This PR improves the model card by:

1. Linking the model to its associated research paper: The Patient is not a Moving Document: A World Model Training Paradigm for Longitudinal EHR by adding the arXiv ID to the metadata.
2. Correcting the model name in the description and inference code snippet from 1.7B to 8B to match the repository name and backbone.
3. Updating the citation section with the official paper details.
