Critical bug fixes: (1) stream selector buttons invisible because div never set to block, (2) duplicate _vtvProSwitch calling non-existent function loadFPTPlayCh(), (3) stream selector not hidden when switching back to VTV, (4) wrong default source changed from iframe-blocked web page to IPTV m3u8 primary source.
