Restore managed Gradio runtime after attestation experiment

Restore the exact original app.py and README from f6ef7f2 and remove only the unused attestation helper/tests added in this wave. Managed Gradio does not expose the exact well-known route safely; document that limitation externally rather than changing SDK behavior.
Merged to preserve the proven managed Gradio runtime contract; source-attestation limitation will be recorded honestly.
