the Multimodal Situational Safety benchmark (MSSBench) to assess the situational safety performance of current MLLMs. The dataset comprises 1,820 language query-image pairs, half of which the image context is safe, and the other half is unsafe. We also develop an evaluation framework that analyzes key safety aspects, including explicit safety reasoning, visual understanding, and, crucially, situational safety reasoning. Our findings reveal that current MLLMs struggle with this nuanced safety problem in the instruction-following setting and struggle to tackle these situational safety challenges all at once, highlighting a key area for future research. Furthermore, we develop multi-agent pipelines to coordinately solve safety challenges, which shows consistent improvement in safety over the original MLLM response. VLSBench: Unveiling Visual Leakage in Multimodal Safety

Safety concerns of Multimodal large language models (MLLMs) have gradually become an important problem in various applications. Surprisingly, previous works indicate a counter-intuitive phenomenon that using textual unlearning to align MLLMs achieves comparable safety performances with MLLMs trained with image-text pairs. To explain such a counter-intuitive phenomenon, we discover a visual safety information leakage (VSIL) problem in existing multimodal safety benchmarks, i.e., the potentially risky and sensitive content in the image has been revealed in the textual query. In this way, MLLMs can easily refuse these sensitive text-image queries according to textual queries. However, image-text pairs without VSIL are common in real-world scenarios and are overlooked by existing multimodal safety benchmarks. To this end, we construct multimodal visual leakless safety benchmark (VLSBench) preventing visual safety leakage from image to textual query with 2.4k image-text pairs. Experimental results indicate that VLSBench poses a significant challenge to both open-source and close-source MLLMs, including LLaVA, Qwen2-VL, Llama3.2-Vision, and GPT-4o. This study demonstrates that textual alignment is enough for multimodal safety scenarios with VSIL, while multimodal alignment is a more promising solution for multimodal safety scenarios without VSIL. SAFE: Multitask Failure Detection for Vision-Language-Action Models

While vision-language-action models (VLAs) have shown promising robotic behaviors across a diverse set of manipulation tasks, they achieve limited success rates when deployed on novel tasks out-of-the-box. To allow these policies to safely interact with their environments, we need a failure detector that gives a timely alert such that the robot can stop, backtrack, or ask for help. However, existing failure detectors are trained and tested only on one or a few specific tasks, while VLAs require the detector to generalize and detect failures also in unseen tasks and novel environments. In this paper, we introduce the multitask failure detection problem and propose SAFE, a failure detector for generalist robot policies such as VLAs. We analyze the VLA feature space and find that VLAs have sufficient high-level knowledge about task success and failure, which is generic across different tasks. Based on this insight, we design SAFE to learn from VLA internal features and predict a single scalar indicating the likelihood of task failure. SAFE is trained on both successful and failed rollouts, and is evaluated on unseen tasks. SAFE is compatible with different policy architectures. We test it on OpenVLA, pi_0, and pi_0-FAST in both simulated and real-world environments extensively. We compare SAFE with diverse baselines and show that SAFE achieves state-of-the-art failure detection performance and the best trade-off between accuracy and detection time using conformal prediction. Safe RLHF-V: Safe Reinforcement Learning from Human Feedback in Multimodal Large Language Models

Multimodal large language models (MLLMs) are critical for developing general-purpose AI assistants, yet they face growing safety risks. How can we ensure that MLLMs are safely aligned to prevent undesired behaviors such as discrimination, misinformation, or violations of ethical standards? In a further step, we need to explore how to fine-tune MLLMs to enhance reasoning performance while ensuring they satisfy safety constraints. Fundamentally, this can be formulated as a min-max optimization problem. In this study, we propose Safe RLHF-V, the first multimodal safety alignment framework that jointly optimizes helpfulness and safety using separate multimodal reward and cost models within a Lagrangian-based constrained optimization framework. Given that there is a lack of preference datasets that separate helpfulness and safety in multimodal scenarios, we introduce BeaverTails-V, the first open-source dataset with dual preference annotations for helpfulness and safety, along with multi-level safety labels (minor, moderate, severe). Additionally, we design a Multi-level Guardrail System to proactively defend against unsafe queries and adversarial attacks. By applying the Beaver-Guard-V moderation for 5 rounds of filtering and re-generation on the precursor model, the overall safety of the upstream model is significantly improved by an average of 40.9%. Experimental results demonstrate that fine-tuning different MLLMs with Safe RLHF can effectively enhance model helpfulness while ensuring improved safety. Specifically, Safe RLHF-V improves model safety by 34.2% and helpfulness by 34.3%. SCI: A Metacognitive Control for Signal Dynamics

Modern deep learning systems are typically deployed as open-loop function approximators: they map inputs to outputs in a single pass, without regulating how much computation or explanatory effort is spent on a given case. In safety-critical settings, this is brittle: easy and ambiguous inputs receive identical processing, and uncertainty is only read off retrospectively from raw probabilities. We introduce the Surgical Cognitive Interpreter (SCI), a lightweight closed-loop metacognitive control layer that wraps an existing stochastic model and turns prediction into an iterative process. SCI monitors a scalar interpretive state SP(t), here instantiated as a normalized entropy-based confidence signal, and adaptively decides whether to stop, continue sampling, or abstain. The goal is not to improve accuracy per se, but to regulate interpretive error ΔSP and expose a safety signal that tracks when the underlying model is likely to fail. We instantiate SCI around Monte Carlo dropout classifiers in three domains: vision (MNIST digits), medical time series (MIT-BIH arrhythmia), and industrial condition monitoring (rolling-element bearings). In all cases, the controller allocates more inference steps to misclassified inputs than to correct ones (up to about 3-4x on MNIST and bearings, and 1.4x on MIT-BIH). The resulting ΔSP acts as a usable safety signal for detecting misclassifications (AUROC 0.63 on MNIST, 0.70 on MIT-BIH, 0.86 on bearings). A safety realignment framework via subspace-oriented model fusion for large language models

The current safeguard mechanisms for large language models (LLMs) are indeed susceptible to jailbreak attacks, making them inherently fragile. Even the process of fine-tuning on apparently benign data for downstream tasks can jeopardize safety. One potential solution is to conduct safety fine-tuning subsequent to downstream fine-tuning. However, there's a risk of catastrophic forgetting during safety fine-tuning, where LLMs may regain safety measures but lose the task-specific knowledge acquired during downstream fine-tuning. In this paper, we introduce a safety realignment framework through subspace-oriented model fusion (SOMF), aiming to combine the safeguard capabilities of initially aligned model and the current fine-tuned model into a realigned model. Our approach begins by disentangling all task vectors from the weights of each fine-tuned model. We then identify safety-related regions within these vectors by subspace masking techniques. Finally, we explore the fusion of the initial safely aligned LLM with all task vectors based on the identified safety subspace. We validate that our safety realignment framework satisfies the safety requirements of a single fine-tuned model as well as multiple models during their fusion. Our findings confirm that SOMF preserves safety without notably compromising performance on downstream tasks, including instruction following in Chinese, English, and Hindi, as well as problem-solving capabilities in Code and Math.

Learning Conformal Abstention Policies for Adaptive Risk Management in Large Language and Vision-Language Models

Large Language and Vision-Language Models (LLMs/VLMs) are increasingly used in safety-critical applications, yet their opaque decision-making complicates risk assessment and reliability. Uncertainty quantification (UQ) helps assess prediction confidence and enables abstention when uncertainty is high. Conformal prediction (CP), a leading UQ method, provides statistical guarantees but relies on static thresholds, which fail to adapt to task complexity and evolving data distributions, leading to suboptimal trade-offs in accuracy, coverage, and informativeness. To address this, we propose learnable conformal abstention, integrating reinforcement learning (RL) with CP to optimize abstention thresholds dynamically. By treating CP thresholds as adaptive actions, our approach balances multiple objectives, minimizing prediction set size while maintaining reliable coverage. Extensive evaluations across diverse LLM/VLM benchmarks show our method outperforms Least Ambiguous Classifiers (LAC) and Adaptive Prediction Sets (APS), improving accuracy by up to 3.2%, boosting AUROC for hallucination detection by 22.19%, enhancing uncertainty-guided selective generation (AUARC) by 21.17%, and reducing calibration error by 70%-85%. These improvements hold across multiple models and datasets while consistently meeting the 90% coverage target, establishing our approach as a more effective and flexible solution for reliable decision-making in safety-critical applications. SynSpill: Improved Industrial Spill Detection With Synthetic Data

Large-scale Vision-Language Models (VLMs) have transformed general-purpose visual recognition through strong zero-shot capabilities. However, their performance degrades significantly in niche, safety-critical domains such as industrial spill detection, where hazardous events are rare, sensitive, and difficult to annotate. This scarcity -- driven by privacy concerns, data sensitivity, and the infrequency of real incidents -- renders conventional fine-tuning of detectors infeasible for most industrial settings. We address this challenge by introducing a scalable framework centered on a high-quality synthetic data generation pipeline. We demonstrate that this synthetic corpus enables effective Parameter-Efficient Fine-Tuning (PEFT) of VLMs and substantially boosts the performance of state-of-the-art object detectors such as YOLO and DETR. Notably, in the absence of synthetic data (SynSpill dataset), VLMs still generalize better to unseen spill scenarios than these detectors. When SynSpill is used, both VLMs and detectors achieve marked improvements, with their performance becoming comparable. Our results underscore that high-fidelity synthetic data is a powerful means to bridge the domain gap in safety-critical applications. The combination of synthetic generation and lightweight adaptation offers a cost-effective, scalable pathway for deploying vision systems in industrial environments where real data is scarce/impractical to obtain. GSPR: Aligning LLM Safeguards as Generalizable Safety Policy Reasoners

As large language models (LLMs) are increasingly integrated into numerous applications across various domains, LLMs' safety becomes a critical concern for both application developers and intended users. Currently, great efforts have been made to develop safety benchmarks with fine-grained taxonomies. However, these benchmarks' taxonomies are disparate with different safety policies. Thus, existing safeguards trained on these benchmarks are either coarse-grained to only distinguish between safe and unsafe, or constrained by the narrow risk taxonomies of a single benchmark. To leverage these fine-grained safety taxonomies across multiple safety benchmarks, in this paper, we propose GSPR, a Generalizable Safety Policy Reasoner to identify unsafe input prompts and LLMs' outputs with violated safety taxonomies through Group Relative Policy Optimization (GRPO). Unlike prior safeguards which only cover a fixed set of risk factors, our GSPR incentivizes its reasoning capability with varied safety taxonomies through our careful cold-start strategy and reward design. Consequently, our GSPR can be trained across multiple safety benchmarks with distinct taxonomies and naturally exhibits powerful generalization ability. We conduct extensive experiments to show that our GSPR significantly improves existing safety guardrails' reasoning capabilities for both safety and category prediction tasks. Moreover, our GSPR not only demonstrates powerful safety generalization abilities but also achieves the least inference token costs with explanations.

Saffron-1: Towards an Inference Scaling Paradigm for LLM Safety Assurance

Existing safety assurance research has primarily focused on training-phase alignment to instill safe behaviors into LLMs. However, recent studies have exposed these methods' susceptibility to diverse jailbreak attacks. Concurrently, inference scaling has significantly advanced LLM reasoning capabilities but remains unexplored in the context of safety assurance. Addressing this gap, our work pioneers inference scaling for robust and effective LLM safety against emerging threats. We reveal that conventional inference scaling techniques, despite their success in reasoning tasks, perform poorly in safety contexts, even falling short of basic approaches like Best-of-N Sampling. We attribute this inefficiency to a newly identified challenge, the exploration--efficiency dilemma, arising from the high computational overhead associated with frequent process reward model (PRM) evaluations. To overcome this dilemma, we propose SAFFRON, a novel inference scaling paradigm tailored explicitly for safety assurance. Central to our approach is the introduction of a multifurcation reward model (MRM) that significantly reduces the required number of reward model evaluations. To operationalize this paradigm, we further propose: (i) a partial supervision training objective for MRM, (ii) a conservative exploration constraint to prevent out-of-distribution explorations, and (iii) a Trie-based key--value caching strategy that facilitates cache sharing across sequences during tree search. Extensive experiments validate the effectiveness of our method. Additionally, we publicly release our trained multifurcation reward model (Saffron-1) and the accompanying token-level safety reward dataset (Safety4M) to accelerate future research in LLM safety. Drive Video Analysis for the Detection of Traffic Near-Miss Incidents

Because of their recent introduction, self-driving cars and advanced driver assistance system (ADAS) equipped vehicles have had little opportunity to learn, the dangerous traffic (including near-miss incident) scenarios that provide normal drivers with strong motivation to drive safely. Accordingly, as a means of providing learning depth, this paper presents a novel traffic database that contains information on a large number of traffic near-miss incidents that were obtained by mounting driving recorders in more than 100 taxis over the course of a decade. The study makes the following two main contributions: (i) In order to assist automated systems in detecting near-miss incidents based on database instances, we created a large-scale traffic near-miss incident database (NIDB) that consists of video clip of dangerous events captured by monocular driving recorders. (ii) To illustrate the applicability of NIDB traffic near-miss incidents, we provide two primary database-related improvements: parameter fine-tuning using various near-miss scenes from NIDB, and foreground/background separation into motion representation. Then, using our new database in conjunction with a monocular driving recorder, we developed a near-miss recognition method that provides automated systems with a performance level that is comparable to a human-level understanding of near-miss incidents (64.5% vs. 68.4% at near-miss recognition, 61.3% vs. 78.7% at near-miss detection).

AutoTrust: Benchmarking Trustworthiness in Large Vision Language Models for Autonomous Driving

Recent advancements in large vision language models (VLMs) tailored for autonomous driving (AD) have shown strong scene understanding and reasoning capabilities, making them undeniable candidates for end-to-end driving systems. However, limited work exists on studying the trustworthiness of DriveVLMs -- a critical factor that directly impacts public transportation safety. In this paper, we introduce AutoTrust, a comprehensive trustworthiness benchmark for large vision-language models in autonomous driving (DriveVLMs), considering diverse perspectives -- including trustfulness, safety, robustness, privacy, and fairness. We constructed the largest visual question-answering dataset for investigating trustworthiness issues in driving scenarios, comprising over 10k unique scenes and 18k queries. We evaluated six publicly available VLMs, spanning from generalist to specialist, from open-source to commercial models. Our exhaustive evaluations have unveiled previously undiscovered vulnerabilities of DriveVLMs to trustworthiness threats. Specifically, we found that the general VLMs like LLaVA-v1.6 and GPT-4o-mini surprisingly outperform specialized models fine-tuned for driving in terms of overall trustworthiness. DriveVLMs like DriveLM-Agent are particularly vulnerable to disclosing sensitive information. Additionally, both generalist and specialist VLMs remain susceptible to adversarial attacks and struggle to ensure unbiased decision-making across diverse environments and populations. Our findings call for immediate and decisive action to address the trustworthiness of DriveVLMs -- an issue of critical importance to public safety and the welfare of all citizens relying on autonomous transportation systems. DREAM: Scalable Red Teaming for Text-to-Image Generative Systems via Distribution Modeling

Despite the integration of safety alignment and external filters, text-to-image (T2I) generative models are still susceptible to producing harmful content, such as sexual or violent imagery. This raises serious concerns about unintended exposure and potential misuse. Red teaming, which aims to proactively identify diverse prompts that can elicit unsafe outputs from the T2I system (including the core generative model as well as potential external safety filters and other processing components), is increasingly recognized as an essential method for assessing and improving safety before real-world deployment. Yet, existing automated red teaming approaches often treat prompt discovery as an isolated, prompt-level optimization task, which limits their scalability, diversity, and overall effectiveness. To bridge this gap, in this paper, we propose DREAM, a scalable red teaming framework to automatically uncover diverse problematic prompts from a given T2I system. Unlike most prior works that optimize prompts individually, DREAM directly models the probabilistic distribution of the target system's problematic prompts, which enables explicit optimization over both effectiveness and diversity, and allows efficient large-scale sampling after training. To achieve this without direct access to representative training samples, we draw inspiration from energy-based models and reformulate the objective into simple and tractable objectives. We further introduce GC-SPSA, an efficient optimization algorithm that provide stable gradient estimates through the long and potentially non-differentiable T2I pipeline. The effectiveness of DREAM is validated through extensive experiments, demonstrating that it surpasses 9 state-of-the-art baselines by a notable margin across a broad range of T2I models and safety filters in terms of prompt success rate and diversity.

MLLM-as-a-Judge for Image Safety without Human Labeling

Image content safety has become a significant challenge with the rise of visual media on online platforms. Meanwhile, in the age of AI-generated content (AIGC), many image generation models are capable of producing harmful content, such as images containing sexual or violent material. Thus, it becomes crucial to identify such unsafe images based on established safety rules. Pre-trained Multimodal Large Language Models (MLLMs) offer potential in this regard, given their strong pattern recognition abilities. Existing approaches typically fine-tune MLLMs with human-labeled datasets, which however brings a series of drawbacks. First, relying on human annotators to label data following intricate and detailed guidelines is both expensive and labor-intensive. Furthermore, users of safety judgment systems may need to frequently update safety rules, making fine-tuning on human-based annotation more challenging. This raises the research question: Can we detect unsafe images by querying MLLMs in a zero-shot setting using a predefined safety constitution (a set of safety rules)? Our research showed that simply querying pre-trained MLLMs does not yield satisfactory results. This lack of effectiveness stems from factors such as the subjectivity of safety rules, the complexity of lengthy constitutions, and the inherent biases in the models. To address these challenges, we propose a MLLM-based method includes objectifying safety rules, assessing the relevance between rules and images, making quick judgments based on debiased token probabilities with logically complete yet simplified precondition chains for safety rules, and conducting more in-depth reasoning with cascaded chain-of-thought processes if necessary. Experiment results demonstrate that our method is highly effective for zero-shot image safety judgment tasks.

SafeAuto: Knowledge-Enhanced Safe Autonomous Driving with Multimodal Foundation Models

Traditional autonomous driving systems often struggle to connect high-level reasoning with low-level control, leading to suboptimal and sometimes unsafe behaviors. Recent advances in multimodal large language models (MLLMs), which process both visual and textual data, offer an opportunity to unify perception and reasoning. However, effectively embedding precise safety knowledge into MLLMs for autonomous driving remains a significant challenge. To address this, we propose SafeAuto, a framework that enhances MLLM-based autonomous driving by incorporating both unstructured and structured knowledge. First, we introduce a Position-Dependent Cross-Entropy (PDCE) loss to improve low-level control signal predictions when values are represented as text. Second, to explicitly integrate safety knowledge, we develop a reasoning component that translates traffic rules into first-order logic (e.g., "red light implies stop") and embeds them into a probabilistic graphical model (e.g., Markov Logic Network) to verify predicted actions using recognized environmental attributes. Additionally, our Multimodal Retrieval-Augmented Generation (RAG) model leverages video, control signals, and environmental attributes to learn from past driving experiences. Integrating PDCE, MLN, and Multimodal RAG, SafeAuto outperforms existing baselines across multiple datasets, enabling more accurate, reliable, and safer autonomous driving. UniSim: A Neural Closed-Loop Sensor Simulator

Rigorously testing autonomy systems is essential for making safe self-driving vehicles (SDV) a reality. It requires one to generate safety critical scenarios beyond what can be collected safely in the world, as many scenarios happen rarely on public roads. To accurately evaluate performance, we need to test the SDV on these scenarios in closed-loop, where the SDV and other actors interact with each other at each timestep. Previously recorded driving logs provide a rich resource to build these new scenarios from, but for closed loop evaluation, we need to modify the sensor data based on the new scene configuration and the SDV's decisions, as actors might be added or removed and the trajectories of existing actors and the SDV will differ from the original log. In this paper, we present UniSim, a neural sensor simulator that takes a single recorded log captured by a sensor-equipped vehicle and converts it into a realistic closed-loop multi-sensor simulation. UniSim builds neural feature grids to reconstruct both the static background and dynamic actors in the scene, and composites them together to simulate LiDAR and camera data at new viewpoints, with actors added or removed and at new placements. To better handle extrapolated views, we incorporate learnable priors for dynamic objects, and leverage a convolutional network to complete unseen regions. Our experiments show UniSim can simulate realistic sensor data with small domain gap on downstream tasks. With UniSim, we demonstrate closed-loop evaluation of an autonomy system on safety-critical scenarios as if it were in the real world.

Holistic Safety and Responsibility Evaluations of Advanced AI Models

Safety and responsibility evaluations of advanced AI models are a critical but developing field of research and practice. In the development of Google DeepMind's advanced AI models, we innovated on and applied a broad set of approaches to safety evaluation. In this report, we summarise and share elements of our evolving approach as well as lessons learned for a broad audience. Key lessons learned include: First, theoretical underpinnings and frameworks are invaluable to organise the breadth of risk domains, modalities, forms, metrics, and goals. Second, theory and practice of safety evaluation development each benefit from collaboration to clarify goals, methods and challenges, and facilitate the transfer of insights between different stakeholders and disciplines. Third, similar key methods, lessons, and institutions apply across the range of concerns in responsibility and safety - including established and emerging harms. For this reason it is important that a wide range of actors working on safety evaluation and safety research communities work together to develop, refine and implement novel evaluation approaches and best practices, rather than operating in silos. The report concludes with outlining the clear need to rapidly advance the science of evaluations, to integrate new evaluations into the development and governance of AI, to establish scientifically-grounded norms and standards, and to promote a robust evaluation ecosystem.

Fool the Hydra: Adversarial Attacks against Multi-view Object Detection Systems

Adversarial patches exemplify the tangible manifestation of the threat posed by adversarial attacks on Machine Learning (ML) models in real-world scenarios. Robustness against these attacks is of the utmost importance when designing computer vision applications, especially for safety-critical domains such as CCTV systems. In most practical situations, monitoring open spaces requires multi-view systems to overcome acquisition challenges such as occlusion handling. Multiview object systems are able to combine data from multiple views, and reach reliable detection results even in difficult environments. Despite its importance in real-world vision applications, the vulnerability of multiview systems to adversarial patches is not sufficiently investigated. In this paper, we raise the following question: Does the increased performance and information sharing across views offer as a by-product robustness to adversarial patches? We first conduct a preliminary analysis showing promising robustness against off-the-shelf adversarial patches, even in an extreme setting where we consider patches applied to all views by all persons in Wildtrack benchmark. However, we challenged this observation by proposing two new attacks: (i) In the first attack, targeting a multiview CNN, we maximize the global loss by proposing gradient projection to the different views and aggregating the obtained local gradients. (ii) In the second attack, we focus on a Transformer-based multiview framework. In addition to the focal loss, we also maximize the transformer-specific loss by dissipating its attention blocks. Our results show a large degradation in the detection performance of victim multiview systems with our first patch attack reaching an attack success rate of 73% , while our second proposed attack reduced the performance of its target detector by 62%

AlphaAlign: Incentivizing Safety Alignment with Extremely Simplified Reinforcement Learning

Large language models (LLMs), despite possessing latent safety understanding from their vast pretraining data, remain vulnerable to generating harmful content and exhibit issues such as over-refusal and utility degradation after safety alignment. Current safety alignment methods often result in superficial refusal shortcuts or rely on intensive supervision for reasoning-based approaches, failing to fully leverage the model's intrinsic safety self-awareness. We propose AlphaAlign, a simple yet effective pure reinforcement learning (RL) framework with verifiable safety reward designed to incentivize this latent safety awareness through proactive safety reasoning.} AlphaAlign employs a dual-reward system: a verifiable safety reward encourages correctly formatted and explicitly justified refusals for harmful queries while penalizing over-refusals, and a normalized helpfulness reward guides high-quality responses to benign inputs. This allows the model to develop proactive safety reasoning capabilities without depending on supervised safety-specific reasoning data. AlphaAlign demonstrates three key advantages: (1) Simplicity and efficiency, requiring only binary prompt safety labels and minimal RL steps for substantial improvements. (2) Breaking the safety-utility trade-off, by enhancing refusal of harmful content and reducing over-refusals, while simultaneously maintaining or even improving general task performance and robustness to unseen jailbreaks. (3) Deep alignment, fostering proactive safety reasoning that generates explicit safety rationales rather than relying on shallow refusal patterns.

Robo3D: Towards Robust and Reliable 3D Perception against Corruptions

The robustness of 3D perception systems under natural corruptions from environments and sensors is pivotal for safety-critical applications. Existing large-scale 3D perception datasets often contain data that are meticulously cleaned. Such configurations, however, cannot reflect the reliability of perception models during the deployment stage. In this work, we present Robo3D, the first comprehensive benchmark heading toward probing the robustness of 3D detectors and segmentors under out-of-distribution scenarios against natural corruptions that occur in real-world environments. Specifically, we consider eight corruption types stemming from adversarial weather conditions, external disturbances, and internal sensor failure. We uncover that, although promising results have been progressively achieved on standard benchmarks, state-of-the-art 3D perception models are at risk of being vulnerable to corruptions. We draw key observations on the use of data representations, augmentation schemes, and training strategies, that could severely affect the model's performance. To pursue better robustness, we propose a density-insensitive training framework along with a simple flexible voxelization strategy to enhance the model resiliency. We hope our benchmark and approach could inspire future research in designing more robust and reliable 3D perception models. Our robustness benchmark suite is publicly available.

Rethinking Autonomy: Preventing Failures in AI-Driven Software Engineering

The integration of Large Language Models (LLMs) into software engineering has revolutionized code generation, enabling unprecedented productivity through promptware and autonomous AI agents. However, this transformation introduces significant risks, including insecure code generation, hallucinated outputs, irreversible actions, and a lack of transparency and accountability. Incidents like the Replit database deletion underscore the urgent need for robust safety and governance mechanisms. This paper comprehensively analyzes the inherent challenges of LLM-assisted code generation, such as vulnerability inheritance, overtrust, misinterpretation, and the absence of standardized validation and rollback protocols. To address these, we propose the SAFE-AI Framework, a holistic approach emphasizing Safety, Auditability, Feedback, and Explainability. The framework integrates guardrails, sandboxing, runtime verification, risk-aware logging, human-in-the-loop systems, and explainable AI techniques to mitigate risks while fostering trust and compliance. We introduce a novel taxonomy of AI behaviors categorizing suggestive, generative, autonomous, and destructive actions to guide risk assessment and oversight. Additionally, we identify open problems, including the lack of standardized benchmarks for code specific hallucinations and autonomy levels, and propose future research directions for hybrid verification, semantic guardrails, and proactive governance tools. Through detailed comparisons of autonomy control, prompt engineering, explainability, and governance frameworks, this paper provides a roadmap for responsible AI integration in software engineering, aligning with emerging regulations like the EU AI Act and Canada's AIDA to ensure safe, transparent, and accountable AI-driven development.

SafePLUG: Empowering Multimodal LLMs with Pixel-Level Insight and Temporal Grounding for Traffic Accident Understanding

Multimodal large language models (MLLMs) have achieved remarkable progress across a range of vision-language tasks and demonstrate strong potential for traffic accident understanding. However, existing MLLMs in this domain primarily focus on coarse-grained image-level or video-level comprehension and often struggle to handle fine-grained visual details or localized scene components, limiting their applicability in complex accident scenarios. To address these limitations, we propose SafePLUG, a novel framework that empowers MLLMs with both Pixel-Level Understanding and temporal Grounding for comprehensive traffic accident analysis. SafePLUG supports both arbitrary-shaped visual prompts for region-aware question answering and pixel-level segmentation based on language instructions, while also enabling the recognition of temporally anchored events in traffic accident scenarios. To advance the development of MLLMs for traffic accident understanding, we curate a new dataset containing multimodal question-answer pairs centered on diverse accident scenarios, with detailed pixel-level annotations and temporal event boundaries. Experimental results show that SafePLUG achieves strong performance on multiple tasks, including region-based question answering, pixel-level segmentation, temporal event localization, and accident event understanding. These capabilities lay a foundation for fine-grained understanding of complex traffic scenes, with the potential to improve driving safety and enhance situational awareness in smart transportation systems. OutSafe-Bench: A Benchmark for Multimodal Offensive Content Detection in Large Language Models

Since Multimodal Large Language Models (MLLMs) are increasingly being integrated into everyday tools and intelligent agents, growing concerns have arisen regarding their possible output of unsafe contents, ranging from toxic language and biased imagery to privacy violations and harmful misinformation. Current safety benchmarks remain highly limited in both modality coverage and performance evaluations, often neglecting the extensive landscape of content safety. In this work, we introduce OutSafe-Bench, the first most comprehensive content safety evaluation test suite designed for the multimodal era. OutSafe-Bench includes a large-scale dataset that spans four modalities, featuring over 18,000 bilingual (Chinese and English) text prompts, 4,500 images, 450 audio clips and 450 videos, all systematically annotated across nine critical content risk categories. In addition to the dataset, we introduce a Multidimensional Cross Risk Score (MCRS), a novel metric designed to model and assess overlapping and correlated content risks across different categories. To ensure fair and robust evaluation, we propose FairScore, an explainable automated multi-reviewer weighted aggregation framework. FairScore selects top-performing models as adaptive juries, thereby mitigating biases from single-model judgments and enhancing overall evaluation reliability. Our evaluation of nine state-of-the-art MLLMs reveals persistent and substantial safety vulnerabilities, underscoring the pressing need for robust safeguards in MLLMs.
